RESUMO

Contemporary reconstruction algorithms yield the potential of reducing radiation exposure by denoising coronary computed tomography angiography (CCTA) datasets. We aimed to assess the reliability of coronary artery calcium score (CACS) measurements with an advanced adaptive statistical iterative reconstruction (ASIR-CV) and model-based adaptive filter (MBAF2) designed for a dedicated cardiac CT scanner by comparing them to the gold-standard filtered back projection (FBP) calculations. We analyzed non-contrast coronary CT images of 404 consecutive patients undergoing clinically indicated CCTA. CACS and total calcium volume were quantified and compared on three reconstructions (FBP, ASIR-CV, and MBAF2+ASIR-CV). Patients were classified into risk categories based on CACS and the rate of reclassification was assessed. Patients were categorized into the following groups based on FBP reconstructions: 172 zero CACS, 38 minimal (1-10), 87 mild (11-100), 57 moderate (101-400), and 50 severe (400<). Overall, 19/404 (4.7%) patients were reclassified into a lower-risk group with MBAF2+ASIR-CV, while 8 additional patients (27/404, 6.7%) shifted downward when applying stand-alone ASIR-CV. The total calcium volume with FBP was 7.0 (0.0-133.25) mm3, 4.0 (0.0-103.5) mm3 using ASIR-CV, and 5.0 (0.0-118.5) mm3 with MBAF2+ASIR-CV (all comparisons p < 0.001). The concomitant use of ASIR-CV and MBAF2 may allow the reduction of noise levels while maintaining similar CACS values as FBP measurements.

RATIONALE AND OBJECTIVES: The aims of this study were to distinguish stents from iodinated contrast on the basis of spectral characteristics on dual-energy computed tomographic (DECT) imaging and to determine whether DECT imaging might provide a more accurate measurement of true stent lumen. MATERIALS AND METHODS: Three stainless steel stents and one cobalt chromium stent were scanned using a multidetector, single-source DECT scanner. Stents 2.5, 3.5, and 4.0 mm in diameter were filled with iodinated contrast, submerged in water, and scanned. Spectral analysis was performed to assess the separation of stents from iodinated contrast. Two independent reviewers measured stent lumen diameter and strut thickness on low-energy (L(0)), high-energy (L(1)), and combined-energy (L(c)) images. Dual-energy full-width half-maximum edge detection analysis was used to provide an independent assessment of stent luminal diameter and strut thickness. RESULTS: Two-dimensional graphical plots of computed tomographic attenuation for the L(0) and L(1) images did not demonstrate a sharp separation between the absorption characteristics of stents and iodinated contrast material. Stent lumens were underestimated by approximately 50% on L(c) images. Observer measurements on L(1) images demonstrated a 24% decrease in strut thickness and a 25% increase in stent luminal diameter compared to L(0) images (P < .0001). Full-width half-maximum measurements did not demonstrate significant changes in stent luminal diameters or strut thicknesses between L(0) and L(1) images. CONCLUSIONS: Spectral analysis did not clearly distinguish stents from iodinated contrast with the DECT system used in this study. The larger stent lumens visualized by the high-energy components of the x-ray spectrum were not related to improved computed tomographic delineation of stent thickness.

Leishmania donovani are the causative agents of kala-azar in humans. They undergo a developmental program following changes in the environment, resulting in the reversible transformation between the extracellular promastigote form in the sand fly vector and the obligatory intracellular amastigote form in phagolysosomes of macrophages. A host-free differentiation system for L. donovani was used to investigate the initial process of promastigote to amastigote differentiation. Within an hour after exposing promastigotes to differentiation signal (concomitant exposure to 37 degrees C and pH 5.5), they expressed the amastigote-specific A2 protein family. At 5 h they started to transform to amastigote-shaped cells, a process that was completed 7 h later. This morphological transformation occurred synchronously, while cells arrested at G1. By sequential exposure to elevated temperature (for 24 h) and then acidic pH, we found that heat was responsible for the growth arrest and acidic pH to its release and subsequent route to differentiation into amastigotes. Lastly, ethanol and Azetidine 2 carboxylic acid (a synthetic proline analog) that induced heat shock response in promastigotes were capable of replacing heat in the differentiation signal.

RESUMO

A new postprocessing algorithm was developed for the diagnosis of breast cancer using electrical impedance scanning. This algorithm automatically recognizes bright focal spots in the conductivity map of the breast. Moreover, this algorithm discriminates between malignant and benign/normal tissues using two main predictors: phase at 5 kHz and crossover frequency, the frequency at which the imaginary part of the admittance is at its maximum. The thresholds for these predictors were adjusted using a learning group consisting of 83 carcinomas and 378 benign cases. In addition, the algorithm was verified on an independent test group including 87 carcinomas, 153 benign cases and 356 asymptomatic cases. Biopsy was used as gold standard for determining pathology in the symptomatic cases. A sensitivity of 84% and a specificity of 52% were obtained for the test group.
