OBJECTIVE: The present study assessed the influence of physical training on cardiac autonomic activity in individuals with spinal cord injury (SCI) during different sleep stages. METHODS: Twenty-six volunteers were allocated into three groups: 9 sedentary individuals without SCI (control, CON); 8 sedentary tetraplegic individuals with chronic SCI (SED-SCI); 9 physically trained tetraplegic individuals with chronic SCI (TR-SCI). All participants underwent nocturnal polysomnography to monitor sleep stages: wakefulness, non-rapid eye movement (NREM) sleep (N1, N2, and N3 stages), and REM sleep. The electrocardiography data obtained during this exam were extracted to analyze the heart rate variability (HRV). RESULTS: Sleep stages influenced HRV in the time [RR interval and root mean square of successive RR interval differences (RMSSD)] and frequency [low-frequency (LF) and high-frequency (HF) powers and LF-to-HF ratio] domains (P < 0.05). SED-SCI individuals showed unchanged HRV compared to CON (P > 0.05). When comparing the TR-SCI and SED-SCI groups, no significant differences in HRV were reported in the time domain (P > 0.05). However, in the frequency domain, more accentuated HF power was observed in TR-SCI than in SED-SCI individuals during the N2 and N3 stages and REM sleep (P < 0.05). Moreover, TR-SCI had higher HF power than CON during the N3 stage (P < 0.05). CONCLUSIONS: TR-SCI individuals have greater HF power, indicative of parasympathetic modulation, than sedentary (injured or not injured) individuals during different sleep stages. Therefore, enhanced parasympathetic activity induced by physical training may improve cardiac autonomic modulation during sleep in individuals with chronic SCI.

RESUMO

We conducted a genealogical analysis of quaternary prevention, an instrument of primary health care to address overmedicalization and iatrogenesis, based on related statements and interviews with the creators of this concept. This tool has been used in the reformulation of care and the doctor-patient relationship, but limited to the risk-benefit assessment by using current scientific evidence. In this study, we analyze the paradoxes of evidence-based medicine (EBM) and discuss the relationship of EBM and quaternary prevention and primary health care (PHC). Finally, we suggest questioning the truth of the evidence for the development of other health paradigms.


Realizamos uma análise genealógica da Prevenção Quaternária, instrumento da Atenção Primária à Saúde de enfrentamento à medicalização e a iatrogenia, a partir de seus enunciados e de entrevistas com seus formuladores. Identificamos que a ferramenta tem sido tanto apresentada como uma reformulação do cuidado e da relação médico-paciente, como também reduzida ao cálculo do risco-benefício por meio da aplicação atualizada de evidências científicas. Analisamos os paradoxos da Medicina Baseada em Evidências e problematizamos sua relação com a Prevenção Quaternária e a Atenção Primária à Saúde. Por fim, sugerimos questionar a verdade das evidências para o desenvolvimento de outros paradigmas de saúde.

CONTEXT: Knowing the methods to assess the external load in Paralympic sports can help multidisciplinary teams rely on scientific evidence to better prescribe and monitor the athlete's development, improving sports performance and reducing the risk of injury/illness of Paralympic athletes. OBJECTIVES: This review aimed to systematically explore the current practices of quantifying the external load in Paralympic sports and provide an overview of the methods and techniques used. EVIDENCE ACQUISITION: A search in PubMed, Web of Science, Scopus, and EBSCO was carried out until November 2022. The measures of interest were objective methods for quantifying the external load of training or competition. The inclusion criteria for the studies were as follows: (1) peer-reviewed article; (2) the population were Paralympic athletes; (3) evaluated during training or competition; (4) reported at least one external load measure; and (5) published in English, Portuguese, or Spanish. EVIDENCE SYNTHESIS: Of the 1961 articles found, 22 were included because they met the criteria, and 8 methods were identified to quantify the external load in training or competition in 8 Paralympic sports. The methods varied according to the characteristics of the Paralympic sports. To date, the devices used included an internal radiofrequency-based tracking system (wheelchair rugby) a miniaturized data logger (wheelchair tennis, basketball, and rugby); a linear position transducer (powerlifting and wheelchair basketball); a camera (swimming, goalball, and wheelchair rugby); a global positioning system (wheelchair tennis); heart rate monitors that assess external load variables in set (paracycling and swimming) and an electronic timer (swimming). CONCLUSIONS: Different objective methods were identified to assess the external load in Paralympic sports. However, few studies showed the validity and reliability of these methods. Further studies are needed to compare different methods of external load quantification in other Paralympic sports.

CONTEXT: Sleep serves many important functions for athletes, particularly in the processes of learning, memory, recovery, and cognition. OBJECTIVES: Define the sleep parameters of Paralympic athletes and identify the instruments used to assess and monitor sleep Paralympic athletes. EVIDENCE ACQUISITION: This systematic review was carried out based on the PRISMA guidelines. The survey was conducted in April 2020, the searches were carried out again in September 2021 to check whether there were new scientific publications in the area of sleep and Paralympic sport, searches were performed in the following databases: PubMed, Web of Science, Scopus, SPORTDiscus, Virtual Health Library (BIREME), and SciELO. This systematic review has included studies that investigated at least one of the following sleep parameters: total sleep time, sleep latency, sleep efficiency, number of awakenings, quality of sleep, daytime sleepiness, and chronotype; the participants were comprised of athletes with disabilities. Studies published at any time in English, Portuguese, and Spanish, were included. EVIDENCE SYNTHESIS: Data extraction and study selection were performed by 2 researchers independently, and a third author was consulted as necessary. The search returned a total of 407 studies. Following the screening based on exclusion and inclusion criteria, a total of 13 studies were considered. Paralympic athletes have a low amount (7.06 h) of sleep with poor quality and sleep latency (28.05 min), and 57.2% have daytime sleepiness, with the majority belonging to the indifferent chronotype (53, 5%). Moreover, 11 studies assess sleep using subjective instruments (questionnaires), and 2 studies used an objective instrument (actigraphy). CONCLUSIONS: Sleep disorders are common among Paralympic athletes, poor sleep quality and quantity, and high rates of daytime sleepiness. Subjective methods are most commonly used to assess sleep.

This study aims to describe the athletes' sleep regularity using the Sleep Regularity Index (SRI) and determine whether factors, such as sex, competitive level and sport type, could affect the sleep/wake rhythm. It is a descriptive, cross-sectional study consisting of 172 athletes (25 ± 7 y old; 45 women). Seventy-three competed in team sports and 99 in individual sports. Furthermore, 56 competed in the international level, 95 in the national and 21 in the regional. We recorded the SRI values for at least 5 d via continuous actigraphy. We recorded a mean SRI value of 73 ± 12. We found no significant differences between athletes' sleep parameters in relation to sleep regularity. Furthermore, SRI data showed no correlations (Spearman's ρ) with sleep parameters, bed time and wake time. Female athletes (p = 0.001) and individual sport athletes (p = 0.001) reported better sleep regularity than their counterparts. International-level athletes reported better sleep regularity than those in other competitive levels (national: p = 0.001; regional: p = 0.024). Our study showed that international level athletes, female athletes and individual sport athletes reported better sleep regularity. Additionally, SRI data showed no correlation with athletes' sleep parameters, bed and wake time.

OBJECTIVES: To evaluate sleep characteristics and investigate the relationship of sleep with injuries and illnesses in Paralympic athletes. DESIGN: Cross-sectional. SETTING: Sports Training Center. PARTICIPANTS: 20 Paralympic athletes of athletics, swimming, and powerlifting. OUTCOMES: Injury and illnesses were recorded during the sports season through the Oslo Sports Trauma Research Center questionnaire (OSTRC-BR). Sleep-wake pattern was monitored for 2 weeks using actigraphy. Chronotype, sleep quality, sleep behavior, and sleep complaints were cross-sectionally collected. RESULTS: Poor sleep quality and poor sleep behavior were highly prevalent. The mean total sleep time was 6.57 ± 49.91. Duration of naps (r= -0.46; p=0.04) was associated with occurrence of health problems; frequency of insomnia (r= 0.51; p= 0.02), Pittsburgh Sleep Quality Index score (r= 0.45; p=0.04), and frequency of awakenings at night (r= 0.58; p= 0.01) were associated with severity of health problems; frequency of movements during sleep was associated with OSTRC-BR cumulative score (r= 0.58; p=0.00); and frequency of nightmares was associated with OSTRC mean score. CONCLUSIONS: Paralympic athletes tend to report poor sleep quality, have poor sleep behavior and sleep less than the recommended. Insomnia symptoms, awakenings at night, movements during sleep and poor sleep quality were associated with the occurrence and/or severity of health problems.

PURPOSE: Radiation-induced cellular senescence is a double-edged sword, acting as both a tumor suppression process limiting tumor proliferation, and a crucial process contributing to normal tissue injury. Endothelial cells play a role in normal tissue injury after radiation therapy. Recently, a study observed an accumulation of senescent endothelial cells (ECs) around radiation-induced lung focal lesions following stereotactic radiation injury in mice. However, the effect of radiation on EC senescence remains unclear because it depends on dose and fractionation, and because the senescent phenotype is heterogeneous and dynamic. METHODS AND MATERIALS: Using a systems biology approach in vitro, we deciphered the dynamic senescence-associated transcriptional program induced by irradiation. RESULTS: Flow cytometry and single-cell RNA sequencing experiments revealed the heterogeneous senescent status of irradiated ECs and allowed to deciphered the molecular program involved in this status. We identified the Interleukin-1 signaling pathway as a key player in the radiation-induced premature senescence of ECs, as well as the endothelial-to-mesenchymal transition process, which shares strong hallmarks of senescence. CONCLUSIONS: Our work provides crucial information on the dynamics of the radiation-induced premature senescence process, the effect of the radiation dose, as well as the molecular program involved in the heterogeneous senescent status of ECs.

Resumo A Saúde Coletiva brasileira analisou frequentemente o neoliberalismo como um fenômeno de esvaziamento do papel do Estado e de ameaça à saúde pública e universal. Tomando como subsídio o pensamento governamental de Foucault, discutimos o neoliberalismo como uma profunda metamorfose, não apenas do Estado, mas dos modos de produção de saúde. Enquanto atualização permanente do liberalismo, o governo neoliberal modifica as fronteiras entre público e privado e fabrica novas formas de normalidade, risco e subjetividade, progressivamente subordinados à verdade da economia e do mercado. Esta racionalidade econômica cria ideais de saúde inspirados em técnicas gerenciais de empresas e produz novas verdades biológicas, sanitárias, psicológicas. Restrita a "empresários bem-sucedidos de si mesmos", a saúde pode se transformar em uma escolha moral e econômica em relação ao comportamento e ao risco individual, desresponsabilizando o Estado e criando um tipo de cidadania econômica destituída de solidariedade. Contudo, o jogo em torno de instituições e práticas de saúde não empresariais segue em aberto. Cabe-nos colocar em dúvida as formas de vida "responsáveis" e "seguras" que foram inventadas para nós, e desenvolver outras governamentalidades menos excludentes e desiguais em relação às que temos naturalizado e praticado.


Abstract Brazilian Public Health has often analyzed neoliberalism as a phenomenon of emptying the role of the State and a threat to public and universal health. Taking Foucault's governmental thought as a subsidy, we discuss neoliberalism as a profound metamorphosis, not only of the State, but of health production. As a permanent update of liberalism, the neoliberal government changes the boundaries between public and private and produces new forms of normality, risk and subjectivity, progressively subordinate to the truth of the logic of the economy and the market. This economic rationality creates new ideals of health, inspired by management techniques of corporations, and produces new biological, sanitary, psychological truths. Restricted to "successful self-entrepreneurs", health may become a moral and economic choice in relation to individual behavior and risk, making the State not responsible and creating a type of economic citizenship devoid of solidarity. However, the game around non-corporate health institutions and practices remains open. It is up to us to question the "responsible" and "safe" life forms that were invented for us and to develop other governmentalities that are less excluding and unequal compared to those that we have naturalized and practiced.

Abstract Aim: This study aims to compare the sleep parameters in Paralympic powerlifting athletes during days with and without training, and to analyze the relationship between the training load and sleep on the same day and the relationship between the previous night's sleep and the training load of the following day. Methods: Actigraphy was used to analyze the sleep parameters of 11 Paralympic powerlifting athletes for 14 days (7 days without and with training), whereas Ratings of Perceived Exertion (RPE) analysis was used to assess training load. In addition, the Horne and östberg chronotype questionnaire and the Epworth Sleepiness Scale were applied. Results: Athletes show morning and indifferent chronotype and low daytime sleepiness. We found that on training days, sleep onset latency (SOL) was lower (average 5.3 min faster), whereas total sleep time (TST) and sleep efficiency (SE) were higher (TST averaged 169 min and SE 7% higher) compared to non-training days. In addition, the TST of the night before the training days correlated positively with the RPE of the following day, and the training volume correlated negatively with the SE of the same day. Conclusion: Our findings show that Paralympic powerlifting training had positive effects in increasing TST and SE and decreasing SOL on training days. These results show the positive effects of this type of training in improving sleep in athletes with physical disabilities. In addition, a good night's sleep the day before training can make it possible to put more effort into the next day's training. Therefore, guiding athletes to sleep more before training with more intense loads is recommended.

ABSTRACT In December 2019, Wuhan, in China, attracted international attention due to a pneumonia outbreak caused by the new coronavirus (2019-nCoV). Infection by 2019-nCoV is more likely in elderly people with comorbidities or with associated chronic diseases. Due to the high transmission rate among humans, this disease is rapidly disseminated, which led to several events being canceled, including the Tokyo 2020 Olympic and Paralympic Games. The aim of this article is to discuss the risk factors for Olympic and Paralympic athletes, as well as for spectators, that justify the decision to postpone the Tokyo Games 2020. Regular physical exercise is associated with health and the prevention of chronic diseases. Although athletes generally appear to be healthy and physically fit, this may not be true. The immune system, which protects the organism from invasive microorganisms, can be affected by the duration and quality of sleep, as well as by physical exercise which influences the quality of the immune response. High volumes of high-intensity physical exercise, as well as changes in sleep patterns during the pre-competition period and the impacts of jet lag on athletes traveling for the Tokyo Games in 2020 may lead to immune system suppression, making these groups more vulnerable to infection by 2019-nCoV. Moreover, during the period planned for the games in 2020 the pandemic may be subsiding in some countries and increasing in others, and this was also taken into consideration as a risk factor. Hence, the decision taken to postpone the Tokyo 2020 Olympic and Paralympic Games until 2021 due to the 2019-nCoV was the correct one, and was extremely important to protect the health of Olympic and Paralympic athletes, as well as spectators. A infecção pelo 2019-nCoV é mais provável em pessoas idosas com comorbidades ou com doenças crônicas associadas. Em virtude da alta taxa de transmissão entre humanos, essa doença tem disseminação rápida, o que fez com que diversos eventos fossem cancelados, dentre eles os Jogos Olímpicos e Paralímpicos de Tóquio 2020. Nesse sentido, o objetivo deste artigo é discutir fatores de risco dos atletas olímpicos e paralímpicos, bem como dos espectadores, que justificam a decisão de adiamento dos Jogos de Tóquio 2020. A prática de exercício físico regular é associada à saúde e à prevenção de doenças crônicas. Embora normalmente pareça que os atletas estão em boa forma e são saudáveis, isso pode não ser verdade. O sistema imunológico, que protege o organismo de microrganismos invasores, pode ser afetado pela quantidade e qualidade do sono, assim como pela prática de exercício físico que influencia a qualidade da resposta imunológica. A prática de exercícios de alta intensidade e grande volume, além das alterações do sono no período pré-competitivo e os impactos do jet lag dos atletas que viajariam para os Jogos de Tóquio no ano de 2020 podem levar à supressão do sistema imunológico, deixando esses grupos mais vulneráveis à contaminação pelo 2019-nCoV. Além disso, no período previsto de ocorrência dos jogos em 2020 a pandemia poderia estar em regressão em alguns países e ascensão em outros, e isso também foi levado em consideração como um fator de risco. Nesse sentido, a tomada de decisão de adiar os Jogos Olímpicos e Paralímpicos de Tóquio 2020 para o ano de 2021 devido ao 2019-nCoV foi correta e de extrema importância para preservar a saúde dos atletas olímpicos e paralímpicos, bem como dos expectadores. RESUMEN En diciembre de 2019, Wuhan, en China, despertó la atención internacional debido a un brote de neumonía causada por el nuevo coronavirus (2019-nCoV). La infección por el 2019-nCoV es más probable en personas de la tercera edad con comorbidades o con enfermedades crónicas asociadas. En virtud de la alta tasa de transmisión entre humanos, esta enfermedad tiene diseminación rápida, lo que hizo con que diversos eventos fuesen cancelados, entre ellos los Juegos Olímpicos y Paralímpicos de Tokio 2020. En ese sentido, el objetivo de este artículo es discutir factores de riesgo de los atletas olímpicos y paralímpicos, bien como de los espectadores, que justifican la decisión de postergación de los Juegos de Tokio 2020. La práctica de ejercicios físicos de forma regular está asociada a la salud y a la prevención de enfermedades crónicas. Aunque normalmente parezca que los atletas están en buena forma y son saludables, eso puede no ser verdad. El sistema inmunológico, que protege el organismo de microorganismos invasores, puede ser afectado por la cantidad y calidad del sueño, así como por la práctica de ejercicios físicos que influencia la calidad de la respuesta inmunológica. La práctica de ejercicios de alta intensidad y gran volumen, además de las alteraciones del sueño en el período precompetitivo y los impactos del jet lag de los atletas que viajarían para los Juegos de Tokio en el año 2020 pueden llevar a la supresión del sistema inmunológico, dejando a esos grupos más vulnerables a la contaminación por el 2019-nCoV. Además, en el período previsto de ocurrencia de los juegos en 2020 la pandemia podría estar en regresión en algunos países y ascensión en otros, y eso también fue llevado en consideración como un factor de riesgo. En ese sentido, la toma de decisión de postergar los Juegos Olímpicos y Paralímpicos de Tokio 2020 para el año 2021 debido al 2019-nCoV fue correcta y de extrema importancia para preservar la salud de los atletas olímpicos y paralímpicos, bien como de los espectadores. Sleep is a physiological state and it is fundamental for physical and cognitive recovery of athletes. Due to strenuous training and competitions, athletes may present sleep complaints compromising good quality and quantity of sleep. Studies have related sleep debt to the occurrence of musculoskeletal injuries in athletes, but the mechanisms that can lead to this are not entirely clear. Studies involving animals and humans have shown that poor sleep quality can cause significant changes in hormones and cytokines. Demonstrating that this hormones changes lead to a decrease of testosterone and growth hormone levels and increased cortisol levels, important hormones in the process of protein synthesis and degradation. In athletes, the sport itself is a risk factor of injuries, and sleep debt may result in overtraining syndrome associated with inflammatory markers and ultimately to immune system dysfunction. Thus, we hypothesize that athletes who have sleep debt are more susceptible to musculoskeletal injuries due to increased catabolic pathway signaling, i.e. protein degradation and decreased anabolic pathway signaling, compromising muscle integrity. In this sense, we indicate the relationship between musculoskeletal injuries and sleep debt involving new targets for immunological signaling pathways that start the reduction of the muscle recovery process.

Rotating and permanent night shiftwork schedules typically result in acute and sometimes chronic sleep deprivation plus acute and sometimes chronic disruption of the circadian time structure. Immune system processes and functionalities are organized as circadian rhythms, and they are also strongly influenced by sleep status. Sleep is a vital behavioral state of living beings and a modulator of immune function and responsiveness. Shiftworkers show increased risk for developing viral infections due to possible compromise of both innate and acquired immunity responses. Short sleep and sleep loss, common consequences of shiftwork, are associated with altered integrity of the immune system. We discuss the possible excess risk for COVID-19 infection in the context of the common conditions among shiftworkers, including nurses, doctors, and first responders, among others of high exposure to the contagion, of sleep imbalance and circadian disruption. RESUMO

Este livro versa sobre um encontro entre a rua e a universidade, especificamente, entre o Consultório na Rua da cidade de Campinas e o Coletivo de Pesquisa Conexões, do Departamento de Saúde Coletiva da Unicamp, cotejado por encontros semelhantes no Rio de Janeiro e em São Luís, no Maranhão. Retenho da leitura o tanto que encontros como esses podem transformar concepções de extensão universitária, de rua, de cuidado, de ação política. Não há, em nenhum dos textos aqui apresentados, a ideia unidirecional de que extensão universitária seja levar o conhecimento produzido na universidade para a população em situação de rua, ou para as equipes que atuam junto a ela. Depreende-se, de outra via, que saberes são construídos nas relações com as ruas, com as pessoas que nelas circulam e com a vida que nela se produz.

A Rede de Pesquisa em Atenção Primária à Saúde (Rede APS) vem buscando, nos últimos oito anos, a participação, o acesso e o intercâmbio de conhecimento científico nacional e internacional sobre Atenção Primária à Saúde (APS) entre pesquisadores, gestores e trabalhadores do Sistema Único de Saúde (SUS). Apresenta-se o contexto de seu surgimento, uma análise cronológica de seus principais marcos históricos e sua relação com o desenvolvimento e com a avaliação de políticas brasileiras de atenção básica. Realizou-se uma análise documental de publicações on-line, documentos oficiais e atas de reuniões e entrevistas com personagens ligados à criação e ao desenvolvimento da Rede. A Rede APS participou diretamente da avaliação de políticas centrais à estruturação da atenção básica brasileira, com destaque para o apoio ao Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica, a difusão de pesquisas sobre o Programa Mais Médicos e dos debates críticos sobre a reformulação da Política Nacional de Atenção Básica. A partir de iniciativas como essa e buscando fortalecer o diálogo e o intercâmbio entre gestores, trabalhadores e pesquisadores do SUS, a Rede pode seguir exercendo um papel relevante na estruturação e na avaliação da APS no País.(AU)


Research Network on Primary Health Care (Rede APS) has been seeking for the last 8 years the participation, access and exchange of national and international scientific knowledge on Primary Health Care (APS) among researchers, managers and workers of Unified Health System (SUS). We present the context of its emergence, a chronological analysis of its main historical events and its relationship with the development of policies related to Brazilian APS. We have carried out a documentary analysis of online publications, official documents and meeting reports and interviews with actors related to its creation and development. Rede APS had an important influence on the evaluation of the National Program for Access and Quality Improvement in Primary Health Care (PMAQ-AB), the dissemination of research on the More Doctors Program and in the critical debates about the reformulation of the National Policy of Primary Health Care (PNAB). Based on initiatives such as this one and seeking to strengthen the dialogue and exchange among managers, workers and researchers, this network can continue to play a relevant role in structuring and evaluating APS in Brazil.(AU)

Resumo Realizamos uma análise crítica sobre a formação discursiva da Medicina de Família e Comunidade (MFC). A MFC é pensada como uma prática atravessada por relações políticas e sociais e que se produz historicamente enquanto uma formação discursiva, através de um jogo complexo e aberto entre os diversos atores, que compreendem, formulam e disputam o formato e o sentido dessa prática. Investigamos essa formação com base em revisão bibliográfica e análise do discurso de documentos que descrevem a emergência da MFC e de entrevistas com personagens ligados diretamente a ela. Pudemos identificar que a formação discursiva da MFC é atravessada pelas histórias da prática médica generalista, familiar e comunitária; da estruturação da Atenção Primária em Saúde; dos programas de Medicina Comunitária; e da formação da Saúde Coletiva no Brasil. Além disso, encontra-se atrelada ao campo científico da medicina generalista e familiar internacional e a sua afirmação como especialidade médica. Enquanto sociedade científica, vem apresentando dubiedades no que se refere às pautas históricas do movimento sanitário e na defesa de um sistema de saúde público, universal e antiprivatista. No entanto, para evitar cristalizações institucionais, entendemos ser necessário pensar a MFC como um campo heterogêneo e sem uma história única.


Abstract We performed a critical analysis on the discursive formation of Family and Community Medicine (FCM). FCM is thought of as a practice crossed by political and social relations that takes place historically as a discursive formation, through a complex and open game between the various actors, who understand, formulate and dispute the format and meaning of this practice. We investigated this training based on bibliographic review and discourse analysis of documents that describe the emergence of FCM and interviews with individuals directly linked to it. We were able to identify that the discursive formation of the FCM is traversed by the histories of general medical practice, family and community; the structuring of Primary Health Care; of Community Medicine programs; and the formation of Collective Health in Brazil. In addition, it is linked to the scientific field of general medicine and international family and its affirmation as a medical specialty. As a scientific society, it has been presenting doubts regarding the historical guidelines of the health movement and the defense of a public, universal and anti-privatization health system. However, in order to avoid institutional crystallization, we believe it is necessary to think of FCM as a heterogeneous field and without a single history.
