[How to optimize lymph node dissection in colorectal cancers. A technique for simple and efficacious clarification]. / Comment optimiser la dissection des ganglions lymphatiques dans les carcinomes colorectaux. Une technique de clarification simple et efficace.

Studies relating to the prognosis of colorectal carcinomas insist on the quality of mesocolic examination and on the frequency of metastases in minute lymph nodes. The aim of our study was to evaluate an easy, rapid and new clearance method recently described by Koren et al. Twenty-one surgical specimens for colorectal carcinomas have been investigated. After lymph node examination with the traditional method, the mesocolic fat was immersed in a clearing solution. After 6 hours, lymph nodes stood out as white chalky nodules, and were dissected. By the traditional method 182 lymph nodes were detected and 89 additional lymph nodes after clearing. In one case, the detection of a metastasis in lymph node after clearing allowed to upstage the tumour from Dukes B (N0) to Dukes C (N+). This easy and inexpensive method allows the detection of minute lymph nodes and helps to establish a more accurate staging.

[Renal manifestations of Kimura disease. Apropos of a case]. / Manifestations rénales de la maladie de Kimura. A propos d'un cas.

A nephrotic syndrome with focal sclerosis associated with Kimura's disease is reported in a young asiatic man. The nephrotic syndrome started three years prior to the usual skin tumors and lymph nodes involvement. Epidemiological, clinical and nosological characteristics of Kimura's disease are discussed. Renal manifestations are the only visceral localisations of the disease. Thirteen Kimura's disease associated with documented nephrotic syndromes have been reported in german-english literature. They display a wide variety of histologic patterns. Their clinical course do not differ from the primary form of nephrotic syndrome with identical histology.

[Incidental discovery of asymptomatic Loeffler fibroplastic endocarditis. Report of a case]. / Découverte fortuite d'une endocardite fibroplastique de loeffler asymptomatique. A propos d'un cas.

The authors report a case of fibroplastic endocarditis discovered during echocardiography in an asymptomatic patient in the context of assessment of systematized electrocardiographic modifications. The diagnosis, strongly suspected on the basis of the cardiac MRI findings, was finally confirmed by endocardial biopsy.

[Bronchial epidermoid carcinoma with unusual disclosure]. / Carcinome épidermoïde bronchique de révélation inhabituelle.

Dissemination has often occurred before the diagnosis of bronchopulmonary cancer. Pancreatic metastases are exceptional and are very rarely the revealing manifestation. The authors report a case of a tumour of the pancreas which led to the discover of an bronchogenic epidermoid carcinoma. The relationship between these two tumours lead to the proposed hypothesis of pancreatic metastasis of a bronchogenic carcinoma. Although this situation is rare, the preoperative work-up for bronchogenic carcinoma should include computed tomography of the pancreas if the abdominal echography does not allow complete visualization of the pancreas in cases with suggestive digestive disorders. If a tumour is observed in the pancreas, scan-guided needle biopsy should be performed to enable the histological examination.
