Severe penile curvature after implantation of an inflatable penile prosthesis.

We describe 6 cases of penile curvature following implantation of inflatable penile prostheses. Exploration revealed severe scarring of the corpora cavernosa, which produced the curvature in 4 patients. In all 4 cases this area was incised and the defect was filled with a polytetrafluoroethylene vascular graft. The penis was straightened and the prosthesis functioned normally. The remaining 2 patients were treated with inflation for 8 weeks with resolution of the curvature. To our knowledge, these cases represent the first reported complications produced by abnormal scar tissue outside of the inflammatory pseudocapsule after implantation of an inflatable penile prosthesis.

Urinary tract evaluation of diethylstilbestrol-exposed female progeny followed in a colposcopy clinic.

Structural abnormalities of the upper genital tract in diethylstilbestrol-exposed (DES-exposed) female progeny have been noted. Since upper genital tract anomalies are often associated with upper urinary tract anomalies, intravenous pyelography was obtained in 102 DES-exposed patients at our institution to determine the incidence of upper urinary tract anomalies. Only two patients had duplication of the ureter. Since this incidence is not greater than that expected in an unexposed population, intravenous pyelogram studies are not routinely recommended for DES-exposed patients.

Malakoplakia of prostate gland.

Malakoplakia of the prostate gland has rarely been reported. Its presentation often mimics carcinoma of the prostate. We report a case in a sixty-three-year-old patient who had a previous history of other granulomatous disease.

Complete eversion and prolapse of bladder.

Complete eversion and prolapse of the urinary bladder is rare. We report a case occurring in an eighty-two-year-old female. We believe only 3 previous cases have been reported, and only 1 of these occurred in an adult.

Sonographic demonstration of Peyronie plaques.

Patients with Peyronie disease have been evaluated with diagnostic ultrasonography to delineate the plaques and identify the presence of calcification. The advantages of this procedure are discussed. This is the first report of the use of diagnostic ultrasound in the evaluation of Peyronie disease.

Scrotal explorations negative for malignancy.

We reviewed 74 cases of scrotal exploration via an inguinal approach that were negative for malignancy. Of these explorations 24 (32 per cent) resulted in radical orchiectomy and 14 (19 per cent) resulted in simple orchiectomy. An additional 17 patients (23 per cent) had some other operation in addition to exploration. The most frequent final diagnoses were epididymitis (26 per cent), testicular torsion (18 per cent) and hydrocele (15 per cent). These data demonstrate that scrotal explorations to eliminate the possibility of malignancy result in a high orchiectomy rate for benign disease.

Torsion of testicular appendages in men.

Torsion of testicular appendages has never been reported in patients more than 18 years old. We herein review 6 surgically proved cases. The possibility of this condition should be considered in the differential diagnosis of scrotal pain and swelling in men.

Testicular torsion in men.

Testicular torsion in men more than 30 years old is rare. Herein is a critical review of 8 well documented cases. Most cases were misdiagnosed initially, had delay in treatment and required orchiectomy. The diagnosis must be considered at the initial evaluation to achieve testicular preservation.
