RESUMO

Introduction: Patients with optic pathway gliomas (OPG) have good survival rates although their long-term quality of life can be affected by the tumor or treatment-related morbidity. This retrospective study sought to describe the clinical presentation and outcomes of children with OPG at a tertiary center in Mexico. Methods: Consecutive patients <18 years-of-age with newly diagnosed OPG between January 2002 and December 2020 at the Hospital Civil de Guadalajara Dr. Juan I. Menchaca in Guadalajara, Mexico were included. Results: Thirty patients were identified with a median age of six years. The most frequent clinical manifestations were loss of visual acuity (40%) and headaches (23%). Neurofibromatosis-1 was found in 23.3% of the patients. Surgery, either biopsy or resection, was done in 20 of 30 patients. Two patients died shortly after initial surgery. The 5-year event-free survival (EFS) was 79.3% ± 10.8% and the 5-year overall survival was 89.5% ± 6.9%. Lower EFS was associated with age less than 3 years, intracranial hypertension at presentation, and diencephalic syndrome. Patients who received surgery as first-line treatment had a 3.1 times greater risk of achieving a performance score of less than 90 points at 6 months after diagnosis (p=0.006). Of 10 patients with vision testing, 5 had improvement in visual acuity, 4 had no changes, and one patient showed worsening. Conclusion: Our data suggests that favorable outcomes can be achieved with OPG in low- and middle-income countries, although a high rate of surgical complications was described leading to a lower overall survival. These data can be used prospectively to optimize treatment at this institute and other middle-income countries through a comprehensive, multidisciplinary approach.

RESUMO

A year since the declaration of the global coronavirus disease 2019 (COVID-19) pandemic, there were over 110 million cases and 2.5 million deaths. Learning from methods to track community spread of other viruses such as poliovirus, environmental virologists and those in the wastewater-based epidemiology (WBE) field quickly adapted their existing methods to detect SARS-CoV-2 RNA in wastewater. Unlike COVID-19 case and mortality data, there was not a global dashboard to track wastewater monitoring of SARS-CoV-2 RNA worldwide. This study provides a 1-year review of the "COVIDPoops19" global dashboard of universities, sites, and countries monitoring SARS-CoV-2 RNA in wastewater. Methods to assemble the dashboard combined standard literature review, Google Form submissions, and daily, social media keyword searches. Over 200 universities, 1400 sites, and 55 countries with 59 dashboards monitored wastewater for SARS-CoV-2 RNA. However, monitoring was primarily in high-income countries (65%) with less access to this valuable tool in low- and middle-income countries (35%). Data were not widely shared publicly or accessible to researchers to further inform public health actions, perform meta-analysis, better coordinate, and determine equitable distribution of monitoring sites. For WBE to be used to its full potential during COVID-19 and beyond, show us the data.

RESUMO

BACKGROUND: Living donor kidney transplantation is the best type of renal replacement therapy for patients with end-stage renal disease. Living kidney donors (LKDs) undergo an extensive evaluation before donating, and many potential LKDs are declined. This study aimed to define the reasons for the decline in LKD candidates referred to our center. METHODS: We retrospectively analyzed clinical data of all potential LKDs evaluated between January 2001 and December 2021 at our institution,Western National Medical Center, Pediatric Hospital. Data were obtained by review of an electronic database. RESULTS: A total of 1332 potential LKDs were evaluated, 796 (59.7%) successfully donated; 20 (1.5%) had a complete evaluation, were accepted for donation, and were on the waiting list for intervention; 56 (4.2%) continued in the evaluation process; 200 (15%) were discharged from the program due to administrative aspects, death (donor or receptor), or cadaveric renal transplantation in order of frequency; 56 (4.2%) withdraw by personal choice; and 204 (15.3%) were rejected for donation. Donor-related reasons included medical contraindications (n = 134, 65.7%), anatomic contraindications (n = 38, 18.6%), immunologic barriers (n = 18, 8.8%), and psychological reasons (n = 11, 5.4%). CONCLUSIONS: Despite the large number of potential LKDs, a significant proportion did not proceed for donation for different reasons; in our description, it represents 40.3%. The largest proportion is because of donor-related causes, and most of the reasons result from the candidate's unnoticed chronic diseases.

RESUMO

Abstract: Introduction: Depressive symptoms are common in elderly surgical patients and are associated with negative outcomes. Although medication can relieve symptoms, only 50% of patients achieve remission using conventional treatments. Recently, ketamine has been shown to improve depression rapidly. However, its use and tolerability in elderly patients has not been widely studied. Material and methods: We conducted a randomized, double-blind, comparative study in patients aged 60 and over who required ophthalmological surgery. Experimental group received 0.5 mg/kg ketamine, whereas the control group received NaCl solution at 0.9%. Both groups received a standardized regimen of conscious sedation. Depressive symptom severity was assessed using the Geriatric Depression Scale-Short Form (GDS-SF) before and after exposure to ketamine infusion; tolerability was also evaluated. A repeated univariate analysis of variance (ANOVA) model examined the direction of changes in depressive symptom severity among groups. Results: 90 patients were randomized. A significant reduction in symptom severity was observed after ketamine infusion (mean change from baseline: -1.6 vs -0.3 in the control group; p = 0.003). No differences emerged between groups in the presence of adverse effects with ketamine infusion. Conclusions: A single infusion of ketamine in elderly patients undergoing ophthalmological surgery is associated with improved depressive symptoms in the first 24 hours of exposure to the drug with a good tolerability profile.

Abstract: Introduction: Quality of life at work (QOL) or quality of work life (QWL) expresses the satisfaction of workers, in this case nurses, which is directly related to the safety and quality of patient care. Objective: To identify, from the scientific literature, levels of satisfaction with quality of life at work among Latin American nurses, associated risk factors and repercussions on health. Methodology: An integrative review was carried out in PubMed, SciELO, BVS and Google Scholar databases related to studies published in the period January 2011 - August 2021, in English, Portuguese and Spanish. Results: 13 articles were selected that met the inclusion criteria, conducted in health personnel that included nurses. Three categories of analysis were established: satisfaction and dissatisfaction with quality of life at work by levels, dimensions and domains; social and labor factors associated with QOL; and quality of life at work and its impact on health. Conclusion: The review made it possible to identify, even in a limited way, that the quality of life at work of Latin American nurses mostly reached medium to low levels, influenced by sociodemographic and labor factors, and with repercussions on nurses' health. Therefore, it is important to study the subject in depth, considering that dissatisfaction with the quality of life at work of these professionals can have repercussions on their own health as well as on the quality of care.

(OR=11.4, IC95% 3.605- 36.664; p=0.000) y (OR=4.6, IC95% 1.678-12.281; p=0.003), respectivamente.

Indoor navigation systems incorporating augmented reality allow users to locate places within buildings and acquire more knowledge about their environment. However, although diverse works have been introduced with varied technologies, infrastructure, and functionalities, a standardization of the procedures for elaborating these systems has not been reached. Moreover, while systems usually handle contextual information of places in proprietary formats, a platform-independent model is desirable, which would encourage its access, updating, and management. This paper proposes a methodology for developing indoor navigation systems based on the integration of Augmented Reality and Semantic Web technologies to present navigation instructions and contextual information about the environment. It comprises four modules to define a spatial model, data management (supported by an ontology), positioning and navigation, and content visualization. A mobile application system was developed for testing the proposal in academic environments, modeling the structure, routes, and places of two buildings from independent institutions. The experiments cover distinct navigation tasks by participants in both scenarios, recording data such as navigation time, position tracking, system functionality, feedback (answering a survey), and a navigation comparison when the system is not used. The results demonstrate the system's feasibility, where the participants show a positive interest in its functionalities.

Abstract Objective: To analyze the relationship between chronic exposures to air pollution with Covid-19 death rate in Mexican cities. Materials and methods: Ecological study in 25 Mexican cities using the report of daily Covid-19 deaths (from February to June 2020) and validated data of air pollutants, considering average concentrations in each city for the last year. Poisson regression models using generalized additive models with adjustment variables (GAM) were used. Results: A significant increase of 3.5% (95% CI 2.3-4.7) was found in Covid-19 death rate for each 1µg/m3 in annual concentration of NO2. The association with PM2.5 was not significant, with an increase of 1.8% for each 1µg/m3. Conclusions: Results suggest an association between Covid-19 mortality and chronic exposure to NO2. This first approximation of the risk associated with air pollution requires a more precise analysis, but is consistent with what was observed in other studies.

Abstract BACKGROUND: In obstetrics, ultrasonic artifactual mirror images are infrequent, but potentially dangerous since they can be mistakenly interpreted as heterotopic pregnancies, precluding diagnostic errors and iatrogenic interventions. These images require a reflection surface such as dilated bowl or plenty bladder to be generated. OBJECTIVE: We report a first trimester scan ghost twin, diagnosed since first trimester of pregnancy. CLINICAL CASE: 39-year-old patient, with pregnancy due to in vitro fertilization, who attends ultrasound evaluation of the first trimester. Routine ultrasound at 11 weeks showed a mirror image and magnetic resonance imaging: a pregnant uterus with a hypoechoic retrouterine structure, with no fetus inside. The ultrasound mirror image reproduced again in the third trimester. The pregnancy came to an end and ended by abdominal route, without being able to demonstrate the structures that were noticed on the MRI. CONCLUSION: Very few reports of obstetric ultrasound mirror images have been reported. We hypothesize that the transient retrouterine hypoecogenic structure observed by MRI was the reflective surface that create the ghost twin image.

The objective was to describe the family stigma perceived by the primary caregivers of people with mental illness, for which we used as a methodology a descriptive exploratory study, based on the eidetic phenomenology, where the life experience was described in detail. To the stigma of mental illness perceived by the primary caregiver, we seek to reveal, describe and interpret the phenomenon. Fourteen primary caregivers of patients were interviewed in depth with the point technique where the saturation of qualitative data was presented, they were analyzed establishing categories and subcategories that allowed to identify clear patterns of stigma, stigma experienced and stigma by association, the study was developed in a Institution that provides mental health services. We can conclude that it is possible to identify clear stigma patterns associated with a mental condition, these patterns are perceived by the main caregiver of the patient with said alteration; this opens the door to new questions related to the scope of the social rejection suffered by families and patients who suffer from mental disorders; therefore, this problem should be the object to continue with investigations of this phenomenon.

RESUMO

INTRODUCTION: Acute appendicitis is the most common surgical disease in emergency surgery, however, it remains a diagnostic problem and represents a challenge despite the experience and the different clinical and paraclinical diagnostic methods. OBJECTIVE: To evaluate in a comparative way the scale of Alvarado, AIR and RIPASA to determine which one is best as a diagnostic test of acute appendicitis in our population in order to arrive to an accurate diagnosis in the shortest possible time and cost. METHOD: Observational, prospective, transversal and comparative study of 137 patients to whom the scale of Alvarado, AIR and RIPASA was applied, who entered the emergency service of the Civil Hospital of Culiacán (México) with abdominal pain syndrome suggestive of acute appendicitis. RESULTS: The Alvarado scale presented sensitivity 97.2% and specificity of 27.6%. AIR presented sensitivity of 81.9% and specificity of 89.5%. RIPASA showed the same results as Alvarado. All tests showed diagnostic accuracy above 80. CONCLUSIONS: Alvarado and RIPASA presented good sensitivity, however, AIR is more specific, and has better accuracy for the diagnosis of acute appendicitis, making a better screening and thus reducing unnecessary surgeries. Therefore, it is recommended to use more AIR than Alvarado and RIPASA.

18F-FDG PET/CT is a useful imaging modality in the diagnosis and follow-up of peripheral nerve sheath tumors, especially in the assessment of tumor grade and biopsy guidance. The case of a 34-years-old woman diagnosed with peripheral nerve sheath tumor located in the posterior mediastinum that generated superior vena cava syndrome and Horner syndrome is presented. 18F-FDG PET/TC was performed to assess the possibility of malignancy. An 18F-FDG PET/CT was performed to determine whether it was benign or malignant, a heterogeneous mass with hypermetabolic areas with a maximum standardized uptake value (SUVmax) of 8.5 was found, and suggested malignancy with multiple grades of differentiation. A tumor biopsy from the region of higher metabolism was recommended with pathology result of malignant peripheral nerve sheath tumor.

Vascular rings are a spectrum of pathologies where the aortic arch, pulmonary artery and/or the supra-aortic vessels have a different origin or an abnormal course; this may or may not produce tracheal and/or esophageal compression. These entities have a variable clinical presentation, with respiratory symptoms being the most common. They are associated with cardiac, gastrointestinal and other system malformations. We reviewed the available literature about the embryology, epidemiology, clinical and Magnetic resonance imaging findings of the most common vascular rings.

RESUMO

The access to medical information (journals, blogs, web-pages, dictionaries, and texts) has been increased due to availability of many digital media. In particular, finding an appropriate structure that represents the information contained in texts is not a trivial task. One of the structures for modeling the knowledge are ontologies. An ontology refers to a conceptualization of a specific domain of knowledge. Ontologies are especially useful because they support the exchange and sharing of information as well as reasoning tasks. The usage of ontologies in medicine is mainly focussed in the representation and organization of medical terminologies. Ontology learning techniques have emerged as a set of techniques to get ontologies from unstructured information. This paper describes a new ontology learning approach that consists of a method for the acquisition of concepts and its corresponding taxonomic relations, where also axioms disjointWith and equivalentClass are learned from text without human intervention. The source of knowledge involves files about medical domain. Our approach is divided into two stages, the first part corresponds to discover hierarchical relations and the second part to the axiom extraction. Our automatic ontology learning approach shows better results compared against previous work, giving rise to more expressive ontologies.

RESUMO

OBJECTIVE: To determine the prevalence of postnatal depression and associated epidemiological features in a population of women from Durango, Mexico. METHODS: Applying a cross-sectional design in public hospitals from Durango, we studied 178 women during their 1 to 13 weeks postpartum. The Edinburg Postnatal Depression Scale was applied and depression was evaluated by using the DSM-IV criteria. In addition, socio-demographic, clinical, and psychosocial data from participants were obtained. RESULTS: Of the 178 women, 58 were depressed (32.6%). The prevalence of depression was significantly higher in women with low level of education, with more than 3 years of living with her partner, and in rural, non-insured women. Multivariate analysis showed that postnatal depression was significantly associated with previous depression, history of postnatal depression, depression, anxiety and stress during pregnancy, stress after pregnancy, trauma, bad relationship with partner, abandonment by partner, unwanted pregnancy, family problems, and living without partner. CONCLUSIONS: The prevalence of postnatal depression in women living in Durango, Mexico, is high. Several socio-demographic, clinical, and psychosocial factors appear to contribute to this condition.
