BACKGROUND: There are many reasons that could lead to finger joint arthroplasty, and the most familiar reason is osteoarthritis. Silicone finger joint are the most commonly used implants. However, these implants might fracture with time and cause wear which will lead to chronic inflammation and synovitis for the patient and then implant failure. OBJECTIVE: The aim of this study is to improve the design of the silicone finger joint and simulate the different designs using finite element analysis (FEA) simulation. METHOD: Three different designs were drawn and FEA has been used in this study using Solidworks software. The first design is the silicone finger joint design without any modification, the second one is modified design with added ribs to the junction of distal stem and hinge and the third design was added filler material inside the body of the artificial joint. An axial force with 625 N that was applied on the upper part of the distal stem which is nearly represents the maximum value of the grip strength for normal males. RESULTS: The results showed improvement on the design in which the concentrated stress at the junction of the distal stem and hinge of the design was distributed. In addition, the Von Mises stress was stable for the modified design with added ribs and the added filler material designs after 15°. CONCLUSION: The design modification could improve the stress distribution and stability of the artificial finger joint and increase the lifetime expectancy of these implants.

Artificial finger joint design has been developed through different stages through the past. PIP (proximal interphalangeal) and MCP (metacarpophalangeal) artificial finger joints have come to replace the amputation and arthrodesis options; although, these artificial joints are still facing challenges related to reactive tissues, reduced range of motion, and flexion and extension deficits. Swanson silicone artificial finger joints are still common due to the physician's preferability of silicone with the dorsal approach during operation. Nevertheless, other artificial finger joints such as the pyrocarbon implant arthroplasty have also drawn the interests of practitioners. Artificial finger joint has been classified under three major categories which are constrained, unconstrained, and linked design. There are also challenges such as concerns of infections and articular cartilage necrosis associated with attempted retention of vascularity. In addition, one of the main challenges facing the silicone artificial finger joints is the fracture occurring at the distal stem with the hinge. The aim of this paper is to review the different artificial finger joints in one paper as there are few old review papers about them. Further studies need to be done to develop the design and materials of the pyrocarbon and silicone implants to increase the range of motion associated with them and the fatigue life of the silicone implants.

Inert-particles spouted bed reactor (IPSBR) is characterized by intense mixing generated by the circular motion of the inert particles. The operating parameters play an important role in the performance of the IPSBR system, and therefore, parameter optimization is critical for the design and scale-up of this gas-liquid contact system. Computational fluid dynamics (CFD) provides detailed modeling of the system hydrodynamics, enabling the determination of the operating conditions that optimize the performance of this contact system. The present work optimizes the main IPSBR operating parameters, which include a feed-gas velocity in the range 0.5-1.5 m/s, orifice diameter in the range 0.001-0.005 m, gas head in the range 0.15-0.35 m, mixing-particle diameter in the range 0.009-0.0225 m, and mixing-particle to reactor volume fraction in the range 2.0-10.0 vol % (which represents 0.01-0.1 kg of mixing particles loading). The effects of these parameters on the average air velocity and average air volume fraction in the upper, middle, and conical regions of the reactor were studied. The specific distance for each region has been measured from the orifice point to be 50 mm for the conical region, 350 mm for the middle region and 550 mm for the upper rejoin. The selected factors were optimized to obtain the minimum air velocity distribution (maximum gas residence time) and the maximum air volume fraction (maximum interfacial area concentration) because these conditions will increase the gas holdup, the gas-liquid contact area, and the mass transfer coefficient among phases. Response surface methodology (RSM) was used to determine the optimum operating conditions. The regression analysis showed an excellent fit of the experimental data to a second-order polynomial model. The interaction between the process variables was evaluated using the obtained three-dimensional surface plots. The analysis revealed that under the optimized parameters of a feed-gas velocity of 1.5 m/s, orifice diameter of 0.001 m, gas head of 0.164 m, mixing-particle diameter of 0.0225 m, and mixing-particle loading of 0.02 kg, the minimum average air velocity and highest air volume fraction were observed throughout the reactor.

Interleukin (IL)-17 plays a critical role in inflammation. Most studies to date have elucidated the inflammatory role of IL-17A, often referred to as IL-17. IL-17F is a member of the IL-17 family bearing 50% homology to IL-17A and can also be present as heterodimer IL-17AF. This study elucidates the distribution and contribution of IL-17A, F and AF in inflammatory arthritis. Neutralizing antibody to IL-17A alone or IL-17F alone or in combination was utilized in the mouse collagen-induced arthritis (CIA) model to elucidate the contribution of each subtype in mediating inflammation. IL-17A, F and AF were all increased during inflammatory arthritis. Neutralization of IL-17A reduced the severity of arthritis, neutralization of IL-17A+IL-17F had the same effect as neutralizing IL-17A, while neutralization of IL-17F had no effect. Moreover, significantly higher levels of IL-17A and IL-17F were detected in peripheral blood mononuclear cells (PBMC) from patients with rheumatoid arthritis (RA) in comparison to patients with osteoarthritis (OA). IL-17A and AF were detected in synovial fluid mononuclear cells (SFMC) in RA and OA, with IL-17A being significantly higher in RA patients. Enriched CD3(+) T cells from RA PBMCs produced singnificantly high levels of IL-17A and IL-17AF in comparison to OA peripheral blood CD3(+) T cells. IL-17A, F and AF were undetectable in T cells from SFMCs from RA and OA. While IL-17A, F, and AF were all induced during CIA, IL-17A played a dominant role. Furthermore, production of IL-17A, and not IL-17F or IL-17AF, was elevated in PBMCs, SFMCs and enriched peripheral blood CD3(+) T in RA.
