AIM: To investigate the role of autonomic nervous system in subpopulations of children with enuresis. METHODS: We included 35 children with enuresis, divided in children with (17) and without nocturnal polyuria (18) and 43 healthy controls. For all participants hormones and neurotransmitters were measured. Patients and controls wore a sleep tracker device and children with enuresis underwent a 24 h blood pressure monitoring, nocturnal urine output measurement and uroflowmetry. RESULTS: Children with enuresis had lower than controls copeptin and aldosterone, with the latter being more prominent in patients without nocturnal polyuria. Dopamine was lower in patients without nocturnal polyuria compared with patients with nocturnal polyuria. Children without polyuria experienced episodes only during NREM sleep, whereas in children with polyuria episodes occurred in both REM and NREM sleep. Children with enuresis experienced a non-dipping phenomenon during sleep which was more prominent in the group without polyuria. CONCLUSION: In patients with nocturnal polyuria, nocturnal enuresis is associated with sympathetic hyperactivity which results in pressure polyuria and significantly lower systolic dipping during sleep. On the contrary, in children without nocturnal polyuria, it is mostly associated with bladder overactivity due to parasympathetic overstimulation as demonstrated by the NREM-related enuretic episodes and the lower aldosterone and dopamine levels.

PURPOSE: Spontaneous muscle hematomas (SMH) are frequently seen in the Emergency Department (ED), particularly as a complication of anticoagulation treatments. To date, there are no standard guidelines for the management of this condition in the ED. This work aims to identify clinical-radiological parameters of SMH at risk of poor outcomes. METHOD: This is a retrospective, observational cohort study conducted in an urban teaching hospital from 2016 to 2019. A multivariate logistic regression analysis was performed to identify parameters independently associated with an adverse outcome defined as the need for treatment (blood products, TAE, surgery) or hospitalization. The parameters analyzed were hematoma size, anticoagulation therapy, age, and the Charlson Comorbidity Index (CCI). ROC analysis was performed to identify the best cut-off hematoma size value to predict poor outcomes. RESULTS: Our study enrolled 231 patients aged between 18 and 96 years, with a mean age of 67 years. In our population, 125 patients (54.1 % %) were on anticoagulant therapy. Multivariate analysis showed that an SMH diameter > 5.5 cm was independently associated with poor outcome ((odds Ratio [95 % CI] 4,009 [1,786-9,001], p 0.001). Among clinical parameters, only advanced age was proved to be an independent predictor of adverse outcomes (odds Ratio [95 % CI] 1,035 [1,003-1,069], p = 0.033) CONCLUSIONS: Our data suggest that an SMH diameter greater than 5.5 cm on a CT scan and advanced age are predictors of poor outcomes. Surprisingly, anticoagulant therapy seems to play a minor role in the outcome of SMHs.

In this follow-up study, we aimed to assess the effectiveness of the "I do not smoke, I exercise" anti-smoking preventive health education program. The program was based on the theory of planned behavior supplemented with life skills teaching and targeted at high school students. The intervention comprised ten one-hour online sessions, administered by physical education instructors. The study cohort comprised 222 students (109 boys, 113 girls) from 11 secondary schools, with an average age of 16.42 ± 1.36 years. Data collection involved pre- and post-intervention self-assessment questionnaires. The examined variables included attitudes towards smoking, intention to smoke, subjective norm, perceived behavioral control (PBC), knowledge about smoking, smoking behavior, exercise behavior, attitudes toward the program's implementation, and satisfaction with the program. A separate paired samples t-test revealed a significant improvement in students' knowledge about smoking (t217 = -5.605, p < 0.001, d = 0.38) and perceived behavioral control (t220 = -2.166, p < 0.05, d = 0.15) following the intervention. However, no significant changes were observed in the remaining variables. In addition, students' overall satisfaction with the implementation of the present health education program was high (M = 5.72 ± 1.39). These findings suggest that the health education smoking prevention intervention incorporates techniques and strategies that influence the perceived behavioral control variable, emphasizing students' strong interest in educationally theorized programs integrating technology into their design. Future studies should consider further examination of tobacco control strategies within the high school context.

INTRODUCTION: Nocturnal enuresis (NE) is common in children, but its pathophysiology is still not fully understood. Despite the recognition of three major pathways, nocturnal polyuria, nocturnal bladder dysfunction and sleep disorders, their inter-relations remain elusive. The autonomic nervous system (ANS) which is greatly involved in both diuresis and sleep might have an important role in NE. METHODS: A comprehensive electronic search of Medline database was performed, to identify articles reporting on the role of the autonomic nervous system (ANS) in enuretic children regarding sleep regulation, cardiovascular function and diuresis-related hormones and neurotransmitters. RESULTS: Of an initial total of 646 articles, 45 studies were finally selected for data extraction according to inclusion criteria, published between 1960 and 2022. Of these studies 26 reported on sleep regulation, 10 on cardiovascular functions and 12 on ANS-associated hormones and neurotransmitters. Evidence on parasympathetic or sympathetic overstimulation in enuretic individuals is suggesting that NE could be attributed to a dysregulation of ANS. Sleep studies have shown increased rapid eye movement sleep time in polyuric enuretic children pointing to sympathetic overactivity, whereas patients with overactive bladder have non-rapid eye movement related enuretic episodes, potentially associated with parasympathetic stimulation. Twenty-four-hour blood pressure monitoring demonstrated "non-dipping" phenomenon, suggesting sympathetic involvement, whereas heart-rate analysis showed parasympathetic hyperfunction. Nocturnal lower levels of arginine-vasopressin, angiotensin II and aldosterone in polyuric children with NE as compared to non-polyuric and controls and potential involvement of dopamine and serotonin in sleep and micturition suggest that ANS-associated hormones and neurotransmitters have a role in the pathogenesis of NE. CONCLUSION: Summarizing the existing data we suggest that ANS dysregulation related either with sympathetic or parasympathetic overactivity may provide a unifying model in understanding the pathogenesis of NE in different enuretic subpopulations. This observation provides new insights in future research and new potential treatment options.

The aim of this study was to examine workers' psychological distress during the COVID-19 pandemic as a function of their individual coping, dyadic coping, and work-family conflict. We also tested the moderating role of gender and culture in these associations. To achieve this aim, we run HLM analyses on data from 1521 workers cohabiting with a partner, coming from six countries (Italy, Spain, Malta, Cyprus, Greece, and Russia) characterized by various degrees of country-level individualism/collectivism. Across all six countries, findings highlighted that work-family conflict as well as the individual coping strategy social support seeking were associated with higher psychological distress for workers, while the individual coping strategy positive attitude and common dyadic coping were found to be protective against workers' psychological distress. This latter association, moreover, was stronger in more individualistic countries.

The present study examined the effectiveness of a school-based health education program promoting healthy diet and exercise, named DIEX, implemented on adolescents. The program is based on the Theory of Planned Behavior (TPB) and is supported by life skills training. The recipients of the program were high school students (n = 367; 168 boys and 199 girls) from 14 schools in Greece who attended 10 1 h sessions implemented by their schoolteachers. Data were collected before and after the program implementation through online questionnaires for: a. attitude, intention, subjective norms, and PBC toward healthy eating; b. knowledge about healthy eating; c. exercise behavior and attitude toward the DIEX program; and d. satisfaction with the DIEX program (only post-program). The results showed that there were significant differences among the pre- and post-measures of students' knowledge and behavior about healthy diet, as well as attitudes and satisfaction toward the program's application. The results show that the DIEX program may have improved nutrition behavior variables. Conclusively, a theory-based behavioral intervention with skills training may result in positive behavioral changes for young students during health education in school settings with the assistance of new technologies.

PURPOSE: To determine the effect of surgical instrumentation on pulmonary function after surgery. METHODS: A literature review was performed to identify articles reporting on the effect of scoliosis surgery on the pulmonary function in patients with spinal muscular atrophy (SMA). Data for each patient were extracted from included studies, and a meta-analysis was performed using the values of forced vital capacity (FVC) before and after surgery. RESULTS: A total of 127 articles were reviewed, and ten articles were selected for data extraction according to inclusion criteria. The results of the meta-analysis showed no difference in respiratory function pre-operatively and at last follow-up. Four out of ten studies demonstrated similar results and five studies reported a decrease in pulmonary function after surgery. Only one study showed improvement in lung function. CONCLUSION: At the moment, there is insufficient evidence in the literature to support that spinal surgery can improve respiratory function. According to our meta-analysis study, vital capacity remains either unchanged or the rate of deterioration is decreased after surgery. Nevertheless, these are both considered favorable outcomes taking into account the natural course of the disease with progressive deterioration of pulmonary function over time.

BACKGROUND: Desert dust outbreaks and dust storms are the major source of particulate matter globally and pose a major threat to human health. We investigated the microorganisms transported with desert dust particles and evaluated their potential impact on human health. METHODS: A systematic review of all reports on the association between non-anthropogenic desert dust pollution, dust microorganisms and human health is conducted. RESULTS: In total, 51 articles were included in this review. The affected regions studied were Asia (32/51, 62.7%) followed by Europe (9/51, 17.6%), America (6/51, 11.8%), Africa (4/51, 7.8%) and Australia (1/51, 2.0%). The Sahara Desert was the most frequent source of dust, followed by Asian and American deserts. In 39/51 studies the dust-related microbiome was analyzed, while, in 12/51 reports, the association of desert dust with infectious disease outbreaks was examined. Pathogenic and opportunistic agents were isolated from dust in 24/39 (61.5%) and 29/39 (74.4%) of the studies, respectively. A significant association of dust events with infectious disease outbreaks was found in 10/12 (83.3%) reports. The infectious diseases that were mostly investigated with dust outbreaks were pneumonia, respiratory tract infections, COVID-19, pulmonary tuberculosis and coccidioidomycosis. CONCLUSIONS: Desert dust outbreaks are vehicles of a significant number of pathogenic or opportunistic microorganisms and limited data indicate an association between dust events and infectious disease outbreaks. Further research is required to strengthen the correlation between dust events and infectious diseases and subsequently guide preventive public health measures.

Introduction: Glucose transporter type 1 (Glut1) deficiency syndrome is a treatable neurometabolic disorder characterized by seizures, developmental delay, and hypoglycorrhachia. Due to the rareness and non-specific clinical manifestations, it is usually mis- or underdiagnosed. Case presentation: We report the case of a toddler who presented with afebrile epileptic seizures and abnormal gait. Brain imaging and electroencephalogram were normal. Further investigation of the cerebrospinal fluid revealed hypoglycorrhachia that was the clue to the diagnosis of Glut1 deficiency syndrome and the initiation of treatment with ketogenic diet. Conclusion: Our case highlights the importance of lumbar puncture while investigating a child with epileptic seizures and abnormal gait or developmental delay, in order not to miss treatable neurometabolic conditions, such as Glut1 deficiency syndrome.

RESUMO

This article will explore the extent to which a focus on the 'local' can tell us something meaningful about recent developments in the governance of displaced migrants and refugees. Taking a multi-sited approach spanning cases in the south and north of Europe, we consider how the challenge of housing and accommodation in particular, a core sector of migrant reception and integration, can shed light on the ways local and city level approaches may negotiate, and sometimes diverge from, national level policy and rhetoric. While it can be said that despite variation, local authorities are by definition ultimately 'always subordinate' (Emilsson, Comparative Migration Studies, 3: 1-17, 2015: 4), they can also show evidence of 'decoupling' across geographies of policy delivery (Pope and Meyer, European Journal of Cultural and Political Sociology, 3: 280-305, 2016: 290). This article traces how possible local variations in different European cases are patterned by ground-level politics, local strategic networks, and pre-existing economic resources in a manner that is empirically detailed through the study of housing.

INTRODUCTION: The purpose of the present study was to examine whether self-regulation strategies can further extend the effect of moderate intensity exercise on smoking delay. METHOD: Participants were 40 adult smokers who were randomly assigned into two groups: control and self-regulation. A repeated measures design was adopted including a neutral condition (20 min video) and an exercise condition (20 min moderate intensity exercise). RESULTS: The results showed that smoking delay increased significantly for both groups; however, the increase for the self-regulation group was significantly larger than that of the control group. CONCLUSIONS: The results support the anti-smoking effects of acute exercise; furthermore, they highlight the usefulness of self-regulation strategies, and in particular goal setting, in extending smoking delay. The present findings provide important evidence for the exercise and smoking literature and useful directions for the development of smoking cessation interventions.

PURPOSE: To correlate conventional invasive pressure indexes of pulmonary circulation with pulmonary first-order arterial mean transit time (MTT) and time to peak enhancement (TTP) measured by means of three-dimensional time-resolved magnetic resonance (MR) angiography in patients with combined pulmonary fibrosis and emphysema (CPFE). MATERIALS AND METHODS: The study was institutional review board approved. All subjects involved in the study provided written informed consent. Eighteen patients with CPFE were enrolled in this study. Thirteen healthy individuals matched for age and sex served as control subjects. Three-dimensional time-resolved MR angiography was performed by using a 3.0-T MR imager. Regions of interest (ROIs) were drawn manually on first-order pulmonary arteries. Within the ROIs, signal intensity-versus-time curves reflecting the first pass of the contrast agent bolus in the pulmonary vessels were obtained. MTT and TTP were calculated. Pulmonary arterial pressure and pulmonary capillary wedge pressure were measured with a double-lumen, balloon-tipped catheter that was positioned in the pulmonary artery. The mean pulmonary arterial pressure (mPAP) and the pulmonary vascular resistance (PVR) were determined. RESULTS: MTT and TTP values were prolonged significantly in patients with CPFE compared with those in the control subjects (P < .001). Mean TTP and mean MTT correlated directly with mPAP and PVR index (P < .005). At multiple linear regression analysis, MTT was the only factor independently associated with PVR index and mPAP. CONCLUSION: Three-dimensional time-resolved MR angiography enables determination of pulmonary hemodynamic parameters that correlate significantly with the pulmonary hemodynamic parameters obtained with invasive methods and may represent a complementary tool for evaluating pulmonary hypertension in patients with CPFE.

The purpose was to assess the diagnostic accuracy of a hand-held Doppler ultrasound (US) machine for the bedside detection of liver and vascular abnormalities after liver transplantation in the intensive care unit. The IRB approved this study, and written informed consent was obtained from all patients or the patient's legal representative. Any liver transplant recipient at our institution who needed a bedside Doppler US examination in the intensive care unit was eligible. Patients underwent routine grey-scale, colour, and spectral Doppler US examinations of the liver with a conventional machine, which was taken as the reference method, and with a hand-held machine on the same day. Examinations followed one another and were performed in a blinded fashion by two radiologists. Over a 4-month period, 24 consecutive patients (16 men, median age 54 years old; 16 cadaveric and 8 living related right liver transplantations) underwent 43 examinations with both conventional and hand-held machines. Image quality and overall satisfaction scores of grey-scale were lower with the hand-held than with the conventional machine. The hand-held was similar to the conventional machine for assessing the patency of portal veins, hepatic veins and the IVC in all patients but one. The hand-held machine failed to detect signals in the right branch of the hepatic artery and in the hilum in two and one cases, respectively. There was no abnormal hepatic arterial flow with the conventional machine in any of the patients, and the results were the same with the hand-held machine. Total examination time was significantly longer with the hand-held machine. The hand-held US machine had a high diagnostic accuracy for both parenchymal and vascular analyses compared with a conventional US machine in the bedside assessment of post-liver transplant patients.
