RESUMO

Many caregivers of people with cognitive impairment spend a significant amount of their time helping patients with instrumental daily functions. Distributed caregiving is an innovative model designed to reduce an individual caregiver's time burden and increase the likelihood of continued independent living for the patient. Echo Show and Google Home platforms were used to enable the participation of remote family members in caregiving, specifically the socialization and entertainment of a person with cognitive impairment. Caregiver interviews, review of medical records, and case study analysis were used to measure caregiver burden, after distributing some components of caregiving to distant family members with human-in-the-loop artificial intelligence. This case explores the use of Alexa, Echo Show, and other commercial technologies in the management of a patient with cognitive impairment. The human-in-the-loop system introduced in this case study is a creative, accessible, low-cost, and sustainable way to potentially reduce caregiver burden and improve patient outcomes with targeted intervention. Targeted distributed caregiving reduced time spent in caregiving, reduced caregiver guilt and frustration, improved patient's compliance with requests for behavior changes (e.g., voiding before leaving the house), and improved the relationship between the caregiver and the person with cognitive impairment. This case study demonstrates how distributed caregiving, including human-in-the-loop artificial intelligence, can lead to better use of technology in reducing the social isolation of persons with cognitive impairment and in reducing caregiver burden.

BACKGROUND: Acute decompensated heart failure (HF) is the leading cause for hospital readmission. Large-scale sustainable interventions to reduce readmission rate have not been fully explored or proven effective. OBJECTIVE: We studied the impact of hospice and palliative care service utilization on 30-day all-cause hospital readmissions for patients with HF. METHODS AND RESULTS: Data were retrieved from the Department of Veterans Affairs Corporate Data Warehouse. The study included 238 116 HF admissions with primary diagnosis of HF belonging to 130 812 patients. Among these patients, 2592 had hospice and palliative care utilizations and 68 245 patients did not. Rehospitalization was calculated within 30 days of index hospitalization. Propensity scores were used to match hospice and nonhospice patients on demographics, Charlson comorbidity categories, and 30-day survival. In the matched group, logistic regression was used to estimate effects of hospice on readmission, controlling for any covariates that had failed to balance. The average age of the matched patients was 74 years old, 14% were African American, 75% Caucasian, 2% Asian, and 17% female. After propensity matching, the odds ratio for readmission was 1.29. The 95% confidence interval for the odds was 1.13 to 1.48, suggesting that veterans receiving services have a higher chance of readmission. CONCLUSION: In a large cohort study of older US Veterans, utilization of hospice and palliative care services was associated with a higher 30-day all-cause readmission rate among hospitalized patients with HF. Further prospective studies should be conducted to confirm results and test generalizability outside the Veterans Affairs system of care.

In response to the H1N1 epidemic, we used community health workers to design and implement a randomized controlled trial to test the efficacy of a new automated call-monitoring system for second and third trimester predominantly Medicaid-eligible pregnant women in an urban free standing birth center to promptly detect symptoms of influenza and assure rapid treatment to prevent adverse outcomes from influenza. Daily automated telephone call to second and third trimester pregnant women asking if the woman experienced flu-like symptoms. Calls continued daily until 38 weeks gestation. A community health worker's voice was used for the automated call recording. Positive responses triggered an immediate referral to a nurse-midwife for prompt treatment with anti-viral medication. Fifty pregnant participants were randomized into daily-automated call group (n = 26) or health information group (n = 24). The automated call group participants ranged in age from 14 to 36 (mean = 23.5, SD = 6.3), 84.7 % identified their race/ethnicity as African-American Non-Hispanic, and 80.7 % were Medicaid-Eligible. In the automated call group, 11.5 % chose to be immunized against H1N1. The mean percent of patients reached daily was 45.1 % (SD = 3.2 %) and at least once every 3 days was 65.1 % (SD = 3.1 %). One pregnant woman in the automated call group contracted H1N1 influenza and received prompt anti-viral treatment without any serious outcomes. Participation in daily-automated telephone calls did not differ significantly between patients younger than 18 years old versus patients 18 years or older. There was also no difference in participation between patients with parity of 0 versus patients with parity ≥1. Participation in daily telephone calls significantly (alpha ≤0.05) increased when a community health worker provided personal follow-up of non-responsive participants. 93.3 % of surveyed pregnant women, who received automated daily calls, recommended to use a similar daily call system in response to a future health crisis. Automated daily phone calls, designed and produced by community health workers, is a feasible, well received strategy to provide urgent health information to an urban, Medicaid-eligible group of pregnant women, regardless of age or parity.

BACKGROUND: Currently, up to 25% of patients with spinal cord injuries may experience neurologic deterioration during the initial management of their injuries. Therefore, more effective procedures need to be established for the transportation and care of these to reduce the risk of secondary neurologic damage. Here, we present more acceptable methods to minimize motion in the unstable spine during the management of patients with traumatic spine injuries. METHODS: This review summarizes more than a decade of research aimed at evaluating different methods of caring for patients with spine trauma. RESULTS: The most commonly utilized technique to transport spinal cord injured patients, the log rolling maneuver, produced more motion than placing a patient on a spine board, removing a spine board, performing continuous lateral therapy, and positioning a patient prone for surgery. Alternative maneuvers that produced less motion included the straddle lift and slide, 6 + lift and slide, scoop stretcher, mechanical kinetic therapy, mechanical transfers, and the use of the operating table to rotate the patient to the prone position for surgical stabilization. CONCLUSIONS: The log roll maneuver should be removed from the trauma response guidelines for patients with suspected spine injuries, as it creates significantly more motion in the unstable spine than the readily available alternatives. The only exception is the patient who is found prone, in which case the patient should then be log rolled directly on to the spine board utilizing a push technique.
