RESUMO

Abstract Pancreatic trauma is a rare but potentially lethal injury because often it is associated with other abdominal organ or vascular injuries. Usually, it has a late clinical presentation which in turn complicates the management and overall prognosis. Due to the overall low prevalence of pancreatic injuries, there has been a significant lack of consensus among trauma surgeons worldwide on how to appropriately and efficiently diagnose and manage them. The accurate diagnosis of these injuries is difficult due to its anatomical location and the fact that signs of pancreatic damage are usually of delayed presentation. The current surgical trend has been moving towards organ preservation in order to avoid complications secondary to exocrine and endocrine function loss and/or potential implicit post-operative complications including leaks and fistulas. The aim of this paper is to propose a management algorithm of patients with pancreatic injuries via an expert consensus. Most pancreatic injuries can be managed with a combination of hemostatic maneuvers, pancreatic packing, parenchymal wound suturing and closed surgical drainage. Distal pancreatectomies with the inevitable loss of significant amounts of healthy pancreatic tissue must be avoided. General principles of damage control surgery must be applied when necessary followed by definitive surgical management when and only when appropriate physiological stabilization has been achieved. It is our experience that viable un-injured pancreatic tissue should be left alone when possible in all types of pancreatic injuries accompanied by adequate closed surgical drainage with the aim of preserving primary organ function and decreasing short and long term morbidity.


Resumen El trauma pancreático es un tipo de trauma poco común potencialmente fatal que está asociado con lesiones de órganos abdominales o vasculares. Usualmente, los signos clínicos son tardíos aumentado el riesgo de complicaciones respecto al manejo y al pronóstico general. Debido a la baja prevalencia de la lesión del trauma, no existe consenso entre los cirujanos alrededor del mundo sobre cómo se debe diagnosticar y tratar adecuadamente este desafío quirúrgico. La precisión en el diagnóstico es difícil por la localización anatómica y las manifestaciones clínicas tardías. El abordaje quirúrgico ha ido cambiando de dirección hacia la preservación del órgano para evitar complicaciones secundarias asociada a la perdida de la función exocrina y endocrina, o de potenciales complicaciones postquirúrgicas incluyendo las dehiscencias y fistulas. El objetivo de este artículo es proponer un algoritmo de manejo del trauma pancreático a través de un consenso de expertos. Las lesiones del páncreas pueden ser manejadas con una combinación de maniobras hemostáticas, empaquetamiento pancreático, sutura de la herida y drenaje quirúrgico cerrado. La pancreatectomía distal con la perdida de tejido vital pancreático debe ser evitadas. Los principios generales de la cirugía de control de daños deben ser aplicados cuando sea necesario para un manejo quirúrgico definitivo cuando y solo cuando la estabilización fisiológica haya sido lograda. En nuestra experiencia, el tejido pancreático sano debe preservarse cuando el trauma se asocia de un manejo mediante un drenaje quirúrgico cerrado con el objetivo de preservar la función primaria del órgano y disminuir a corto y largo tiempo las morbilidades.

BACKGROUND: Trauma information systems are needed to improve decision making and to identify potential areas of intervention. OBJECTIVE: To describe the first year of experience with a trauma registry in two referral centers in southwest Colombia. METHODS: The study was performed in two referral centers in Cali. Patients with traumatic injuries seen between January 1 and December 31, 2012, were included. The collected information included demographics, mechanism of trauma, injury severity score (ISS), and mortality. A descriptive analysis was carried out. RESULTS: A total of 17,431 patients were registered, of which 67.8% were male with an average age of 30 (±20) years. Workplace injuries were the cause of emergency consultations in 28.2% of cases, and falls were the most common mechanism of trauma (37.3%). Patients with an ISS ≥15 were mostly found in the 18-35-year age range (6.4%). Most patients who suffered a gunshot wound presented an ISS ≥15. A total of 2.5% of all patients died, whereas the mortality rate was 54% among patients with an ISS ≥15 and a gunshot wound. CONCLUSION: Once the trauma registry was successfully implemented in two institutions in Cali, the primary causes of admission were identified as falls and workplace injuries. The most severely compromised patients were in the population range between 18 and 35 years of age. The highest mortality was caused by gunshot wounds. OBJETIVO: Describir el primer año de experiencia del registro de trauma en dos centros de referencia de trauma del suroccidente Colombiano. MÉTODOS: Estudio realizado en dos centros de referencia de Cali. Se incluyeron pacientes con trauma o lesiones externas entre el 1-Ene y el 31-Dic-2012. Se recolectó información demográfica, relacionada con mecanismos de trauma, severidad (ISS) y mortalidad. Se presenta un análisis descriptivo. RESULTADOS: Se registraron 17,431 pacientes. El 67.8% de los pacientes eran de género masculino con edad promedio de 30 (±20) años. Las lesiones laborales fueron causa de consulta a urgencias en un 28.2%, y las caídas el mecanismo de trauma más frecuente (37.3%). Los pacientes con ISS ≥15 en su mayoría se encontraban en el rango de edad de 18-35 años (6.4%). El 28% de los pacientes que sufrieron lesión por arma de fuego presentaron un ISS ≥15. El 2.5% de los pacientes murieron y aquellos pacientes con ISS ≥15 y lesión por arma de fuego presentaron mortalidad del 54%. CONCLUSIÓN: una vez se logró implementar el registro de trauma en dos instituciones en Cali, se identificó que la principal causa de ingreso fue secundaria a caídas y las lesiones laborales. Los pacientes más severamente comprometidos están en el rango de población entre 18 a 35 años. La mayor mortalidad se presentó secundario a lesiones de causa externa por arma de fuego.

Attention-deficit/hyperactivity disorder (ADHD) is commonly known to occur during childhood, characterized by excessive inattention and/or hyperactivity and impulsivity as well as lack of emotional self-control and motivation. The prevalence of adult ADHD in the general population is estimated to be between 2-5%. The aim of this review is to provide an overview regarding current controversies related to ADHD within the adult population. The concept of ADHD in adults has been questioned and criticized by professionals over the last years. Overall, adult ADHD is well evidenced based on epidemiological data, genetic data, neuroimaging, psychosocial impairment and treatment effectiveness. Although, research within this field has been significantly improved, suggestions for future research are provided, in order to be able to clarify the remaining questions regarding this disorder throughout adulthood. One of the most important changes to be made in the near future should be to increase educational training on ADHD in adults.

Nutritional value of seafood for human consumption is worldwide recognized. Some information have been generated in other countries, nevertheless, there is limited information describing the chemical composition of some fishery important species caught in the Gulf of Nicoya. For this reason, we studied the levels of proximal components of the edible parts (fresh) of three commercially important species. The meat samples of snook Centropomus unionesis, the shrimp Trachypenaeus byrdi and the bivalve Polymesoda radiata, were collected from the Puntarenas local fish market during the fishing season of February 2009 to January 2010. Proximate composition analysis was determined according to AOAC methodology, and evaluated the moisture content, and protein and lipid composition of shellfish meats. The results indicated that the moisture content ranged from 74.6-80.6g/100g for snook 76.9-80.0g/100g for shrimp and 77.9-89.5g/100g for green mussel. After the moisture, the protein was the most abundant chemical fraction (6.8 to 21g/100g) showing the highest values in February for the shrimp and green mussel, and December for snook. The largest fluctuations in the lipid content were found in the snook, ranging from 0.7g/100g to 5.6g/100g; the highest values in this fraction were found in shrimp, green mussel and snook, for July, February and April samples respectively. Considering these results, we concluded that fish and shrimp species studied are a good alternative for human consumption as a source of protein and low lipid content.

Delusions are the persistent and often bizarre beliefs that characterise psychosis. Previous studies have suggested that their emergence may be explained by disturbances in prediction error-dependent learning. Here we set up complementary studies in order to examine whether such a disturbance also modulates memory reconsolidation and hence explains their remarkable persistence. First, we quantified individual brain responses to prediction error in a causal learning task in 18 human subjects (8 female). Next, a placebo-controlled within-subjects study of the impact of ketamine was set up on the same individuals. We determined the influence of this NMDA receptor antagonist (previously shown to induce aberrant prediction error signal and lead to transient alterations in perception and belief) on the evolution of a fear memory over a 72 hour period: they initially underwent Pavlovian fear conditioning; 24 hours later, during ketamine or placebo administration, the conditioned stimulus (CS) was presented once, without reinforcement; memory strength was then tested again 24 hours later. Re-presentation of the CS under ketamine led to a stronger subsequent memory than under placebo. Moreover, the degree of strengthening correlated with individual vulnerability to ketamine's psychotogenic effects and with prediction error brain signal. This finding was partially replicated in an independent sample with an appetitive learning procedure (in 8 human subjects, 4 female). These results suggest a link between altered prediction error, memory strength and psychosis. They point to a core disruption that may explain not only the emergence of delusional beliefs but also their persistence.

La información disponible sobre la composición química de algunos recursos pesqueros que se capturan en el Golfo de Nicoya es muy limitada. Por tal razón se determinaron los niveles de los componentes proximales de las partes comestibles (fresco) de tres especies de importancia comercial. Las muestras fueron adquiridas en el Mercado de Puntarenas entre febrero 2009 y enero 2001 durante la temporada de pesca. Los análisis de la composición proximal se determinaron según la metodología de AOAC. Los resultados indicaron que el contenido de humedad varió entre 74.6-80.6g/100g en robalo (Centropomus unionesis), 76.9-80.0g/100g en camarón (Trachypenaeus byrdi) y 77.9-89.5g/100g en almeja verde (Poymesoda radiata). Después de la humedad la fracción química más abundante fue la proteína presentando los valores más altos en diciembre para el robalo y febrero para el camarón y la almeja verde. Las mayores fluctuaciones en el contenido de lípidos se presentaron en el robalo, variando desde 0.7g/100g hasta 5.6g/100g. Los valores más altos en esta fracción fueron encontrados en julio, febrero y abril en camarones, almeja y robalo respectivamente. Se concluye que la especie de pescado y camarón estudiados son una buena alternativa para el consumo humano por ser una fuente importante de proteínas y por su bajo contenido en lípidos.

INTRODUCTION: Aberrant experience of agency is characteristic of schizophrenia. An understanding of the neurobiological basis of such experience is therefore of considerable importance for developing successful models of the disease. We aimed to characterise the effects of ketamine, a drug model for psychosis, on sense of agency (SoA). SoA is associated with a subjective compression of the temporal interval between an action and its effects: This is known as "intentional binding". This action-effect binding provides an indirect measure of SoA. Previous research has found that the magnitude of binding is exaggerated in patients with schizophrenia. We therefore investigated whether ketamine administration to otherwise healthy adults induced a similar pattern of binding. METHODS: 14 right-handed healthy participants (8 female; mean age 22.4 years) were given low-dose ketamine (100 ng/mL plasma) and completed the binding task. They also underwent structured clinical interviews. RESULTS: Ketamine mimicked the performance of schizophrenia patients on the intentional binding task, significantly increasing binding relative to placebo. The size of this effect also correlated with aberrant bodily experiences engendered by the drug. CONCLUSIONS: These data suggest that ketamine may be able to mimic certain aberrant agency experiences that characterise schizophrenia. The link to individual changes in bodily experience suggests that the fundamental change produced by the drug has wider consequences in terms of individuals' experiences of their bodies and movements.

BACKGROUND: Our sense of body ownership is profound and familiar, yet it may be misleading. In the rubber-hand illusion, synchronous tactile and visual stimulation lead to the experience that a rubber hand is actually one's own. This illusion is stronger in schizophrenia. Given the evidence that ketamine, a noncompetitive N-methyl-D-aspartate antagonist reproduces symptoms of schizophrenia, we sought to determine whether the rubber-hand illusion is augmented by ketamine. METHODS: We studied 15 healthy volunteers in a within-subjects placebo-controlled study. All volunteers carried out two versions of the rubber-hand task, each under both placebo and ketamine infusions. In one task, they saw a rubber hand being stroked in synchrony with tactile stimulation of their real, hidden hand. In the other, stroking of the real and rubber hands was asynchronous. We recorded subjective changes in sense of ownership, as well as participants' ability to localize their hidden hand. RESULTS: Ketamine was associated with significant increases in subjective measures of the illusion and in hand mislocalization. Although asynchronous visuotactile stimulation attenuates the strength of the illusion during both placebo and ketamine, there remained a significant illusory effect during asynchronous visuotactile stimulation under ketamine compared with placebo. The strength of the illusion during asynchronous visuotactile stimulation correlated with other subjective effects of the drug. CONCLUSIONS: Ketamine mimics the perturbed sense of body ownership seen in schizophrenia, suggesting that it produces a comparable alteration in integration of information across sensory domains and in the subjective and behavioral consequences of such integration.

Reproduction and growth of the fish Lutjanus guttatus (Pisces: Lutjanidae) in the Gulf of Nicoya, Costa Rica. The fish Lutjanus guttatus is important in the fisheries of Golfo de Nicoya, Costa Rica where they are captured with varied gear mainly by the artisanal fleet. We measured specimens from the commercial catch of 2002-2006. Gonadal state was determined macroscopically and age with otoliths The total length-total weight relationship was y = 0.0236x(2.8153) and total length-eviscerated weight y = 0.0216x(2.8129). Simplified relationships were y = 0.0173x3 and y = 0.0162x3. There is year-round reproduction with peaks in March (dry season) and September (rainy season). Male-female sexual ratio was 1:1. The age-length key for the gulf shows availability until the 6 years of age. The von Bertalanffy growth curve is L(t) = 65,9 (1-e(-0.13(t+2,66))).

Una de las pesquerías más importantes en el Golfo de Nicoya es la del pargo mancha (Lutjanus guttatus); su captura se realiza con diversas artes y principalmente por la flota artesanal. Estudiamos especímenes de la captura comercial artesanal entre el 2002 y 2006. Se analizaron las relaciones morfométricas para aplicar la ecuación P = aLb; el análisis del estado de madurez gonadal se hizo macroscópicamente; se determinó la edad a través de la lectura de anillos anuales de los otolitos sagitales y los parámetros de crecimiento fueron aplicados en la ecuación de von Bertalanffy: L (t) = * [1-exp (-K*(t-t0))]. La relación longitud total-peso total es y = 0.0236x 2.8153 y longitud total-peso eviscerado y = 0.0216x2.8129; las relaciones simplificadas correspondientes fueron y = 0.0173x3 y y = 0.0162x3. La reproducción es todo el año con dos picos en marzo y setiembre (época seca y lluviosa, respectivamente). La proporción macho-hembra es 1:1. Se generó para la población del Golfo de Nicoya una clave edad-talla donde su utilidad se presenta como adecuada hasta los 6 años de edad. La curva de crecimiento von Bertalanffy es L (t)=65,9(1-e -0,13(t+2,66)).

The Arabidopsis (Arabidopsis thaliana) inositol polyphosphate 6-/3-kinase gene (AtIpk2beta) is known to participate in inositol phosphate metabolism. However, little is known about its physiological functions in higher plants. Here, we report that AtIpk2beta regulates Arabidopsis axillary shoot branching. By overexpressing AtIpk2beta in the wild type and mutants, we found that overexpression of AtIpk2beta leads to more axillary shoot branches. Further analysis of AtIpk2beta overexpression lines showed that axillary meristem forms earlier and the bud outgrowth rate is also accelerated, resulting in more axillary shoot branches. The AtIpk2beta promoter/beta-glucuronidase (GUS) fusion (AtIpk2betaGUS) expression pattern is similar to that of the auxin reporter DR5GUS. Moreover, AtIpk2beta can be induced in response to exogenous indole-3-acetic acid (IAA) treatments. In addition, AtIpk2beta overexpression plants exhibit IAA-related phenotypes and are more resistant to exogenous IAA treatments. Further analysis employing reverse transcription-polymerase chain reaction shows that some genes, including auxin-biosynthesis (CYP83B1), auxin-transport (PIN4), and auxin-mediated branching genes (MAX4 and SPS), are regulated by AtIpk2beta. Taken together, our data provide insights into a role for AtIpk2beta in axillary shoot branching through the auxin signaling pathway.

La utilizacion de losas de hormigon pretensado postesados ha ido adquiriendo cada vez mayor importancia en la construccion de entrepisos de edificios en Norteamerica y Australia en los ultimos 15 anos. El estudio se desarrolla con los puntos que se senala: Cap.I: Losas sin vigas de hormigon pretensado, postesadas con cables no adheridos como alternativa de solucion en los entrepisos de edificios. Cap.II: Metodos de calculo de losas sin vigas y su aplicacion en estructuras pretensadas. Cap.III: Analisis del comportamiento de una losa continua sin vigas, postesada en dos direcciones.
