RESUMO
A comparative evaluation of the antishock effectiveness of the ablation of intracranial hematomas by means of various surgical accesses was made. The method of mathematical analysis based on the principle of "moving averages was used. It was shown that the potential antishock effectiveness of the ablation of intracranial hematomas was maximum after using cutter openings and minimum after using the osteoplastic access.

A modified method of skeletal traction by the pelvis and bones of the lower extremities was used in 18 patients with the complicated trauma of the thoracic and lumbar parts of the spine. The method was shown to be highly efficient to prevent decubitus and facilitated management of the patients resulting in a simultaneous correction and fixation of the injured segments of the spine.

In order to evaluate severity of the brain trauma the authors used such criteria as the state of the patient's consciousness, systolic arterial pressure level, character of external signs of the brain and skull trauma as well as the patient's response to the antishock therapy used at the prehospital stage.

The authors have performed an analysis of the clinical picture of the associated cranio-cerebral trauma accompanied by shock in 181 patients. It has been shown that of the greatest diagnostic significance in the period of shock is the assessment of the patient's consciousness, hemodynamics and respiration, the external symptoms of the injury of the brain and skull, investigation of the eye symptoms, motor disturbances, pathological pyramid symptoms.

The indications and contraindications for calling a neurosurgeon to the surgical hospitals for associated cranio-cerebral traumas against the background of shock are considered. Main clinico-neurological manifestations of the cranio-cerebral trauma in comparison with the level of the systolic arterial pressure and state of the patient's consciousness are used as criteria for calling a neurosurgeon.

A method for parametrical prognosis of outcomes of associated cranio-cerebral traumas has been developed on the basis of simple clinical features such as pulse rate, systolic arterial pressure, severity of the trauma expressed in numbers and the level of consciousness. The outcome is prognosticated by the formula obtained as a result of multifactorial regression analysis reliable in 85--90% of cases.
