RESUMO

The aim and objective of this study is to report the recurrence of the treatment of the anterior open bite after 5 years of treatment. A female patient aged 8 years 6 months old had a negative vertical crossing of -4.7 mm. Early treatment with a fixed palatal grid was performed for 1 year. At the end of the treatment, the vertical overpass was 1.1 mm and after 2 years of control, the vertical overpass remained positive with 1.7 mm showing the stability of the treatment. After 5 years, and due to the sucking and lingual interposition habit, there was a recurrence of the open bite. Interdisciplinary follow-up is extremely important to eliminate the factors responsible for the origin of deleterious habits, thus solving the changes resulting from these habits and providing long-term stability. INTRODUCTION: The influence of aligners on the activity of the masticatory muscles is still controversial, especially regarding the behaviour associated with awake bruxism (AB). OBJECTIVE: To compare the frequency of AB behaviours between patients treated with aligners and fixed appliances. METHODS: The sample comprised 38 Class I patients (mean age 22.08 years), divided by simple randomisation into two groups: OA group; orthodontic aligners (n 19) and FA group; fixed appliance (n 19). The frequency of AB was investigated by the ecological momentary assessment using an online device (mentimeter), during 7 following days at different timepoints, before and after appliance placement and in the 2nd, 3rd, 4th and 6th months of orthodontic treatment. These variables were also evaluated: level of anxiety by the State-Trait Anxiety Inventory, stress by the Perceived Stress Scale, catastrophising related to pain and degree of hypervigilance by the Pain Vigilance and Awareness Questionnaire, and the presence of facial pain evaluated by the DC/TMD. RESULTS: There was no difference between groups in the frequency of AB behaviours, with mean of 53.5% for group OA and 51.3% for FA. The most frequent behaviour was slightly touching the teeth, and in FA group, there was a significant reduction in this behaviour soon after appliance placement. The groups did not differ concerning the degree of anxiety, stress, catastrophising, hypervigilance and facial pain. CONCLUSION: The orthodontic treatment performed with aligners or fixed appliances did not influence the frequency of AB during the 6 months of treatment. ABSTRACT The aim of this study was to evaluate the precision and accuracy of molds made with four commercial brands of alginate scanned at different times using digital model analysis. Eighty molds of a standard Typodont were made using 4 types of alginate (CCC: Cavex Color Change; IA: Identic Alginate; HY5: Hydrogum 5 and JP: Jeltrate Plus). The molds were scanned at four times: immediately (T1), 24h (T2), 72h (T3) and 120h (T4) after molding. Measurements were taken in three dimensions: anteroposterior, transverse and vertical. Significant differences in dimensional changes were noted between the materials over time (p <0.05). Anteroposterior dimensional variation was noted between times, especially for IA and JP. For transverse and vertical variables, a difference was found between the groups, especially at 24 h, 72 h and 120 h. CCC presented significant dimensional shrinkage only at T120 (transverse). IA and JP presented larger dimensional distortions in the vertical measurements. The molding materials used were not dimensionally stable when evaluated after 120 hours of molding. However, such evidence suggests that alginates with longer storage time, such as Cavex Color Change, are more accurate than conventional alginates.


RESUMO O objetivo do presente estudo foi avaliar a precisao e acurácia por meio de análise de modelos digitais de moldes digitalizados obtidos com quatro marcas comerciais de alginato em diferentes tempos. Oitenta moldes de um Typodont padrao foram obtidos utilizando 4 tipos de alginato (CCC: Cavex Color Change; IA: Identic Alginate; HY5: Hydrogum 5 e JP: Jeltrate Plus). Os moldes foram escaneados em quatro tempos: mediatamente (T1), 24h (T2), 72h (T3) e 120h (T4) após a moldagem. As medidas foram feitas em très dimensoes: ântero-posterior, transversal e vertical. Diferenças significativas nas mudanças dimensionais foram observadas entre os materiais ao longo do tempo (p <0,05). Variação dimensional ântero-posterior foi observada entre os tempos, principalmente para IA e JP. Para as variáveis transversal e vertical houve diferenga entre os grupos, principalmente em 24h, 72h e 120h. CCC apresentou contração dimensional significativa apenas em T120 (transversal). IA e JP apresentaram maiores distorções dimensionais na vertical. Os materiais de moldagem utilizados nao foram dimensionalmente estáveis quando avaliados após 120 horas de moldagem. No entanto, tais evidencias sugerem que os alginatos com maior tempo de armazenamento, como Cavex Color Change, sao mais precisos do que os alginatos convencionais.

It is undeniable that extra-alveolar mini-implants anchorage has revolutionized Orthodontics. Correspondingly, the proper understanding of mini-implants biomechanics allowed to broaden the range of dental movements as never seen before in clinical practice. However, in order to produce better treatments, especially regarding the effects in occlusal plane, it is important to be aware of the numerous possibilities of applying force systems based on skeletal anchorage. Thus, this paper aims to address, by means of clinical cases, the application of biomechanics concepts that are extremely relevant to the proper employment of extra-alveolar mini-implants.

RESUMO É inegável que a ancoragem com mini-implantes extra-alveolares revolucionou a nossa especialidade. No mesmo sentido, o correto entendimento da biomecânica com mini-implantes permitiu ampliar a gama de movimentos dentários como nunca visto na prática clínica. Deve-se, porém, atentar às diversas possibilidades dos sistemas de aplicação das forças decorrentes do uso da ancoragem esquelética, com vistas a produzir tratamentos cada vez mais eficientes, principalmente em relação aos efeitos no plano oclusal. Assim, o presente artigo versa, por meio de casos clínicos, sobre a aplicação de conceitos da biomecânica que são extremamente relevantes para o correto emprego dos mini-implantes extra-alveolares.

AIMS: To evaluate changes in transverse dimensions of upper arch and thickness of the buccal bone plate in patients undergoing orthodontic treatment during the leveling phase. MATERIALS AND METHODS: Sixteen patients aged between 11 and 30 years, presenting with class I and II malocclusion, with minimum of 2 mm crowding treated without extraction, were divided into two groups: GI (n = 8), self-ligating brackets (Easyclip Aditek), and GII (n = 8), conventional preadjusted brackets (3M Unitek). The intercanines, interpremolar, and intermolar widths were measured in dental casts using a digital caliper, before (T1) and after 6 months of treatment (T2). Measurements of the thickness of the bone plate were performed by using cone-beam computed tomography (CBCT) scans and Dolphin three-dimensional program. Paired and nonpaired t tests were applied to analyze the results in and between the groups for the changes in the transversal arch dimensions and buccal bone thickness. RESULTS: In all statistical tests, the significance level was set at 5%. A nonsignificant increase in transverse dimensions and a slight reduction on thickness of the buccal bone plate were observed in both groups. CONCLUSION: The dimensional changes of transverse and thickness of the bone plate during the initial phase of treatment were similar, regardless of the bracket design. CLINICAL SIGNIFICANCE: It should be noted that the bracket design does not influence the treatment course regarding transversal dimensions and buccal bone thickness.

The purpose of this retrospective investigation was to evaluate the dentoalveolar and skeletal cephalometric changes of the Bionator appliance on individuals with a Class II division 1 malocclusion. Lateral cephalograms of 44 patients were divided into two equal groups. The control group comprised 22 untreated Class II children (11 males, 11 females), with an initial mean age of 8 years 7 months who were followed without treatment for a period of 13 months. The Bionator group (11 males, 11 females) had an initial mean age of 10 years 8 months, and were treated for a mean period of 16 months. Lateral cephalometric headfilms were obtained of each patient and control at the beginning and end of treatment. The results showed that there were no changes in forward growth of the maxilla in the experimental group compared with the control group. However, the Bionator treatment produced a statistically significant increase in mandibular protrusion, and in total mandibular and body lengths. There were no statistically significant differences in craniofacial growth direction between the Bionator group and the control group, although the treated patients demonstrated a greater increase in posterior face height. The Bionator appliance produced labial tipping of the lower incisors and lingual inclination of the upper incisors, as well as a significant increase (P < 0.01) in mandibular posterior dentoalveolar height. The major effects of the Bionator appliance were dentoalveolar, with a smaller significant skeletal effect. The results indicate that the correction of a Class II division 1 malocclusion with the Bionator appliance is achieved not only by a combination of mandibular skeletal effects, but also by significant dentoalveolar changes.
