The system of the therapeutic services control in a rural region of this country has been developed and introduced over the recent three years. The key problem in the design of the system lay in search of criteria for the quality of the work of the treatment and prophylactic institutions and every medical worker, which would reflect the final results of their activities. Introduction of the system of the therapeutic services control brought about the reduction of temporary and steady work losses in patients with respiratory diseases as well as the decrease of the premature death rate because of respiratory and circulatory diseases.

Retrospective expert appraisal of the causes of premature death provides the possibility for raising the quality of health care, eliminating defects in the work of polyclinics and hospitals and also consolidating the main chains of health care. Reduction of premature mortality for preventable causes can lengthen expectation of life. When assessing the quality of health care it is recommended to apply an integral indicator of premature mortality.

The role of T-system immunity in reparatory regeneration of the bone tissue in animals.

It has been demonstrated in experiments on mice (CBA X C57BL) X F1, thymectomized and irradiated by 800 R, with the haemopoietic system restored by bone marrow (B-mice) that in these animals, as compared with the controls, the changes in cellular immunity (inhibition of natural killer cells and stimulation of individual functions of the phagocytizing cells) are accompanied by considerable inhibition of osteogenesis. Compensatory regeneration of broken thigh-bone in B-mice is delayed by 5--10 days in various elements of the regenerated tissue in comparison with normal mice. Anatomical formation of the provisional callus in B-mice is not completed on day 21 and approaches a 14-day regenerate of the controls. The obtained results suggest the participation of T-system immunity in the reparatory regeneration of bone tissue.

Peculiarities of the composition and functional state of lymphocytes in patients with impaired bone tissue regeneration.

In patients with delayed bone tissue regeneration, changes in the indices of cellular immunity were revealed, consisting in a decreased number, both absolute and relative, of T lymphocytes, and in their reduced ability to respond to PHA stimulation by the reaction of blastic transformation. Autoimmunization to bone tissue antigen was observed in the same patients. This was confirmed by the reactions of blastic transformation and cytotoxicity of lymphocytes as well as by the reaction of leukocyte migration inhibition in the presence of bone antigen. It is recommended to take into account the indices of cellular immunity in treating patients with poorly healing bone fractures.
