RESUMO

Immersive media, particularly Extended Reality (XR), is at the forefront of revolutionizing the healthcare industry. Healthcare provides XR with "silver bullet" use cases that add value and societal effect to the technology. Healthcare interventions frequently require imaging or visualization to be applied correctly, and the sensation of presence that XR can provide is crucial as a training aid for healthcare learners. From anatomy to surgical training, multimodal immersion in the reality of a medical situation increases the impact of an XR resource compared to the usual approach. Thus, healthcare has become a specialized focus for the immersive media sector, with a multitude of development and research underway. This research subject, which followed on from the previous one, yielded an eclectic group of works spanning the gamut of immersive media applications in healthcare. The underlying theme in these works remains a consistent focus on calibrating, validating, verifying, and standardizing procedures, instruments, and technologies in order to constantly rigorously streamline the means and materials that will integrate immersive technologies in healthcare. In that spirit, we share the findings from this research topic as a motivator for rigorous and evidence-based use of immersive media in digital and connected health.

The immunoglobulin heavy-chain variable region gene (IgHV) mutational status is considered the gold standard of prognostication in chronic lymphocytic leukemia (CLL) and is currently determined by Sanger sequencing that allows the analysis of the major clone. Using next-generation sequencing (NGS), we sequenced the IgHV gene from two independent cohorts: (A) 270 consecutive patient samples obtained at diagnosis and (B) 227 patients from the UK ARCTIC-AdMIRe clinical trials. Using complementary DNA from purified CD19+CD5+ cells, we demonstrate the presence of multiple rearrangements in independent experiments and showed that 24.4% of CLL patients express multiple productive clonally unrelated IgHV rearrangements. On the basis of IgHV-NGS subclonal profiles, we defined five different categories: patients with (a) multiple hypermutated (M) clones, (b) 1 M clone, (c) a mix of M-unmutated (UM) clones, (d) 1 UM clone and (e) multiple UM clones. In population A, IgHV-NGS classification stratified patients into five different subgroups with median treatment-free survival (TFS) of >280(a), 131(b), 94(c), 29(d), 15(e) months (P<0.0001) and a median OS of >397(a), 292(b), 196(c), 137(d) and 100(e) months (P<0.0001). In population B, the poor prognosis of multiple UM patients was confirmed with a median TFS of 2 months (P=0.0038). In conclusion, IgHV-NGS highlighted one quarter of CLL patients with multiple productive IgHV subclones and improves disease stratification and raises important questions concerning the pre-leukemic cellular origin of CLL.

Molecular vibrations and electron-vibrational interactions are central to the control of biomolecular electron and energy-transfer rates. The vibrational control of molecular electron-transfer reactions by infrared pulses may enable the precise probing of electronic-vibrational interactions and of their roles in determining electron-transfer mechanisms. This type of electron-transfer rate control is advantageous because it does not alter the electronic state of the molecular electron-transfer system or irreversibly change its molecular structure. For bridge-mediated electron-transfer reactions, infrared (vibrational) excitation of the bridge linking the electron donor to the electron acceptor was suggested as being capable of influencing the electron-transfer rate by modulating the bridge-mediated donor-to-acceptor electronic coupling. This kind of electron-transfer experiment has been realized, demonstrating that bridge-mediated electron-transfer rates can be changed by exciting vibrational modes of the bridge. Here, we use simple models and ab initio computations to explore the physical constraints on one's ability to vibrationally perturb electron-transfer rates using infrared excitation. These constraints stem from the nature of molecular vibrational spectra, the strengths of the electron-vibrational coupling, and the interaction between molecular vibrations and infrared radiation. With these constraints in mind, we suggest parameter regimes and molecular architectures that may enhance the vibrational control of electron transfer for fast coherent electron-transfer reactions.

AIM: The aim of the study was to assess the acute effect of front crawl sprint resisted swimming on the inter-arm coordination and the fluctuation of the swimming velocity. METHODS: Nine female swimmers swam four all-out trials of 25 m, without and with low, moderate and high added resistance. Four camcorders were used to record the underwater movement. The inter-arm coordination was quantified using the index of coordination and the hip intra-cyclic velocity variation was analysed by calculating the coefficient of variation and the difference between the maximum and minimum velocities normalized for the mean stroke cycle velocity. RESULTS: One-way repeated measures analyses of variance showed that swimmers change their inter-arm coordination from a "catch up" toward an "opposition" mode. Concerning the hip horizontal intra-cyclic velocity variation, it was increased significantly during resisted swimming. CONCLUSION: These findings indicate that although resisted swimming cause an increase in the index of coordination and consequently it could be speculated a better propulsive continuity, the velocity fluctuations of the hip within a stroke cycle are not decreased, probably due to the inability of the swimmers to overcome effectively the concrete added resistances used in this study. Thus, at least concerning its acute effect, the hypothesis that the sprint resisted swimming could led to a technical improvement can not be satisfied.

AIM: The aim of this study was to examine the changes of maximal inspiratory mouth pressure (PImax) during a 400-m front crawl swimming trial. METHODS: Eleven well-trained competitive swimmers (age: 17.6+/-0.8 years, mean+/-SE) performed a 400-m front-crawl trial with maximum effort (296.2+/-4.76 s). Then, on different days they swam a 300-m, 200-m and 100-m trials at a velocity corresponding to the 400-m trial. Measurements of PImax from residual volume at upright body position before and immediately after each trial were conducted using a portable mouth pressure meter. RESULTS: Maximal inspiratory mouth pressure did not change significantly after the 100-m and 200-m trial compared to baseline (131.8+/-9.7 and 123.7+/-10.3 vs. 140.9+/-8.9 cmH(2)O, P>0.05). However, PImax was significantly lower after the 300-m (118.8+/-7 cmH(2)O, P=0.02) and 400-m trials (118.1+/-9.9 cmH(2)O, P=0.01). CONCLUSIONS: These results indicate that during a maximum effort of 400-m front crawl, the reduction of inspiratory muscle strength occurs after 300-m. This should be considered for competitive swimming training by implementing swim race distance-specific respiratory muscle training.

Complex interventional radiology (IR) procedures contribute an increasing percentage of the overall medical radiation exposure of the population making accurate dosimetry a challenge. Magnetic resonance (MR) based polymer gel dosimetry has been widely employed in complex dosimetric problems in radiotherapy. The aim of this note is to investigate the feasibility of normoxic gel dosimetry in IR. Dose response, energy dependence and dose rate dependence were investigated in irradiation set-ups relevant to IR for a particular normoxic gel, based on methacrylic acid (MAA) as the monomer and including tetrakis-hydroxy-methyl-phosphonium chloride (THPC) as antioxidant. The gel presents a linear dose response beyond a 25 cGy threshold. No significant energy dependence was observed in the useful range of interventional radiology (80-110 kVp). A linear correlation between the gel response and dose rate was observed in the range of dose rates relevant to IR (5-8 cGy min(-1)). These results demonstrate a reduction of gel sensitivity at very low dose rate levels. A possible explanation of this effect is suggested.

The aim of this research was to study the effect of hand paddles on the pitch, the sweepback angles and other stroke kinematic characteristics of the hand during front crawl swimming. Ten female competitive swimmers swam without and with small (116 cm (2)) and large (268 cm (2)) paddles. Four cameras (60 Hz) were used to record the underwater strokes and the digitizing was undertaken using the Ariel Performance Analysis System. When the size of the paddles increased, the stroke length, the mean swimming velocity and the total duration of the stroke were significantly increased (p < 0.05), while the average velocity of the hand during the pull and the push phases were significantly decreased (p < 0.05). The stroke rate was decreased significantly (p < 0.05) only when using large paddles. The relative duration of the separate phases of the stroke, the magnitude of the medial-lateral displacements and the pitch and sweepback angles of the hand were not modified, indicating that the use of hand paddles did not caused significant alterations in the orientation and the movement of the hand during the underwater stroke in front crawl swimming.

Vineyard surveys of 11 wine producing grape cultivars, were carried out in sixteen vineyards, in five winemaking regions in Greece, during 2002 and 2003. The occurrence of various Aspergillus spp. in bunches of berries at setting, veraison and ripening at harvest time was investigated. Aspergillus niger aggregate and A. carbonarius were predominantly isolated from sampled berries. Although the prevailing Aspergillus spp. isolates belonged to A. niger aggregate, isolates of A. carbonarius were the most efficient ochratoxin A (OTA) producers. Of 50 tested isolates of A. carbonarius 42% produced amounts of OTA, exceeding 25 ppb, while none of the 85 isolates of A. niger aggregate tested produced above 16 ppb.

BACKGROUND: Segmental colitis associated with diverticulosis (SCAD) has been defined as chronic colonic inflammation surrounding diverticula with rectal sparing. Distinguishing this condition from inflammatory bowel disease may be difficult. Our aim was to evaluate the epidemiological and clinical characteristics of SCAD in our area. METHODS: Retrospective case identification with prospective follow-up was done. Patients with endoscopic findings suggestive of SCAD were enrolled. The epidemiological, clinical, and histological characteristics of these patients were analyzed. RESULTS: Out of 605 patients with diverticulosis, 23 cases of SCAD were identified (3.8%). Four patients had histological characteristics suggestive of ulcerative colitis, in 1 case the histology was suggestive of ischemic colitis, 6 patients had histology compatible with SCAD, and the remaining patients had either transitional mucosa or minimal lesions. Four cases were refractory to conservative treatment (mesalamine and antibiotics) and surgery was required. No cases of extension of colonic inflammation in diverticula-free areas were found. CONCLUSIONS: Segmental colitis associated with diverticulosis is not a rare disorder. It may occur with a spectrum of clinical and histologic features and may be confused with ulcerative colitis. The majority of the cases respond to medical therapy with antibiotics and/or mesalamine, whereas few cases are refractory and need surgery. No evolution to inflammatory bowel disease was observed.

AIM: The aim of the present study was compare linear kinematics and energy changes of the barbell, along with the angular kinematics of the leg movement during the snatch technique, between male adolescent and adult weightlifters. METHODS: Two S-VHS cameras operating at 60 fields per second recorded the heaviest lifts of 14 male adolescent and 9 adult top-level weightlifters under competitive conditions. The spatial co-ordinates of selected points on the body and the barbell were calculated using the direct linear transformation procedure. A low pass digital filter with 4 Hz cut-off frequency was used for the smoothing of raw co-ordinate data. The ''t''-test for independent samples was used for the statistical treatment of data. RESULTS: The results revealed that there were no significant differences between the adolescent and the adult weightlifters in the majority of the kinematic variables. However, adolescent weightlifters extended their knees significantly slower (t21 = 4.211, p < 0.05) during the 1st pull and their ankles during the 2nd pull (t21 = 2.440, p < 0.05) than the adult weightlifters did. Moreover, the average relative power output was significantly greater for the adult weightlifters during both the 1st (t21 = 2.303, p < 0.05) and the snd pull (t21 = 2.611, p < 0.05). CONCLUSION: These results indicate that the adolescent weightlifters were characterised from a high level of snatch technique and differentiated from the adults predominantly at the less powerful execution of the movement.

The complete sequence of the nuclear ribosomal DNA gene complex of the phytopathogenic fungus Verticillium dahliae has been determined. The tandemly repeated unit was 7216 bp long and appears to be the shortest rDNA cluster described so far among filamentous fungi. Primer pairs were designed for amplification of the region spanning half of the 28S subunit, the intergenic spacer (IGS), and the 5' end of 18S subunit of a number of Verticillium strains, isolated from various hosts and geographic origins. Great heterogeneity was detected in the amplified products of the IGS region resulting in fragments varying from 1.6 to 2.0 kb. The majority of Verticillium isolates were classified into two groups with 1.6- and 1.7-kb amplified products, respectively. The former group included 31 V. dahliae, 7 V. longisporum, and 1 V. albo-atrum isolates, whereas the latter included 10 V. dahliae and 1 V. albo-atrum isolates. Sequence analysis of representative PCR products of the above groups identified a "hot-spot" region harboring most of larger insertions, whereas most of the small changes were due to transitions and transversions. One V. longisporum isolate with a 2.0-kb PCR product contained 13 perfectly conserved tandem repeats of 39 bp long. The presence of similar incomplete sequences in the corresponding regions of V. dahliae, V. longisporum, and V. albo-atrum isolates revealed a particular standard motif of insertions in the IGS region of the genus and is discussed.
