RESUMO

The detection of vasospasm plays an important role for the definition of the strategy of treatment in patients with subarachnoid hemorrhage (SAH). Besides the invasive contrast angiography, the non-invasive 99m-Tc-HMPAO brain perfusion SPECT and transcranial Doppler sonography (TCD) are recently used for this purpose. For the investigation of the possibilities of the last two methods, comparative studies were performed in 29 SAH patients in the subacute phase of the disease. In the detection of regional brain ischaemia the sensitivity of the SPECT study was 90%, the specificity was 28% and the accuracy was 50%, the corresponding values of TCD were 82%, 71% and 75%. In patients, in whom chronic neurological symptoms, or morphological abnormalities could be excluded, the specificity of the SPECT study was 67% at unchanged sensitivity. Based on the results it is concluded, that the brain perfusion SPECT and the TCD investigation are useful methods for the detection of vasospasm after SAH. The combination of the two methods is recommended for the correct diagnosis of vasospasm in SAH. Further investigations are necessary to clarify the prognostic impact of the result of these investigations.

RESUMO

Bone mineral content (BMC) in the radius of 23 osteogenesis imperfecta (OI) patients (1-26 yr) was measured with SPA technique. Comparing the values of OI type I. patients with that of type III. patients, the decrease of BMC was more prominent in the latter group, however, the two group hadn't been distinct from each other at p < 0.05 level. Based on the measurements of 39 relatives (mother, father, brother) the method seems not to be helpful in screening of gene-carrying relatives because of the high variability of the phenotype of the disease. Follow-up SPA studies-avoiding the repositioning errors-inform about the therapeutic effect.

RESUMO

Bone mineral content (BMC) was measured in 64 children (33 boys [aged 4-17] and 31 girls [aged 2-12]) having no signs of metabolic bone disease, at the distal third of their radius using single photon absorptiometry (SPA), to establish normative data. Relationships were studied between the obtained bone mineral content and the age, weight, as well as height of children. The obtained BMC values were compared with that of healthy Swedish and American children. Authors suggest the routine use of SPA in home children suffering from metabolic bone diseases.

RESUMO

The authors have examined supposed causes of positive Th-201 stress scintigraphy. Patients were divided into two groups according to the presence of exercise-induced transitory hypoperfusion or in absence of it. In both groups negative coronarograms were verified. Results of two groups were compared according to parameters of the left ventricle function, pulmonary pressure, to the presence of disorder of system stimulus-conduction (LBBB, RBBB), mitral prolapsus, foramen ovale apertum, anomalous coronary anatomy, level of load during exercise, body weight, heart volume-index. Significant difference was found in disorder of left ventricle motion, ejection fraction and in comprehensive value of left ventricle function (F1).

A combination of Cisplatin and Epirubicin was administered through the external carotid artery system according to a standard protocol as preoperative or palliative therapy in 14 oral cancer patients. With the aim of clarifying the subacute side-effects of this chemotherapy on the intracerebral circulation, rCBF SPECT investigations with 99mTc-HMPAO were performed before and after the cytostatic treatment. The interval between SPECT studies was 24 (SD +/- 12) days. In concordance with the neurological condition, visual assessment of the rCBF SPECT reconstructed transversal slices revealed no perfusion abnormalities and no changes on the follow-up. No significant changes were found in the side activity ratios in 22 regions of each hemisphere (unpaired t-test) in the whole group of patients. It was concluded that within the investigated time interval this highly effective combination of cytostatics administered perfectly through the external carotid artery system causes no serious disturbances in the cerebral circulation in oral cancer cases.

RESUMO

The value of bone scanning in the primary staging and clinical course of breast cancer was studied in a 10-years follow-up. Bone metastases (BM) were detected at the time of primary diagnosis in 6.8% of the patients. After primary therapy, the skeleton was the first site of the relapse in 42% of the cases. During the follow-up, BM were revealed in 25% of 623 patients a mean of 22.3 months after the operation. BM were manifested in 17% of the cases with small primary (pT1) tumors and negative axillae. Forty-three percent of the patients had no symptoms at the time of BM diagnosis. The mean survival after the appearance of BM in 163 patients was 18.2 months. Thirteen patients survived more than 5 years after the diagnosis of BM. The routine use of bone scanning is suggested at the time of primary diagnosis, and then yearly in a minimum 5-years period, independently of the symptoms.
