RESUMO

This study aimed to assess platelets as a possible model for screening the accumulation of mitochondrial DNA mutations, particularly during normal ageing. For this purpose we isolated platelets from young and old donors selected by lack of systemic and haematological diseases. We studied the accumulation of a particular deletion (4977-bp deletion) that usually accumulates in an age-related manner in different post-mitotic tissues, such as brain, heart and skeletal muscle, and in some non-post-mitotic tissues (skin, liver). Using different primers, we failed to detect this particular species of deletion in platelets both from young and old individuals. However, we cannot exclude the presence of other species of deletions or point mutations affecting the mitochondrial DNA in platelets during the aging process.

Despite the many instruments available for assessing elderly people, there is a need for additional methods to measure mental decline that would also be applicable in cross sectional and longitudinal studies. With this purpose in mind, our group developed and checked a new instrument, the Index of Mental Decline (IMD), which consists of five clusters of items intended for the assessment of cognition, personal interrelationships, affective disorders, apathy and somatic complaints. To improve its consistency, all clusters and items were evaluated individually, according to their clinical impact. Three levels of symptom importance were determined: absent to very mild, mild to moderate, severe to very severe. Inter-rater reliability and test-retest reliability were demonstrated in a sample of 59 subjects, and proved to be satisfactory. The validity of the IMD was tested in a group of 203 patients, in whom a clinical diagnosis of probable dementia (DSM III-R criteria) had been formulated. The results suggest the effectiveness of the IMD both in quantifying mental decline and monitoring clinical symptoms. The IMD cannot be the first step of diagnostic procedure, but it can be useful for evaluating mental decline in elderly subjects with cognitive disorders. In longitudinal studies, the presence of the same caregiver or informant is compulsory.

RESUMO

1. In a multicenter, placebo-controlled, double-blind clinical trial in 156 elderly patients with psychopathologic symptoms, glycosaminoglycan polysulfate was found to be a therapeutically effective agent in the treatment of the earliest manifestations of a dementing process. 2. Treatment with glycosaminoglycan polysulfate in the daily dosage of 600 LRU, administered on the basis of a divided dosage schedule for 24 weeks, was significantly superior to an inactive placebo on several outcome measures including the SCAG Total and factor scores (i.e., Cognitive Dysfunction, Withdrawal, Agitation/Irritability and Depression), the NOWLIS Total and Fatiguability factor scores, the MMSE, the HAM-D Total and Vegetative Symptoms factor score and the CGI Severity of Illness and Global Improvement. 3. The drug was well tolerated; vital signs and laboratory measures did not show clinically significant changes within the experimental period.

RESUMO

The diabetic disease is one of the main causes of morbidity and mortality in the societies at high industrial development. In the first part, are treated the epidemiologic and physiopathologic aspects of it, with special attention to the peculiarities of the senile diabetes mellitus. Beside, is examined the therapy of the diabetes in elderly people. The classical pharmacologic therapy is discussed and suggested with the other therapeutic means represented by the physical activity and by the diet. The last part is dedicated to the problems that an elderly diabetic can have during the dental treatments, by discussing the possible acceptable cures.
