RESUMO

Heterotopic ossification is a benign process of mature laminar bone formation in the soft tissues. A synonymous term used to describe this pathology in muscle is myositis ossificans. The pathogenesis is unclear, but is likely multifactorial. The basic pathology is thought to be ectopic production of osseous tissue as part of a repair process in response to tissue injury. This report describes a case of heterotopic ossification of the quadratus lumborum muscle as an incidental finding. This case highlights that treatment is based on symptoms and conservative management might be appropriate for the asymptomatic patient.

PURPOSE: To compare the complications experienced for 2 different brands of peripherally inserted central venous catheters (PICC), Cook Turbo-Ject and the Bard PowerPICC Solo2. The rationale for this project revolved around concern that one of the PICCS in question had high rates of complications. METHODS: A prospective clinical trial was conducted after obtaining approval from the University of Saskatchewan Human Research Ethics Committee. All PICCs were implanted at the Royal University Hospital Medical Imaging Department by an interventional radiologist. Patient randomization was achieved by alternating the brand of PICC implanted in sequential patients. All the subjects were inpatients from a single surgical ward. Patients were excluded from the study if they had a known uncorrected coagulopathy, or if they were being treated for venous thrombosis. This project was financially supported by the Summer Student Research Fund, College of Medicine, University of Saskatchewan. RESULTS: A total of 53 PICCs (25 Bard and 28 Cook) were inserted over the study period. The mean PICC dwell time was 23.3 days for both the Bard and Cook PICCs, respectively. No statistically significant differences were detected in study group demographics, technical placement of the PICCs, or in the complications encountered. DISCUSSION: Both the Cook Turbo-Ject and the Bard PowerPicc Solo2 PICCs provided acceptable venous access for a wide variety of clinical indications.

BACKGROUND: Bi-directional communication between the follicle and oocyte is necessary to regulate follicle and oocyte development. Currently, it is not practical to monitor the serial growth of individual follicles during assisted reproduction. The ovarian stimulation phase length (SPL) is an indirect measure of mean follicular growth rate. The objective of this study was to test the hypothesis that a short or long SPL would be associated with suboptimal outcomes in women undergoing in vitro fertilization (IVF). MATERIALS AND METHODS: A retrospective cohort study was conducted in 140 women who underwent IVF. Follicle development was monitored every 2-3 days during ovarian stimulation using transvaginal ultrasonography. Once > 3 follicles reached ≥ 17 mm, human chorionic gonadotropin (hCG) was administered. Oocyte retrieval was performed approximately 35 hours after hCG. Oocytes underwent IVF on the day of collection and were evaluated daily thereafter. Embryos were transferred on days 3 or 5, depending on the number and quality of embryos available. Associations between SPL, age, follicle, oocyte, embryo and pregnancy outcomes were evaluated (SPSS version 17.0; SPSS Inc., Chicago, IL, USA). RESULTS: A SPL of 11 days was associated with an optimal number of follicles that developed to ≥ 6 mm, ≥ 10 mm and ≥ 15 mm; serum estradiol concentrations; and number of oocytes collected (p<0.05). Gradual reductions in the number of developing follicles, serum estradiol concentrations and number of oocytes collected occurred with SPL less than or greater than 11 days (p<0.05). The SPL did not influence endometrial, embryo or pregnancy outcomes (p>0.05). Associations between SPL and outcomes were not influenced by age (p>0.05). CONCLUSION: The ovarian SPL can be used to predict the number of follicles that develop, oocytes collected and serum estradiol concentrations, but not embryo or pregnancy outcomes.
