RESUMO

Textile and cosmetic industries generate large amounts of dye effluents requiring treatment before discharge. This wastewater contains high levels of reactive dyes, low to none-biodegradable materials and chemical residues. Technically, dye wastewater is characterised by high chemical and biological oxygen demand. Biological, physical and pressure-driven membrane processes have been extensively used in textile wastewater treatment plants. However, these technologies are characterised by process complexity and are often costly. Also, process efficiency is not achieved in cost-effective biochemical and physical treatment processes. Membrane distillation (MD) emerged as a promising technology harnessing challenges faced by pressure-driven membrane processes. To ensure high cost-effectiveness, the MD can be operated by solar energy or low-grade waste heat. Herein, the MD purification of dye wastewater is comprehensively and yet concisely discussed. This involved research advancement in MD processes towards removal of dyes from industrial effluents. Also, challenges faced by this process with a specific focus on fouling are reviewed. Current literature mainly tested MD setups in the laboratory scale suggesting a deep need of further optimization of membrane and module designs in near future, especially for textile wastewater treatment. There is a need to deliver customized high-porosity hydrophobic membrane design with the appropriate thickness and module configuration to reduce concentration and temperature polarization (CP and TP). Also, energy loss should be minimized while increasing dye rejection and permeate flux. Although laboratory experiments remain pivotal in optimizing the MD process for treating dye wastewater, the nature of their time intensity poses a challenge. Given the multitude of parameters involved in MD process optimization, artificial intelligence (AI) methodologies present a promising avenue for assistance. Thus, AI-driven algorithms have the potential to enhance overall process efficiency, cutting down on time, fine-tuning parameters, and driving cost reductions. However, achieving an optimal balance between efficiency enhancements and financial outlays is a complex process. Finally, this paper suggests a research direction for the development of effective synthetic and natural dye removal from industrially discharged wastewater.

Diabetes mellitus type 2 is characterized by insulin resistance, which can be combined with relatively decrease secretion of insulin hormone in the body. Metformin is usually recommended as a first-line treatment for diabetes mellitus-type 2, as it has a significant role in decrease mortality. This study aims to evaluate the B12 levels in Iraqi patients with type 2 diabetes, who were using the metformin drug in short-, medium- and long-term periods. The study included 202 patients, who were classified into four groups, according to their use of the Metformin drug as a hypoglycemic agent, while the fourth group did not use metformin and was termed as 'the standard group'. The results showed that the levels of vitamin B12 in patients using Metformin drugs in short-, medium- and long-term periods were, (444.4±17.21)pg/ml, (403.0±20.34)pg/ml and (261.7±14.70)pg/ml, respectively, compared to the standard group (469.7±26.37)pg/ml. The conclusion of this study was that a significant reduction was seen in the level of vitamin B12 in patients using the Metformin drug for medium- and long-term periods compared to the standard group, who did not use the Metformin drug.

In this study, hydrophobic functionalized carbon nanotubes (fCNTs) and silica nanoparticles (fSiO2NPs) were incorporated into polyvinylidene fluoride (PVDF) flat-sheet membranes to improve their performance in membrane distillation (MD). The performance of the as-synthesized membranes was evaluated against commercial reference polytetrafluoroethylene (PTFE) flat-sheet membranes. The water contact angle (WCA) and liquid entry pressure (LEP) of the PVDF membrane were compromised after incorporation of hydrophilic pore forming polyvinylpyrrolidone (PVP). These parameters were key in ensuring high salt rejections in MD processes. Upon incorporation of fCNTS and fSiO2NPs, WCA and LEP improved to 103.61° and 590 kPa, respectively. Moreover, the NP additives enhanced membrane surface roughness. Thus, an increase in membrane roughness improved WCA and resistance to membrane wetting. High salt rejection (>99%) and stable fluxes (39.77 kg m-2 h-1) were recorded throughout a 3 h process evaluation where 3.5 wt% NaCl solution was used as feed. These findings were recorded at feed temperature of 60 ℃. Evidently, this study substantiated the necessity of high feed temperatures towards high rates of water recovery.

Produced water (PW) is, by volume, the largest waste product of the oil- and gas-exploration industry and contains pollutants such as hydrocarbons and heavy metals. To meet the stringent environmental regulations, PW must be treated before discharging into the environment. The current study proposes a novel treatment method where PW is used to prepare oil-in-water emulsion with potential applications within the oil-exploration industry. The emulsions are prepared by applying hollow fiber membrane emulsification (ME) on PW, which inherently contains oil, as to-be-dispersed phase. The results demonstrate that the average droplet size of the emulsions is a function of pressure applied on to-be-dispersed phase and could be customized from 0.24 to 0.65 µm by varying the pressure from 0.25 to 1 bar, respectively. Stability of the emulsions was verified under high pressure and a temperature and storage period of more than 24 h. The calculations showed that an ME unit with <100 kg weight and <1 m3 volume is appropriate to transform the daily average volume of PW from the Danish part of the North Sea into the emulsions. The study provides a novel route, which also complies well with the requirements (low-weight and small spatial footprints) of the offshore oil rigs, to treat and reuse PW within the oil production process and, therefore, eliminates its environmental footprint.

BACKGROUND: Younger and older adults attending the Emergency Department (ED) are a heterogeneous population. Longer length of ED stay is associated with adverse outcomes and may vary by age. AIMS: To evaluate the associations between age and (1) clinical characteristics and (2) length of ED stay among adults attending ED. METHODS: The NOttingham Cohort study in the Emergency Department (NOCED)-a retrospective cohort study-comprises new consecutive ED attendances by adults ≥ 18 years, at a secondary/tertiary care hospital, in 2019. Length of ED stay was dichotomised as < 4 and ≥ 4 h. The associations between age and length of ED stay were analysed by binary logistic regression and adjusted for socio-demographic and clinical factors including triage acuity. RESULTS: 146,636 attendances were analysed; 75,636 (51.6%) resulted in a length of ED stay ≥ 4 h. Attendances of adults aged 65 to 74 years, 75 to 84 years and ≥ 85 years, respectively, had an increased risk (odds ratio (95% confidence interval) of length of ED stay ≥ 4 h of 1.52 (1.45-1.58), 1.65 (1.58-1.72), and 1.84 (1.75-1.93), compared to those of adults 18 to 64 years (all p < 0.001). These findings remained consistent in the subsets of attendances leading to hospital admission and those leading to discharge from ED. DISCUSSION AND CONCLUSION: In this real-world cohort study, older adults were more likely to have a length of ED stay ≥ 4 h, with the oldest old having the highest risk. ED target times should take into account age of attendees.

Computer-aided diagnosis (CAD) systems can be used to process breast ultrasound (BUS) images with the goal of enhancing the capability of diagnosing breast cancer. Many CAD systems operate by analyzing the region-of-interest (ROI) that contains the tumor in the BUS image using conventional texture-based classification models and deep learning-based classification models. Hence, the development of these systems requires automatic methods to localize the ROI that contains the tumor in the BUS image. Deep learning object-detection models can be used to localize the ROI that contains the tumor, but the ROI generated by one model might be better than the ROIs generated by other models. In this study, a new method, called the edge-based selection method, is proposed to analyze the ROIs generated by different deep learning object-detection models with the goal of selecting the ROI that improves the localization of the tumor region. The proposed method employs edge maps computed for BUS images using the recently introduced Dense Extreme Inception Network (DexiNed) deep learning edge-detection model. To the best of our knowledge, our study is the first study that has employed a deep learning edge-detection model to detect the tumor edges in BUS images. The proposed edge-based selection method is applied to analyze the ROIs generated by four deep learning object-detection models. The performance of the proposed edge-based selection method and the four deep learning object-detection models is evaluated using two BUS image datasets. The first dataset, which is used to perform cross-validation evaluation analysis, is a private dataset that includes 380 BUS images. The second dataset, which is used to perform generalization evaluation analysis, is a public dataset that includes 630 BUS images. For both the cross-validation evaluation analysis and the generalization evaluation analysis, the proposed method obtained the overall ROI detection rate, mean precision, mean recall, and mean F1-score values of 98%, 0.91, 0.90, and 0.90, respectively. Moreover, the results show that the proposed edge-based selection method outperformed the four deep learning object-detection models as well as three baseline-combining methods that can be used to combine the ROIs generated by the four deep learning object-detection models. These findings suggest the potential of employing our proposed method to analyze the ROIs generated using different deep learning object-detection models to select the ROI that improves the localization of the tumor region.

Some of the groundwater aquifers in the Puglia Region, Italy, suffer from high salinity and potential micropollutant contamination due to seawater infiltration and chemical discharge. The objective of this study is twofold: to evaluate the performance of the recently reported alumina-doped silica nanofiltration membranes for water potabilization, and to provide a possible solution to improve the groundwater quality in the Puglia Region while maintaining a low energy-footprint. Two lab-made alumina-doped silica membranes with different pore structures, namely S/O = 0.5 and S/O = 2, were tested with real groundwater samples and their performances were compared with those of a commercial polymeric membrane (Dow NF90). Moreover, groundwater samples were sparked with acetamiprid, imidacloprid, and thiacloprid to test the membrane performance in the presence of potential contamination by pesticides. At a trans-membrane pressure of 5 bar, NF90 could reduce the groundwater conductivity from 4.6 to around 1.3 mS·cm-1 and reject 56-85% of the model pesticides, with a permeate flux of 14.2 L·m-2·h-1. The two inorganic membranes S/O = 2 and S/O = 0.5 reduced the permeate conductivity to 3.8 and 2.4 mS·cm-1, respectively. The specific energy consumption for all three membranes was below 0.2 kWh·m-3 which indicates that the potabilization of this groundwater by nanofiltration is commercially feasible.

The anodizing industry generates several alkaline and acidic wastewater streams often with high concentrations of heavy metals. In this study, nanofiltration (NF) was used to treat wastewater from individual baths, i.e., wastewater from color rinse, alkaline pickling rinse, acidic pickling rinse and anodizing rinse, as well as a mixture of all the wastewater streams. The experiments were carried out by using a commercial membrane (NF99HF) exhibiting pure water permeability of 10 L/(m2·h·bar). For all wastewater streams except one, pH was adjusted to bring it within the recommended pH limits of the membrane, whereby part of the heavy metals precipitated and was removed. The NF of the color rinse offered high-quality permeate (heavy metals below detection limit) and high permeability (9 L/(m2·h·bar)), whereas the nanofiltration of the alkaline pickling rinse exhibited no permeability. The NF of the acidic pickling rinse showed a permeability of 3.1-4.1 L/(m2·h·bar), but low ion rejection (7%-13%). NF of the neutralized mixed wastewater, after the removal of precipitate, produced high-quality permeate with a stable permeability of 1 L/(m2·h·bar). Treatment of the mixed wastewater is therefore the best option if the water has to be discharged. If the water has to be reused, the permeate conductivity in the color rinse and anodizing rinse baths have been reduced significantly, so the treatment of these streams may then be a better option.

With the exception of thymectomy, immune modulatory treatment strategies and clinical trials in myasthenia gravis over the past 50 y were mainly borrowed from experience in other nonneurologic autoimmune disorders. The current experimental therapy paradigm has significantly changed such that treatments directed against the pathological mechanisms specific to myasthenia gravis are being tested, in some cases as the initial disease indication. Key advances have been made in three areas: (i) the expanded role and long-term benefits of thymectomy, (ii) complement inhibition to prevent antibody-mediated postsynaptic membrane damage, and (iii) neonatal Fc receptor (FcRn) inhibition as in vivo apheresis, removing pathogenic antibodies. Herein, we discuss these advances and the potential for these newer therapies to significantly influence the current treatment paradigms. While these therapies provide exciting new options with rapid efficacy, there are anticipated challenges to their use, especially in terms of a dramatic increase in cost of care for some patients with myasthenia gravis.

An integrated membrane process for the treatment of wastewaters from a flue gas desulfurization (FGD) plant was implemented on a laboratory scale to reduce their salt content and to produce a water stream to be recycled in the power industry. The process is based on a preliminary pretreatment of FGD wastewaters, which includes chemical softening and ultrafiltration (UF) to remove Ca2+ and Mg2+ ions as well as organic compounds. The pretreated wastewaters were submitted to a reverse osmosis (RO) step to separate salts from water. The RO retentate was finally submitted to a membrane distillation (MD) step to extract more water, thus increasing the total water recovery factor while producing a high-purity permeate stream. The performance of RO and MD membranes was evaluated by calculating salts rejection, permeate flux, fouling index, and water recovery. The investigated integrated system allowed a total recovery factor of about 94% to be reached, with a consequent reduction of the volume of FGD wastewater to be disposed, and an MD permeate stream with an electrical conductivity of 80 µS/cm, able to be reused in the power plant, with a saving in fresh water demand.

BACKGROUND: Ruta graveolens is one of the most used phytomedicines. To date, there is no report of determining the bioactivity of R. graveolens against cariogenic causing bacteria (Streptococcus mutans and Streptococcus sobrinus). OBJECTIVE: The objective of the present study was to determine the antibacterial activity and metabolite profile of R. graveolens against S. mutans and S. sobrinus. MATERIALS AND METHODS: R. graveolens plant material was collected and processed in the month of February. The plant material was extracted by Soxhlet apparatus using methanol solvent. Two strains of S. mutans and two strains of S. sobrinus were isolated from dental caries-active participants and cultured on mitis salivarius-bacitracin agar. The antibacterial susceptibility testing of methanolic extract of R. graveolens was performed by disc diffusion method. The metabolite profile of the plant extract was determined using electrospray ionization-tandem mass spectrometry. RESULTS: The methanolic extract of R. graveolens showed a promising antibacterial activity against S. mutans and S. sobrinus. Two compounds named γ-fagarine and kokusaginine were identified from the methanolic extract of R. graveolens. CONCLUSION: The study concluded that R. graveolens contains significant antibacterial activity. However, further investigations are suggested to understand the anticaries properties of these pure compounds.

BACKGROUND: In the UK, the National Institute for Health and Care Excellence recommends either fracture risk assessment tool (FRAX) or QFracture to estimate the 10 year fracture risk of individuals. However, it is not known how these tools compare in determining risk and subsequent treatment using set intervention thresholds or guidelines. METHODS: The 10 year major osteoporotic (MO) and hip (HI) fracture risks were calculated for 100 women attending osteoporosis clinic in 2010 using FRAX and QFracture, and subsequent agreement to treatment between the tools was looked at using National Osteoporosis Foundation and National Bone Health Alliance thresholds (FRAX-20/3 and QFracture 20/3). We also looked at using these thresholds for QFracture and comparing them with the National Osteoporosis Guideline Group (NOGG) guidelines for FRAX (FRAX-NOGG). RESULTS: The 10 year risk for MO fracture for FRAX was 17.0% (IQR 10.8-24.0) and that of QFracture was 15.8% (IQR 9.5-27.7) (p=0.732). The 10 year risk for HI fracture for FRAX was 5.0% (IQR 2.1-8.9) and that of QFracture was 8.1% (IQR 2.5-21.6) (p<0.001). The agreement between FRAX-20/3 and QFracture-20/3 was greater than the agreement between FRAX-20/3 and FRAX-NOGG or QFracture-20/3 and FRAX-NOGG. CONCLUSIONS: The calculated 10 year risk for MO fracture between FRAX and QFracture was similar, whereas that of HI fracture was significantly different. The agreement to treatment between QFracture-20/3 and FRAX-NOGG was only 45%. Treatment decisions can differ depending on the fracture calculation tool used when coupled with certain intervention thresholds or guidelines.

Produced water represents the largest wastewater stream from oil and gas production. Generally, its high salinity level restricts the treatment options. Membrane crystallization (MCr) is an emerging membrane process with the capability to extract simultaneously fresh water and valuable components from various streams. In the current study, the potential of MCr for produced water treatment and salt recovery was demonstrated. The experiments were carried out in lab scale and semi-pilot scale. The effect of thermal and hydrodynamic conditions on process performance and crystal characteristics were explored. Energy dispersive X-ray (EDX) and X-ray diffraction (XRD) analyses confirmed that the recovered crystals are sodium chloride with very high purity (>99.9%), also indicated by the cubic structure observed by microscopy and SEM (scanning electron microscopy) analysis. It was demonstrated experimentally that at recovery factor of 37%, 16.4 kg NaCl per cubic meter of produced water can be recovered. Anti-scaling surface morphological features of membranes were also identified. In general, the study provides a new perspective of isolation of valuable constituents from produced water that, otherwise, is considered as a nuisance.

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is a rapidly progressive neurodegenerative disease that results in increasing disability and that is uniformly fatal. Since its approval in the 1990s, riluzole remains the sole treatment for ALS offering modest survival benefit. While significant advances have been made in the symptomatic management of the disease, more effective drug therapy targeting disease progression is sorely needed. AREAS COVERED: Advances in the understanding of pathogenic mechanisms involved in disease development and progression have provided multiple avenues for developing effective treatment strategies. This review highlights recent discoveries relating to these diverse mechanisms and their implications for the development of drug therapy. Previous human clinical trials that have targeted these pathways are mentioned and ongoing drug trials are discussed. EXPERT OPINION: The search for effective drug therapy faces important challenges in the areas of basic science and animal research, translation of these results into human clinical trials, inherent bias in human studies and issues related to delays in clinical diagnosis. How these issues may be addressed and why ALS research constitutes fertile grounds for drug development not only for this devastating disease, but also for other more prevalent neurodegenerative diseases, is discussed in this review.
