INTRODUCTION: Teratoma is the most common congenital tumor, but the orbital location is rare. It is composed of tissues from ectoderm, mesoderm, and endoderm. CLINICAL PRESENTATION: Congenital orbital teratoma commonly presents as unilateral proptosis, with rapid growth, leading to exposure keratopathy. DIAGNOSIS: Prenatal ultrasound may detect the orbital mass, computed tomography (CT) scans, and magnetic resonance (MR) imaging are better in demonstrating multilocular cystic and solid mass, without bone erosion. Laboratory tests should include alfa-fetoprotein (AFP) and B-human chorionic gonadotropin (B-HCG), and histopathologically, it contains all three germ cell layers components. The management is surgical removal of the lesion, the mature teratoma has a benign behavior, and the immature has a poor prognostic. We describe a rare case of congenital orbital teratoma with intracranial extension of the lesion, in which was treated with orbital exenteration. After surgery, AFP levels decreased, the middle face displacement has improved and development milestones were appropriate.

BACKGROUND: For more than 30 years, deep brain stimulation (DBS) has been a therapeutic tool for Parkinson's disease (PD) treatment. DBS can ameliorate several motor and non-motor symptoms and improve the patients' quality of life. OBJECTIVES: To analyze the global scientific production of original and review articles on Parkinson's disease treatment using deep brain stimulation. DESIGN AND SETTING: Descriptive, bibliometric study with a quantitative approach. METHOD: The research protocol was conducted in March 2023 using the Web of Science database. Six hundred eighty-four articles were included in the analysis. Data were imported into RStudio Desktop Software, linked to R Software. The Bibliometrix R package, its Biblioshiny web interface, and VOSviewer software were used for the analysis. RESULTS: The international production began in 1998. Movement Disorders is the journal with the largest number of published articles and the most cited. Michael Okun and Andres Lozano are the authors who produced the most in this area. The University of Florida is the most active affiliated institution in Brazil. The United States has the largest number of collaborations and is mainly published by local researchers. In contrast, countries such as the United Kingdom and Canada have a high number of multi-country publications. The 15 most cited studies predominantly investigated subthalamic nucleus stimulation. CONCLUSION: DBS for Parkinson's disease is a relatively novel therapeutic approach, with studies that have expanded over the last twenty-five years. Most scientific production was quantitative and restricted to specialized journals. The United States, Europe, and China held the most articles.

ABSTRACT BACKGROUND: A bibliometric analysis covering only the production of original studies or considering world production until 2022 has yet to be conducted. The creation and advancement of vaccines have also influenced research priorities, demonstrating the need for a new approach to this subject. OBJECTIVES: To analyze worldwide scientific production related to coronavirus disease 2019 (COVID-19) and the aged and to describe what has already been produced. DESIGN AND SETTING: Bibliometric analysis with a quantitative approach. METHOD: The search terms "COVID-19," "SARS-CoV-2," "Aged," and "Elderly" were used to retrieve articles from the Web of Science database. A total of 684 articles were included in the analysis. Data were imported into RStudio Desktop Software and linked to R Software. The Bibliometrix R package and VOSviewer software were used for analysis. RESULTS: Most articles were published in 2020. These were produced by 4,937 authors and published in 379 journals. The keyword most used by the authors was "COVID-19." Publications from 77 countries were obtained. China had the highest article production ranking, and Spain collaborated the most. The articles addressed the implications of the pandemic on the aged, the relationship between vaccination in the aged, and the implications for the disease itself. CONCLUSION: Further research should be conducted, mainly concerning vaccines and vaccination of the aged, owing to the need for and importance of immunization in this risk group, including assessing the long-term effects of vaccines.

RESUMO

This bibliometric analysis aims to analyze the global scientific production of COVID-19 and vaccines. First, a search for scientific articles was performed using the advanced query in the Web of Science™ database, more precisely in its core collection, on 18 February 2023. Data from 7754 articles were analyzed using the Bibliometrix R package and the Biblioshiny application. The evaluated articles were published mainly in 2022 (60%). The scientific journals that published the most about COVID-19 and vaccines were "Vaccines", "Vaccine" and "Human Vaccines & Immunotherapeutics". The University of Oxford was the most productive institution, with the authors of the articles mainly originating from the United States, China and the United Kingdom. The United States, despite having carried out the most significant number of collaborations, published mainly with local researchers. The 15 most cited articles and the KeyWords Plus™ evidenced the focus of the published articles on the safety and efficacy of vaccines against COVID-19, as well as on the evaluation of vaccine acceptance, more specifically on vaccine hesitancy. Research funding came primarily from US government agencies.

RESUMO Objetivo: apreender as representações sociais elaboradas por discentes de enfermagem sobre a prevenção da COVID-19. Método: pesquisa exploratória-descritiva e qualitativa. Os dados foram produzidos por meio de entrevistas com 32 participantes, realizadas no período de setembro de 2021 a julho de 2022. O Cenário do estudo foi uma instituição de ensino superior pública, localizada na região nordeste do Brasil. Os dados foram processados utilizando o software IRaMuTeQ. Resultados: a gênese das representações sociais da prevenção da COVID-19 apontou dois eixos: Organização de ideias sobre as medidas de prevenção da COVID-19; e a Eficiência, confiabilidade e fonte das informações obtidas. Considerações Finais: as representações sociais evidenciaram o saber sobre as medidas de prevenção da COVID-19, oportunizaram o esclarecimento sobre a origem dos saberes dos discentes e de especificidades que consideram importantes. Espera-se que esse estudo possa servir como alicerce para novas pesquisas e/ou intervenções com discentes de enfermagem.


ABSTRACT Objective: to understand the social representations elaborated by nursing students about the prevention of COVID-19. Method: exploratory-descriptive and qualitative research. Data were produced through interviews with 32 participants, conducted in the period from September 2021 to July 2022. The Study Setting was a public higher education institution, located in the northeastern region of Brazil. Data were processed using IRaMuTeQ software. Results: the genesis of the social representations of COVID-19 prevention pointed to two axes: Organization of ideas about COVID-19 prevention measures, and Efficiency, reliability and source of the information obtained. Final Considerations: the social representations showed the knowledge about the prevention measures of COVID-19, provided an opportunity to clarify the origin of the students' knowledge and the specificities they consider important. It is hoped that this study can serve as a foundation for further research and/or interventions with nursing students.

Consideraciones Finales: Las representaciones sociales mostraron el conocimiento sobre las medidas de prevención del COVID-19, proporcionando una oportunidad para aclarar el origen del conocimiento de los estudiantes y las especificidades que ellos consideran importantes. Se espera que este estudio pueda servir de base para futuras investigaciones y/o intervenciones con estudiantes de enfermería.

Abstract Objective To analyze the historical clinical outcomes of children with myelomeningocele (MMC) meeting the criteria for fetal surgery, but who underwent postnatal primary repair. Methods Data from children undergoing postnatal MMC repair between January 1995 and January 2015 were collected from the Neurosurgery Outpatient Clinic's medical records. Children were included if they had ≥1 year of postoperative follow-up andmet the criteria for fetal surgery. The children's data were then stratified according to whether they received a shunt or not. The primary outcome was mortality, and secondary outcomes were educational delays, hospitalization, recurrent urinary tract infections (UTIs), and renal failure. Results Over the 20-year period, 231 children with MMC were followed up. Based on clinical data recorded at the time of birth, 165 (71.4%) qualify of fetal surgery. Of the 165 patients, 136 (82.4%) underwent shunt placement. The mortality rate was 5.1% in the group with shunt and 0% in the group without, relative risk (RR) 3.28 (95% confidence interval, 95% CI, 0.19-55.9). The statistically significant RRs for adverse outcomes in the shunted group were 1.86 (95% CI, 1.01-3.44) for UTI, 30 (95% CI, 1.01-537) for renal failure, and 1.77 (95% CI, 1.09-2.87) for hospitalizations. Conclusion Children with MMC qualifying for fetal surgery who underwent shunt placement were more likely to have recurrent UTIs, develop renal failure, and be hospitalized. Since approximately half of the shunt procedures could be avoided by fetal surgery, there is a clinical benefit and a possible financial benefit to the implementation of this technology in our setting.


Resumo Objetivo Analisar os resultados clínicos históricos de crianças commielomeningocele (MMC) com critérios para cirurgia fetal,mas que foram submetidas a cirurgia pós-natal. Métodos Dados de crianças submetidas à correção deMMCpós-natal entre janeiro de 1995 e janeiro de 2015 foram coletados nos prontuários do Ambulatório de Neurocirurgia. Foram incluídas crianças se tivessem ≥ 1 ano de acompanhamento pósoperatório e atendessem os critérios para cirurgia fetal. As informações dessas crianças foram então estratificadas de acordo com se receberam ou não derivação do líquido cefalorraquidiano. O desfecho primário foi a mortalidade e os desfechos secundários foram atrasos educacionais, hospitalização, infecções recorrentes do trato urinário einsuficiência renal. Resultados Durante o período de 20 anos, 231 crianças com MMC foram acompanhadas. Com base nos dados clínicos registrados no momento do nascimento, 165 (71,4%) atendiam critérios para a cirurgia fetal. Dos 165 pacientes, 136 (82,4%) foram submetidos à colocação de derivação do líquido cefalorraquidiano. A taxa de mortalidade foi de 5,1% no grupo com derivação do líquido cefalorraquidiano e 0% no grupo sem risco relativo (RR) 3,28 (intervalo de confiança 95%, IC 95%, 0,19-55,9). Os RRs estatisticamente significativos para resultados adversos no grupo com derivação do líquido cefalorraquidiano foram 1,86 (IC 95%, 1,01-3,44) para infecção do trato urinário, 30 (IC 95%, 1,01-537) para insuficiência renal e 1,77 (IC 95%, 1,09-2,87) para hospitalizações. Conclusão Crianças com MMC com critérios para cirurgia fetal submetidas à colocação de derivação do líquido cefalorraquidiano eram mais propensas a ter infecções recorrentes do trato urinário, desenvolver insuficiência renal e serem hospitalizadas. Como aproximadamente metade dos procedimentos de derivação poderiam ser evitados por cirurgia fetal, há um benefício clínico e um possível benefício financeiro com a implementação dessa tecnologia em nosso meio.

OBJECTIVE: To identify symptoms of COVID-19 in adults in the scientific literature. METHOD: Systematic review of studies published from December 1, 2019 to April 21, 2020 from the Scopus, Web of Science and PubMed databases, in order to answer the following research question: "What are the symptoms caused by COVID-19 in adults?" using the keywords "Symptoms", "Clinical Manifestations", "Coronavirus", "COVID-19". RESULTS: Of the total 105 references, 13 references that addressed the symptoms of COVID-19 were selected. Fever and normal or dry cough were symptoms present in all studies. CONCLUSION: The symptoms identified in adult patients were fever, normal or dry cough, headache, pharyngalgia, dyspnea, diarrhea, myalgia, vomiting, sputum or expectoration, anxiety or chest pain, fatigue, nausea, anorexia, abdominal pain, rhinorrhea, runny nose or nasal congestion, dizziness, chills, systemic pain, mental confusion, hemoptysis, asthma, taste disorder, smell disorder, belching and tachycardia.

BACKGROUND: The pandemic of the new coronavirus has culminated in a scientific race to seek knowledge about this virus and its treatments, vaccines and preventive strategies, in order to reduce its impact on healthcare and economics worldwide. Método Una revisión sistemática realizada en las bases de datos Scopus, Web of Science y PubMed con estudios publicados del 1 de diciembre de 2019 al 21 de abril de 2020, con el fin de responder a la pregunta orientadora: "¿Cuáles son las manifestaciones sintomáticas causada por COVID-19 en adultos?" utilizando las palabras clave: "Síntomas", "Manifestaciones clínicas", "Coronavirus", "COVID-19". Resultados Del total de 105 referencias, se seleccionaron 13 que abordaron las manifestaciones sintomáticas de COVID-19, con fiebre y tos normal o seca presente en todos los estudios. Conclusión Las manifestaciones sintomáticas identificadas en pacientes adultos fueron: fiebre, tos normal o seca, dolor de cabeza, faringalgia, disnea, diarrea, mialgia, vómitos, esputo o expectoración, angustia o dolor en el pecho, fatiga, náuseas, anorexia, dolor abdominal, rinorrea, secreción nasal o congestión nasal, mareos, escalofríos, dolor sistémico, confusión mental, hemoptisis, asma, alteración del gusto, alteración del olfato, eructos y taquicardia.

RESUMO

Objective: to verify in the scientific literature the symptomatic manifestations of COVID-19 in adults. Method: a systematic review carried out in the Scopus, Web of Science and PubMed databases with studies published from December 1, 2019 to April 21, 2020, in order to answer the guiding question: "What are the symptomatic manifestations caused by COVID-19 in adults?" using the keywords: "Symptoms", "Clinical manifestations", "Coronavirus", "COVID19". Results: Of the total of 105 references, 13 were selected that addressed the symptomatic manifestations of COVID-19, with fever and normal or dry cough present in all studies. RESUMO

Much controversy remains on the current management of Chiari I deformity (CID) in children, with many clinical, surgical and ethic-legal implications. The Brazilian Society of Pediatric Neurosurgery (SBNPed, in the Portuguese acronym) has put together a panel of experts to analyze updated published data on the medical literature about this matter and come up with several recommendations for pediatric neurosurgeons and allied health professionals when dealing with CID. Their conclusions are reported herein, along with the respective scientific background.

OBJETIVO: discutir a forma correta de utilização, indicações de uso, eficácia e manejo de cada tipo de máscara, assim como sugerir, conforme evidências científicas e recomendações de órgãos de saúde, alternativas eficazes para aumentar a vida útil deste produto. MÉTODO: revisão narrativa utilizando artigos provenientes das bases de dados PubMed, Scopus e Web of Science, e recomendações de agências de saúde. Realizada em abril de 2020. RESULTADOS: máscaras de proteção respiratória podem ser reutilizadas se estiverem íntegras, limpas e secas. Para aumentar a durabilidade de máscaras cirúrgicas, sugere-se a utilização com papel toalha para absorver a umidade. Máscaras de tecido são recomendadas, desde que sejam lavadas adequadamente para reutilização. CONCLUSÃO: embora necessitem estudos sobre o reaproveitamento de máscaras, com a expansão da COVID-19 e a escassez mundial deste equipamento, torna-se essencial que os profissionais da saúde e a população se mantenham atualizados a respeito das suas recomendações de uso.

MÉTODO: revisión narrativa con utilización de artículos provenientes de las bases de datos PubMed, Scopus e Web of Science, y recomendaciones de agencias de salud, realizada en abril de 2020.Resultados: las mascarillas de protección respiratoria pueden ser reutilizadas siempre que se encuentren limpias y secas, en tanto y en cuanto mantengan su integridad. Para aumentar la durabilidad de las mascarillas quirúrgicas, se sugiere su utilización con papel absorbente para absorber la humedad. Las mascarillas de tela son recomendables, siempre y cuando se laven adecuadamente para reutilización. Conclusión: aunque sean necesarios estudios acerca del reaprovechamiento de los barbijos, con la expansión del COVID-19 y la escasez mundial de este producto, es imprescindible que, tanto los profesionales de la salud como la población en general, se mantengan actualizados sobre sus recomendaciones de uso.


OBJECTIVE: To discuss the correct use, indications for use, effectiveness and handling of the different types of mask, as well as suggest, based on scientific evidence and recommendations from health agencies, effective ways to increase the useful life of this product. METHOD: Narrative literature review using articles from the PubMed, Scopus and Web of Science databases, and recommendations from health agencies conducted in April 2020.Results: Protective respiratory devices (particulate respirators) can be reused if it is clean, dry and their layers and shape are intact. To increase the durability of surgical masks, the use of paper towels to absorb moisture is suggested. Cloth masks are recommended, as long as they are washed properly for reuse. CONCLUSION: Despite the scarce number of studies on the reuse of masks, given the expansion of COVID-19 and the worldwide shortage of this equipment, health professionals and the population must be constantly updated on the recommendations for use of these protective devices.

INTRODUCTION: The surgical treatment of Chiari type 1 (CM1) malformation is controversial and depends largely on the preference of the surgeon. The evolution of neuroimaging resulted in an increased number of asymptomatic patients incidentally diagnosed. PURPOSE: To study retrospectively a population of 24 symptomatic patients with CM1 operated between 1999 and 2017 in which intraoperative ultrasonography (IOUS)-assisted posterior fossa-C1 decompression was used to decide whether the dura mater should be opened (CVD+) or not (CVD). RESULTS: Most of the patients complained of headache or neck pain, 15 had hydrosyringomyelia and 14 had some spinal cord involvement. Patients were categorized in improved, unchanged, or worse according the preoperative signs and symptoms. Overall, 19 patients improved, 3 deteriorated, and 2 remained unchanged. Among these, 4 out 5 had syringohydromyelia. CONCLUSIONS: IOUS-assisted posterior fossa-C1 decompression is our preferred option to treat CM1. Children submitted to intradural procedures, initially or subsequently, had increased postoperative complications. CSF fistula or pseudomeningocele was the major cause of complication. The final result seems to correlate with the preoperative neurological status.

To evaluate the effect of immersion in 3% sodium hypochlorite solution in the resistance to cyclic fatigue of three nickel-titanium (NiTi) rotary file systems, ProTaper Next (PTN), Hyflex CM (CM), and Hyflex EDM (EDM), in a mechanical model featuring axial movement. Ninety instruments of three different NiTi rotary file systems, PTN (size 25, 0.06 taper), CM (25, 0.06), and EDM (25/~, variable taper), were randomly divided according to a 3 × 3 factorial design and tested under dynamic immersion in a 3% NaOCl solution (1 or 5 min) or without immersion, making a total of 9 groups (n = 10). Files were tested in an artificial root canal with 45° angle and 5 mm radius apical curvature being submitted to back-and-forth movements until fracture. Statistical analysis was performed using two-way factorial ANOVA with Bonferroni post-hoc tests, at a significance level of 5%. Instruments were evaluated for reliability using a Weilbull approach. Regardless of the immersion treatment, PTN had on average 1200 ± 178 cycles to fracture, CM had 1949 ± 362, and EDM had 5573 ± 853, which was a significantly different (P < 0.01). The NaOCl immersion promoted a significant reduction in the mean number of cycles to fracture (P = 0.01), and was reflected in a significant reduction of the characteristic life of the instruments of the CM end EDM groups. Within this study conditions, EDM instruments performed better to cyclic fatigue followed by CM and then PTN. Immersion in NaOCl decreased the resistance to cyclic fatigue of all tested instruments, but affected more those manufactured from CM wire.

BACKGROUND: The lack of specific prosthetic-related outcome instruments for Spanish amputees must be addressed. OBJECTIVE: To elaborate a culturally equivalent version of the Prosthesis Evaluation Questionnaire in the Spanish language. STUDY DESIGN: Cross-cultural questionnaire validation. METHODS: Two-step process for cultural adaptation: forward and backward translations of English original and Spanish translated versions; assessment of both construct and criterion validity and reliability in a group of vascular amputees. RESULTS: A total of 61 patients were recruited, 44 men (72.1%) and 17 women (27.9%), with a median age of 71.1 years (standard deviation: 7.7 years; range: 51-87 years). In the Prosthesis Evaluation Questionnaire-Spanish, the lowest scores were for gait and frustration, and the highest scores were for noise and stump health. Internal consistency of the questionnaire was acceptable (>0.70) for four of the scales used in the Prosthesis Evaluation Questionnaire but poor (<0.50) for the scales relating to appearance and stump health. Correlations with the quality-of-life levels as measured by the Short Form-36 were positive and mostly significant. CONCLUSION: Prosthesis Evaluation Questionnaire-Spanish could assess the quality of life in patients who have undergone vascular amputations and then been fitted with a prosthetic limb. The questionnaire shows adequate criteria validity when compared with other instruments for measuring quality of life. CLINICAL RELEVANCE: The Prosthesis Evaluation Questionnaire-Spanish could be a valid and reliable instrument for assessing adaptation to prostheses in vascular amputees. The questionnaire adds information relevant to the patient and the physician and may identify cases with poor expected adaptation to the prosthesis.
