RESUMO

The purpose of this comprehensive literature review is to present the available evidence on the effects of methamphetamine on mental and oral health, as well as provide an overview of the most widely used medical and dental care strategies in the management of meth mouth. For this purpose, PubMed and Google Scholar electronic databases were searched for relevant articles, yielding 115 search results, which were further scrutinized for their relevance, leaving 55 for a detailed review. The analysis of the gathered data indicates that a comprehensive patient-centered approach that takes into consideration the physical, mental, and social aspects is crucial for mitigating the detrimental effects of increasing methamphetamine use.

Ectodermal dysplasia is a genetic disorder characterized by the abnormal development of two or more ectodermally driven structures, leading to various clinical manifestations such as sparse hair, dry skin, and hypodontia or anodontia. The absence of teeth significantly impacts the quality of life for individuals affected by this condition. This article presents a clinical case report of a patient with ectodermal dysplasia who underwent full mouth rehabilitation using computer-aided design/computer-aided manufacturing (CAD/CAM) technology to fabricate a mandibular complete denture and a maxillary overdenture.

The fabrication of periodontal ligament (PDL) models for in vitro dental studies has seen a wide range of techniques and materials being utilized. This paper introduces a novel dental technique that employs a digital workflow for the fabrication of artificial PDL using three-dimensional printing of flexible resin. This innovative approach offers several advantages, including enhanced accuracy and realism in simulating PDL. The digital workflow facilitates a streamlined fabrication process, ensuring efficiency and precision. By presenting this novel technique, this digital approach contributes to the advancement of in vitro dental research, providing researchers with a reliable and realistic model for studying various dental phenomena.

RESUMO

Digital replication of an existing cast gold onlay anatomy to a more esthetic material for a new complete denture is challenging. A technique is presented that uses digital technology to fabricate a new maxillary complete denture with monolithic zirconia onlays that duplicate the anatomy of an existing complete denture with cast gold onlays.

PURPOSE: The effect of a silicone-based sealer on fiber post retention has not been the subject of research and remains unclear. This study aimed to evaluate and compare the impact of a silicone-based sealer and an epoxy resin sealer on fiber post retention. MATERIAL AND METHODS: Forty extracted human mandibular premolars that were caries-free, single-rooted, and possessed straight single canals were the specimens of the present study. Teeth were randomly divided into two groups (n = 20 each). All specimens were prepared using gutta-percha. The sealer used in the first group was Guttaflow 2, while AH26 was used in the second group. Post spaces were prepared immediately after obturation for each specimen. Prefabricated fiber posts were luted in both groups using a self-adhesive resin cement (RelyX-U200). Each specimen was vertically secured using a universal testing machine (Instron Corp.) and a constant pull-out force at a loading rate of 0.5 mm/min was applied until the point of dislodgement. Data were analyzed using t-tests. RESULTS: A significant difference was found between the mean of maximum loads of the silicone-based sealer and epoxy resin sealer groups (p = 0.0001). The mean maximum load for the epoxy resin sealer group was 194.62 N, while that for the silicone-based sealer group was 111.64 N. CONCLUSION: Fiber post retention in teeth obturated using silicone-based sealer was inferior to that in teeth obturated with epoxy resin sealer. Therefore, the sealer used during obturation can significantly affect fiber post retention.

BACKGROUND: Provisional fixed dental prosthesis (FDP) plays an important role during fixed prosthodontic therapy till the definitive. Discoloration of materials used for provisional FDPs can cause patient dissatisfaction and may create doubt about the color stability of the definitive FDP. Surface roughness is the other major property to be taken into consideration for provisional FDP materials. Smokeless tobacco is reported to affect the color stability and surface roughness of different prosthetic materials. OBJECTIVE: The aim of the current study was to evaluate the effect of two types of smokeless tobacco (black and white) on color stability and surface roughness of 3D printed, CAD/CAM milled, and conventional provisional FDP resin materials. METHODS: A total of 144 disc-shaped specimens were fabricated using four techniques CAD/CAM subtractive technique (milling), CAD/CAM additive technique (3D Printing), and conventional technique using autopolymerized PMMA, and autopolymerized Bis-acrylic resins. Each group was subdivided into three subgroups of twelve specimens each, and were submerged into three solutions (artificial salivary substitute, black smokeless tobacco, white smokeless tobacco). The change in color and surface roughness was evaluated and the data collected were statistically analyzed. RESULTS: It was observed that black smokeless tobacco caused the maximum color change and the effect was highest in autopolymerized PMMA resin specimens (ΔE = 9.343 ± 0.489), followed by 3D printed (ΔE = 7.187 ± 0.391), autopolymerized Bis-acryl (ΔE = 6.464 ± 0.453) and milled (ΔE = 4.978 ± 0.227). White smokeless tobacco was found to cause a maximum change in surface roughness and the effect was highest in autopolymerized Bis-acryl specimens (ΔRa = 0.321 ± 0.015 µm), followed by autopolymerized PMMA (ΔRa = 0.297± 0.015 µm), 3D printed (ΔRa = 0.191 ± 0.019 µm), and milled (ΔRa = 0.168 ± 0.014 µm). Statistically significant (p-value < 0.05) differences were observed among all techniques and solutions. CONCLUSIONS: The change in color and surface roughness were maximum in the case of FDPs prepared using autopolymerizing resins, followed by 3D printed, and CAD/CAM milled reins.

RESUMO

Provisional restoration is essential for function, esthetics, occlusion, and tissue health. However, when multiple teeth are involved, making a direct splinted provisional restoration could be challenging due to the combined undercuts between the prepared teeth. This article describes a dental technique using polytetrafluoroethylene (PTFE) tape as a spacer to fabricate a splinted provisional restoration for multiple prepared teeth. The PTFE tape spacer will compensate for discrepancies in the path of insertion between prepared teeth, thus facilitating the removal of the direct splinted provisional restoration intact.

This article summarizes the clinical steps involved in maxillary and mandibular complete-arch implant rehabilitation using a complete digital workflow. The maxillary arch was recorded using the double digital scan technique, and the mandibular arch using the triple digital scan technique. The digital protocol used in this case report allowed the recording of implant positions via scan bodies, soft tissues, and, most importantly, the interocclusal relationship in the same visit. A new technique for mandibular digital scan was described that uses soft tissue landmarks by creating windows in the patient's provisional prostheses to superimpose the three digital scans and consequently fabricate and verify the maxillary and mandibular prototype prostheses and definitive complete-arch zirconia prostheses.

OBJECTIVES: Aramany's classification of postsurgical maxillectomy defects was introduced for partially edentulous situations, and has been widely used for education and effective communication among practitioners. Numerous classifications of maxillofacial defects, based on surgical procedure, resultant defects, or prosthodontist's perspective after rehabilitation, exist in the literature. However, no single classification has incorporated all these factors. The purpose of this review was to highlight the classification systems and describe a pragmatic classification series for edentulous maxillary arch defects (maxillectomy) by applying the Aramany classification criteria, to enhance treatment outcomes and communication among practitioners. MATERIAL AND METHODS: An electronic search of the literature published in English was conducted using the PubMed/MEDLINE and Google Scholar database. Keywords used were "maxillectomy classification" AND "surgical resection," "maxillectomy classification" AND "complete edentulous." In addition, a manual search was also performed followed the same criteria in the following journals: Journal of Prosthetic Dentistry and Journal of Prosthodontics. RESULTS: Several classification systems for partial dentition were found in terms of size, location, dentition, and extension of the defect (isolated or communication defects). The findings revealed a variety of maxillectomy defect classifications for partially dentate, considering surgical factors and rehabilitation. However, no study or classification system exist for the edentulous arch defects. CONCLUSIONS: Different classification systems for maxillectomy defects exist in the literature, only for partially dentate patients. To the authors best knowledge, no classification system for completely edentulous maxillary arch defects have been proposed till date. A simple classification system with clear characteristics for edentulous maxillectomy dental arch defects has been proposed. This classification was modeled after Aramany classification for easier memorization and application.

Background and Objectives: The present systematic review and meta-analysis undertake a comparison of studies that examine the accuracy of robot-assisted dental implant placement in relation to static computer-assisted implant surgery (SCAIS), dynamic computer-assisted implant surgery (DCAIS), and freehand procedures. This study aims to provide a comprehensive understanding of the precision of robot-assisted dental implant placement and its comparative efficacy in relation to other placement techniques. Methods: The guidelines recommended by Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) were used to organize and compose this review. Four electronic databases (PubMed, Web of Science, Scopus, and Cochrane) were systematically searched for pertinent articles. Articles were selected following the inclusion and exclusion criteria. Qualitative and quantitative analyses of the selected articles were performed. Results: The initial electronic search resulted in 1087 hits. Based on the inclusion and exclusion criteria, five articles were selected for qualitative analysis, out of which three were considered for quantitative analysis. Three parameters were considered for accuracy evaluation (angular, coronal, and apical deviation). The mean angular deviation was -1.22 degrees (95% CI, -1.06--1.39), the mean coronal deviation was -0.15 mm (95% CI, -0.24--0.07), and the mean apical deviation was -0.19 mm (95% CI, -0.27--0.10). Conclusions: The robotic implant system was found to have significantly lower angular deviations and insignificantly lower coronal and apical deviations compared to DCAIS. Within the limitations of this review, it can be concluded that robot-assisted implant placement in resin models permits higher accuracy compared to DCAIS and SCAIS systems. However, due to the limited number of comparative studies with high heterogeneity, the findings of this review should be interpreted with caution. Further research is necessary to confirm the clinical application of robotics in implant surgery.

INTRODUCTION: Controlling the cross-contamination between the dental clinic and laboratory is of utmost importance to maintain the health of dental healthcare personnel (DHCP) and patients. The aim of this paper was to review the current literature with regard to the use of chlorhexidine as a prosthetic disinfectant in prosthodontic practice. MATERIALS AND METHODS: A scoping review of the literature was performed in Medline/PubMed, Ovid Embase, and the Cochrane Library. A search for all literature published from 1980 to 2021 was based on the following keywords: ['Chlorhexidine/gluconate' OR 'chlorhexidine', OR 'gluconate', OR 'denture disinfectants', OR 'antimicrobial', OR 'disinfectant', OR 'impression disinfectants, OR prosthesis' OR 'biofilm, microbiology'] OR [teeth]. We reviewed the disinfectant in terms of its mechanism of action, antimicrobial effectiveness, disinfection techniques, clinical applications, corrosiveness/damage to the structure of prostheses, and reasonable shelf life. RESULTS: Chlorhexidine was tested under different concentrations ranging from 0.2 to 5%. It provided a significant reduction in biofilm viability but had a minimum effect on Candida albicans with a variable effect result that showed no significant differences in the dimensional changes by immersion of alginate dental impressions for no more than 10 minutes and no clinically significant dimensional differences on aluwax, polyether, condensation siloxane, and polyvinyl siloxane were noticed. Nonetheless, chlorhexidine altered the surface of the silicone and acrylic resins and affected the long-term hardness of the relining material. CONCLUSION: Within the limitations of this review, the use of chlorhexidine disinfectant demonstrates a good measure in the reduction of contamination and cross-infection and has a minimal effect on the dimensional stability of most impression materials. Further studies with in-vitro testing are required to confirm these findings.

Newly introduced provisional crowns and fixed dental prostheses (FDP) materials should exhibit good physical and mechanical properties necessary to serve the purpose of their fabrication. The aim of this systematic literature review and meta-analysis is to evaluate the articles comparing the physical and mechanical properties of 3D-printed provisional crown and FDP resin materials with CAD/CAM (Computer-Aided Designing/Computer-Aided Manufacturing) milled and conventional provisional resins. Indexed English literature up to April 2022 was systematically searched for articles using the following electronic databases: MEDLINE-PubMed, Web of Science (core collection), Scopus, and the Cochrane library. This systematic review was structured based on the guidelines given by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). The focused PICO/PECO (Participant, Intervention/exposure, Comparison, Outcome) question was: 'Do 3D-printed (P) provisional crowns and FDPs (I) have similar physical and mechanical properties (O) when compared to CAD/CAM milled and other conventionally fabricated ones (C)'. Out of eight hundred and ninety-six titles, which were recognized after a primary search, twenty-five articles were included in the qualitative analysis, and their quality analysis was performed using the modified CONSORT scale. Due to the heterogeneity of the studies, only twelve articles were included for quantitative analysis. Within the limitations of this study, it can be concluded that 3D-printed provisional crown and FDP resin materials have superior mechanical properties but inferior physical properties compared to CAD/CAM milled and other conventionally fabricated ones. Three-dimensionally printed provisional crowns and FDP materials can be used as an alternative to conventional and CAD/CAM milled long-term provisional materials.

RESUMO

This clinical report describes the prosthetic restoration of a failing dentition subsequent to methamphetamine abuse. The treatment involved the use of endosteal dental implants and milled cobalt-chromium, screw-retained, implant-supported fixed dental prostheses. At the 1-year follow-up, the prosthetic rehabilitation had markedly improved the patients' health, esthetics, and function.

RESUMO

This clinical report describes the successful prosthetic rehabilitation of a deficient lower lip in an edentulous patient who had undergone surgery for removal of a squamous cell carcinoma of the anterior floor of the mouth and vestibule. The rehabilitation used a combined approach of an extraoral lip prosthesis joined by 3 magnets to an intraoral implant-retained mandibular resection prosthesis. The outcome demonstrated rehabilitation of the lower third of the face by eliminating loss of fluids and by improving the facial profile, lip contour and competence, esthetics, the patient's eating ability, speech intelligibility, and reported quality of life.

Treating patients with complete edentulism presents unique clinical challenges. With advancements in digital dentistry and computer-engineered complete denture fabrication, new clinical challenges occur. This report describes a modification to a computer-engineered complete denture maxillomandibular relation-recording protocol to manage a patient with an increased interarch distance.

AIM: The aim of this systematic review was to evaluate and synthesise the existing evidence on the effect of the prosthetic implant abutment design and fabrication process on mechanical, biological and aesthetic clinical outcomes. MATERIALS AND METHODS: Two electronic databases (PubMed and Embase) were searched in August 2016 to identify clinical studies evaluating the clinical outcomes of CAD/CAM abutments. The studies were screened and two reviewers used the full text to extract data independently. A qualitative synthesis was performed on the extracted data and summary tables were prepared. Due to heterogeneity in the studies included, no meta-analysis was performed. RESULTS: Twenty-four studies were included in this review. Of these, 13 studies focused solely on CAD/CAM abutment and did not include a control group, or a comparison with conventional implant abutments. Eleven studies compared clinical outcomes of CAD/CAM abutments with conventional abutments. There were only three clinical trials and the majority of the studies were observational or case series studies. The most commonly reported clinical outcomes measured were soft tissue volume and aesthetic scores, survival and success rates, and marginal bone levels. CONCLUSION: The results of the review demonstrate that CAD/CAM abutments had overall good survival and success rates and that they provide comparable, if not better, clinical outcomes when compared with conventional abutments. However, existing evidence is weak as few randomised control trials were conducted and follow-up periods were, in general, short. Conflict-of-interest and funding statement: The authors report no financial or other relationships that might lead to a conflict of interest. This systematic review was conducted as a part of the 2016 Foundation of Oral Rehabilitation Consensus Conference on "Prosthetic Protocols in Implant-based Oral Rehabilitation". The authors received no funding to conduct the systematic review, but received reimbursement to participate in the Consensus Conference.
