RESUMO

The article demonstrates that the restrictions introduced during the coronavirus pandemic affected the dynamics of mutual settlements between the territorial funds of the mandatory medical insurance (MMI, TFMMI). According to the results of the first nine months of 2020, medical institutions of a number of subjects were not able to earn even a half of the amount of sum that was presented for payment in 2019 on medical care support of patients from other regions. It was established that besides such relevant features of the Russian health care system as large territory, fragmentation of population, necessity to maintain a number of state medical institutions, there is also a number of factors that during the pandemic were most aggravated in those "weak" places in the MMI system that have been stagnating for a long time. Among them, insufficiently large listing of medical services covered by MMI, inability to provide medical services to all those in need due to shortage of equipment, working areas, qualified care, etc. It is noted that among the general trends of MMI, the Russian medical insurance system (not only that it is not a classical one), especially during the pandemic, is broken out of social insurance: two funds, structures, costs of informatization, etc. However, in Russia, even in such conditions, health care is funded through the MMI system up to 50%. The pandemic demonstrated that no private sector, no paid services, no commercial insurance companies can cover the whole spectrum of health care complicities during the pandemic crisis. Namely, the MMI system bore the brunt of the survival of medicine during the pandemic. It is concluded that namely the need in medical institutions with sufficient reserve of bed fund with MMI services can input into development of necessary stable basis for survival in difficult conditions of pandemic crisis. The proposal of the Audit Chamber of the Russian Federation of further reforming of the mandatory medical insurance system is considered reasonable and logical especially in difficult situation of struggle against COVID-19. The article also presents data concerning the state of MMI in 2020, at the height of the pandemic crisis in the regions of the Russian Federation.

RESUMO

The article notes that the number of citizens who apply for paid medical services is growing all over the world. The main reasons for this growth are: an increase in the level of well-being of the population; an increase in the areas of care for their own health; the inability to get the necessary assistance in state (free) structures that provide medical care; the complete absence of a number of medical services in free polyclinics, etc. The paper also presents data on the conduct of the study, which determines that on the basis of surveys and statistical data, as well as when studying reviews, it can be noted that in their significant part, consumers of medical services note that, first of all, the poor quality of public medical services prompted them to turn to paid services, but a number of respondents note that public free services in the field of medicine, especially recently, have significantly increased the level of quality. It is also concluded that from the point of view of consumers, one of the main advantages of purchasing services in commercial medical institutions is the ability to visit a doctor without waiting lists and guaranteed on the day of the appointment. Among the general trends in the paid medical services market, we can mention measures to consolidate the format and expand the areas of multi-specialty in private clinics, as well as to create conditions and factors, especially in the situation of a pandemic crisis, to provide high-quality medical services, including overcoming the consequences of the coronavirus epidemic and preventing its spread in the network of medical institutions. Statistical data and conclusions on the main areas of multidisciplinary research in the current state of paid medical services in the context of the pandemic crisis are presented. The review of new directions of providing medical services on a daily basis under the influence of the current state of the pandemic is made.

The article considers the relevance of marketing research in the field of paid medical services. Due to the fact that the market of paid services in Russia has increased significantly, medical management took neighboring position to medical marketing. The article emphasizes that in Russia annually the number of private medical institutions increases. This trend is directly related to to the fact that free medical care is not always able to provide necessary services vital for the patient. In turn, the existing increased competition pushes private medical institutions to actively fight for the client. The statistical data demonstrates that the loss of five customers per day results in losing of up to two million rubles monthly. It id established that due to increasing numbers of private medical institutions an urgent need for new logistics and marketing research of all medical production arises to have more comprehensive picture of the provision of a paid medical care. It is concluded that reaching these marketing and marketing research tasks determine the new special place of marketing technologies. The marketing in the health care system acts as basic system of principles targeted to creating and studying the demand and supply of medical services. In the health care market, the role of the consumer is played by patients whose needs must be met by medical clinics. The marketing in the health care system is the process of developing and providing medical services. The corresponding data and main conclusions related to provision of paid medical services in the Krasnodar region is presented.
