The aim of this work was to value the evolution of environmental Aspergillus contamination in hospital with respect to aeraulic system and bathrooms works. An analysis on levels of air and surface Aspergillus contamination were determined in patient's rooms and various common sites in a ward of an hospital in Genoa in 1999. As high contamination levels were found, in summer 1999 a radical disinfection and revision of the aeraulic system was settled. In spite of these interventions some samples maintained an high contamination level, even if from only two rooms, so a further disinfection of bathrooms and surfaces became necessary. A period of sampling was conducted after this last intervention until November 2002. At the beginning of our surveillance 59% positive samples and about 50% of them with high contamination level (>1000 CFU/m3) were found. After the revision of the aeraulic system the reduction of positive samples was significative (14.2%), besides all the positive samples regarded only two rooms. In the last valuation period, after a further disinfection of bathrooms and surfaces of the above mentioned rooms, all the samples taken resulted with a contamination level lower than 10 CFU/m3. This findings underlines the importance of environmental surveillance looking for all the contaminated sources; in particular the aeraulic and hydraulic system as well as the proximity hospital building yard not sufficiently protected.

In the context of the important changes of a political economic, social and health order upon which the Argentina, Brazil, Paraguay and Uruguay have now embarked in the framework of the commitment undertaken to create instruments for the Common Market of the South America (MERCOSUR), the importance of leprosy for public health in this new region is analysed. In this connection a description is given of the background and reasons which led to the creation of the MERCOSUR Committee for technical cooperation on leprosy, composed of the heads of the national leprosy control programmes of the countries in question, for the purpose of implementing the Protocol of Intention signed by the representatives of the Ministers of member countries, with the object of establishing policies of technical cooperation in activities to control this disease, with a view to attaining the goal proposed by WHO of eliminating leprosy as a public health problem by the year 2000. Using the data contributed by the different programmes, the epidemiological situation existing in this region in December 1995 is outlined, with analysis of certain epidemiological, demographic and operational variables, showing that MERCOSUR has a prevalence of 6.03 per 10,000 population, one of the highest rates in comparison with other WHO regions; a high percentage of cases lost to view (abandons); and a low rate of coverage with multidrug therapy. In this region a total of 33,654 new cases were detected during 1995, of which more than 50% were multibacillary forms, while nearly 10% of them were youngsters of less of 15 years of age. The action carried out jointly among the four countries, the successes achieved and the results to be achieved in the short term are also described.

To investigate the initial stages of recognition of the self idiotype (Id) by T cells, we examined the early increase in cytoplasmic free calcium ([Ca2+]i) occurring in murine CD4+ T cells specific for a model Id, Id315, following their interaction with the Id. The changes in [Ca2+]i were monitored with stopped-flow fluorometry by loading T cells with fura 2, a Ca(2+)-binding fluorescent dye. An increase of [Ca2+]i in the Id-specific T cell line was dependent on the presence of both antigen-presenting cells (APC) and Id315. When T cells were mixed with APC pulsed with M315 for 90 min at 37 C, a significant increase in T cell [Ca2+]i was observed within one second. A pronounced elevation in [Ca2+]i was also observed in T cells after their interaction with APC which had been pulsed for 90 min with VL-315 Id-containing proteins (such as VL-315, L315, Fv-315 or Fab'-315 fragments). In contrast, pulsing APC for 5 min with the VL fragment produced little or no change in the [Ca2+]i. These results suggest that VL must be further processed by APC before it can be recognized by T cells. Indeed, a synthetic VL region peptide (positions 91-108, designated as P18) produced an elevation in T cell [Ca2+]i when mixed with APC without pulsing.
