RESUMO

AIM: Clinicocytological evaluation of efficacy of combined treatment of chronic obstructive bronchitis (COB) in exacerbation with application of laser radiation of blood. MATERIALS AND METHODS: Combined treatment with the use of He-Ne intravenous and transcutaneous radiation of blood was given to 32 patients with COB. 27 COB patients treated without blood irradiation served control. Mean age of the patients (39 males and 20 females) was 59 +/- 9.5 years. In addition to conventional methods of examination and control of the treatment effect, cytological and bacteriological tests of BAL precipitate smears were made. RESULTS: Combined COB treatment with the use of laser blood radiation has an antiinflammatory action, promotes normalization of mucociliary transport, activation of phagocytosis and immune defense, cleansing of bronchial tree, reduction of obstruction (by FEV per 1 s), effective management of exacerbations. Hospital stay decreased by 3-4 days. Blood irradiation has the advantages as a noninvasive method. CONCLUSION: Clinical, cytological and bacteriological tests, determination of FEV provide a significant assessment of treatment efficacy in dynamics and facilitate the choice of the most effective regimen for management of COB in exacerbation.

RESUMO

Hyperbaric oxygenation (HBO) effects on the levels of circulating glycosylated proteins with various life spans in the blood stream (glycosylated hemoglobin (GH) and serum glycosylated fructose amine (SF) were studied in diabetes mellitus patients not suffering from vascular complications but developing various microangiopathies. Measurements of blood glycosylated proteins and mean daily glycemia carried out over the course of HBO treatment in the diabetics without microangiopathies evidenced an essential reduction of blood GH, SF, and glucose, reaching, in some cases, the normal values. Patients with vascular involvement (lower limb angiopathy, nephropathy, retinopathy) exposed to HBO developed but a trend to reduction of blood levels of glycosylated proteins and glucose. These results indicate a positive therapeutic effect of HBO courses on the status of diabetics with slightly manifest microvascular involvement. In cases with grave vascular involvement exposure to HBO appears inefficient.

RESUMO

The authors discuss the results of the use of plasmapheresis in multimodality treatment of patients with stage II essential hypertension. At present the prevalence, ever increasing accessibility and relative simplicity of plasmapheresis make it possible to use it for the treatment of essential hypertension. Analysis of the data obtained shows the high efficacy of plasmapheresis in essential hypertension. Since plasmapheresis turned out effective in combination with drug therapy and alone as well, it is concluded that plasmapheresis is not only an approach to overcoming resistance to hypotensive therapy but also an independent treatment modality that exerts a considerable effect on the immunologic and hemorheologic blood properties. The above-indicated allows recommending plasmapheresis for the treatment of patients suffering from essential hypertension, since it may lead to a long-term decrease of arterial pressure, to the enhancement of the sensitivity to hypotensive drugs, to the diminution of drug requirements, thereby ameliorating the course and prognosis of the disease in patients suffering from essential hypertension.

RESUMO

Cytochemistry of peripheral blood lymphocyte enzymes was studied in 58 patients with State II essential hypertension (36 of these were treated with traditional hypotensive therapy and 22 with plasmapheresis + drug therapy). alpha-GPDH, LDH, and acid phosphatase activities were found reduced in these patients. The degree of the enzymic activity reduction (SDH included) depended on the disease standing and family history of patients. Plasmapheresis elevated the activities of energy enzymes and reduced arterial pressure, i.e. these parameters were in inverse correlation. In the patients treated by routine hypotensive therapy energy enzymes' activity was unchanged, despite the clinical effect.
