Unfounded splenectomy in portal hypertension exposes the patients to several risks. This is apparent from presented case-histories as well as reviews and the authors' experience. The authors emphasize general principles of the approach to surgery of the spleen in portal hypertension.

The authors give an account of twelve cases of inflammatory acute abdominal attacks which complicated the course after abdominal operations. Although rare complications are involved, they are very serious and to a certain extent instructive. In the conclusion the authors summarize some diagnostic, therapeutic and assessment rules for these uncommon situations.

In the years 1981-1985 21 patients were operated upon for symptomatic abdominal aortic aneurysms. Atypical symptoms such as acute pancreatitis, weight loss, blood in the stool or urine, incorrect interpretation of abdominal pain and physical examination findings caused erroneous initial diagnosis and delay in referring the patient to surgical treatment in nine cases. Long history prior to admission was noted even in patients with ruptured aneurysms; mortality in these patients was high, 66%. Best results were achieved with aneurysm resection; indirect aneurysm occlusion in combination with axillofemoral bypass attempted in two patients was not successful.

The author gives original information on the situation in Czechoslovakia regarding the treatment of varicose veins. The prevalence of varicose veins is 20%, the incidence is more than 1% in the adult population. Heredity accounts for 75% of these cases. In one year, only 35 to 129 patients are operated for varicose veins in a department of Clinical Surgery. The serious complications were analysed over a period of 10 years: there were 37 cases of pulmonary embolism and 10 deaths out of 95,000 operations for varices. The long-term results, that is from 5 - 15 years, were considered to be satisfactory in more than 90% of the cases operated. In Czechoslovakia, sclerotherapy is still a complementary method. However, the good results of sclerosing varices have been proven by the statistics issued by the phlebological clinics set up by the Czech Dermatological Association.
