INTRODUCTION: : Mesh fixation in laparoscopic ventral hernia is accomplished using tacks or tacks with transfascial sutures. This is a painful operation and the pain is believed to be more due to transfascial sutures. We describe a method of transfascial suturing which fixes the mesh securely and probably causes less pain. METHOD: : Up to six ports may be necessary, three on each side. A suitable-sized mesh is used and fixed with tacks all around. A 20G spinal needle is passed from the skin through one corner of the mesh. A 0 prolene suture is passed through into the peritoneum. With the prolene within, the needle is withdrawn above the anterior rectus sheath and passed again at an angle into the abdomen just outside the mesh. A loop of prolene is thus created which is tied under vision using intra-corporeal knotting. CONCLUSION: : This method gives a secure mesh fixation and causes less pain than conventional methods. This technique is easy to learn but needs expertise in intra-corporeal knotting.

KEY POINTS: Cardiac hypertrophy following endurance-training is thought to be due to hypertrophy of existing cardiomyocytes. The benefits of endurance exercise on cardiac hypertrophy are generally thought to be short-lived and regress to sedentary levels within a few weeks of stopping endurance training. We have now established that cardiomyocyte hyperplasia also plays a considerable role in cardiac growth in response to just 4 weeks of endurance exercise in juvenile (5-9 weeks of age) rats. The effect of endurance exercise on cardiomyocyte hyperplasia diminishes with age and is lost by adulthood. We have also established that the effect of juvenile exercise on heart mass is sustained into adulthood. ABSTRACT: The aim of this study was to investigate if endurance training during juvenile life 'reprogrammes' the heart and leads to sustained improvements in the structure, function, and morphology of the adult heart. Male Wistar Kyoto rats were exercise trained 5 days week-1 for 4 weeks in either juvenile (5-9 weeks of age), adolescent (11-15 weeks of age) or adult life (20-24 weeks of age). Juvenile exercise training, when compared to 24-week-old sedentary rats, led to sustained increases in left ventricle (LV) mass (+18%; P < 0.05), wall thickness (+11%; P < 0.05), the longitudinal area of binucleated cardiomyocytes (P < 0.05), cardiomyocyte number (+36%; P < 0.05), and doubled the proportion of mononucleated cardiomyocytes (P < 0.05), with a less pronounced effect of exercise during adolescent life. Adult exercise training also increased LV mass (+11%; P < 0.05), wall thickness (+6%; P < 0.05) and the longitudinal area of binucleated cardiomyocytes (P < 0.05), despite no change in cardiomyocyte number or the proportion of mono- and binucleated cardiomyocytes. Resting cardiac function, LV chamber dimensions and fibrosis levels were not altered by juvenile or adult exercise training. At 9 weeks of age, juvenile exercise significantly reduced the expression of microRNA-208b, which is a known regulator of cardiac growth, but this was not sustained to 24 weeks of age. In conclusion, juvenile exercise leads to physiological cardiac hypertrophy that is sustained into adulthood long after exercise training has ceased. Furthermore, this cardiac reprogramming is largely due to a 36% increase in cardiomyocyte number, which results in an additional 20 million cardiomyocytes in adulthood.

BACKGROUND: Propolis is a natural resinous mixture produced by honeybees, which exhibits anti-microbial, anti-inflammatory, cytostatic and cariostatic properties. The aim of the study was to evaluate the anti-bacterial efficacy of a propolis based dentifrice on Mutans Streptococci colonizing the oral cavity of young patients using Dentocult® SM strip mutans test. METHODS: Screening of 367 male subjects within the age group of 7-12 years was carried out. A total of 30 children were included in the study. They were instructed to use a Propolis dentifrice (Probee,™ Quasi-Medical Products, Seoul Propolis) daily for three minutes over a period of four weeks. Plaque and salivary samples were collected at baseline, 1(st) week, 3(rd) week and 4(th) week and were analyzed for Mutans Streptococci count using Dentocult® SM strip Mutans kit (Orion Diagnostica Oy, Finland). Student paired t-test and Friedman test were used for statistical analysis. RESULTS: It was unveiled that mean Mutans streptococci count at 1(st) week and 4(th) week, showed significant reduction (p≤0.0001), compared to baseline scores. Using Friedman's test, statistically significant difference was found between baseline and 1(st) week, 3(rd) week and 4(th) week follow up (P < 0.001). CONCLUSION: Propolis dentifrice reduces in-vivo microbial load in microenvironments especially against Mutans streptococci in the oral cavity of young patients. Thus, it's potential to be inculcated and used as an alternative measure to prevent dental caries can be considered and further investigation involving greater number of participants is recommended.
