RESUMO

UNLABELLED: To define the informative value of Doppler studies in the early diagnosis of left ventricular (LV) diastolic dysfunction in patients with hypertensive disease (HD), the authors examined 74 patients with grade 1-2 HD, including 65 men and 9 women aged 43 to 63 years. All the patients underwent echocardiography (echoCG), Doppler echoCG (DechoCG), tissue DechoCG (TDechoCG), and treadmill. According to the echoCG LV mass index (LVMI), all the patients were divided into 2 groups: 1) 33 patients with increased LVMI and 2) 41 with normal LVMI. A control group consisted of 20 apparently healthy patients. Groups 1 and 2 showed a preponderance of patients with concentric LV hypertrophy (CLVH) and those with concentric LV remodeling, respectively. In accordance with DechoCG, the signs of primary LV diastolic dysfunction ((E/A = 0.8+/-0.1; IVRT = 103+/-15) were found only in Group 1 patients. TDechoCG displayed the signs of primary LV diastolic dysfunction in both groups (e' or =10 ratio, suggests elevated LV filling pressure as an early stage of diastolic dysfunction.

RESUMO

The search for reliable Dopplerographic markers of incipient diastolic cardiac failure became especially actual in recent years. We examined 71 patients with II degree arterial hypertension (AH) with Dopplerographical signs of diastolic left ventricular (LV) dysfunction. For the assessment of severity of diastolic dysfunction we carried out complex Dopplerographic evaluation of maximal velocities of transmitral blood flow and motion of fibrous annulus of the mitral valve (MV). We revealed relationships between standard parameters of transmitral flow, Doppler visualization of tissues, and tolerance to physical effort. We found moderate negative correlation between exercise tolerance and maximal velocity of movement of lateral part of MV fibrous annulus E' (r=-0.4, p=<0.001), E/E' ratio (r=-0.65, p=<0.001). E/E' ratio had the strongest linear relationship with exercise tolerance. We showed that combined assessment of velocity of early LV diastolic filling and velocity of MV fibrous annulus motion (E/E') was possible in all patients with preserved LV systolic function. In patients with AH without myocardial hypertrophy with incipient LV diastolic dysfunction increase of E/E' > or = 10 allows to suggest lowering of exercise tolerance and might be indicative of early signs of diastolic heart failure.

RESUMO

The spontaneous motor activity and pose reflexes of male adult rats (Wistar) were observed in the course of high pressure nitrogen compression up to 4,1 MPa. The experiments were carried out under normoxic and hypoxic conditions. Stabile rat motor cortex oxygen tension was recording during the nitrogen compression up to 7,1 MPa under normoxic condition. Sensitivity to nitrogen high pressure to be on the increase under hypoxic conditions. In its turn, resistibility to nitrogen high pressure to be on the decrease under hypoxic conditions (oxygen partial pressure from 0,012 to 0,004 MPa). Quantity of high dencity heat shock proteins (Hsp70) rats motor cortex neurons was 3,44 times higher after course of high pressure nitrogen compression up to 4,1 MPa. For hypoxic exposure (6% O2) the difference was less pronounced - 2,2 times. Data about rat motor cortex neurons Hsp70 concentration under high nitrogen pressure and low oxygen pressure may turn to be a clear base for explanation hypoxic influence on processes of nitrogen narcosis.
