RESUMO

We present a comprehensive dynamical analysis of a chaotic chemical model referred to as the autocatalator, when subject to a periodic administration of one substrate. Our investigation encompasses the dynamical characterization of both unforced and forced systems utilizing isospikes and largest Lyapunov exponents-based parameter planes, bifurcation diagrams, and analysis of complex oscillations. Additionally, we present a phase diagram showing the effect of the period and amplitude of the forcing signal on the system's behavior. Furthermore, we show how the landscapes of parameter planes are altered in response to forcing application. This analysis contributes to a deeper understanding of the intricate dynamics induced by the periodic forcing of a chaotic system.

BACKGROUND: Central nervous system (CNS) neoplasms are highly frequent solid tumours in children and adolescents. While some studies have shown a rise in their incidence in Europe, others have not. Survival remains limited. We addressed two questions about these tumours in Spain: (1) Is incidence increasing? and (2) Has survival improved? METHODS: This population-based study included 1635 children and 328 adolescents from 11 population-based cancer registries with International Classification of Childhood Cancer Group III tumours, incident in 1983-2007. Age-specific and age-standardised (world population) incidence rates (ASRws) were calculated. Incidence time trends were characterised using annual percent change (APC) obtained with Joinpoint. Cases from 1991 to 2005 (1171) were included in Kaplan-Meier survival analyses, and the results were evaluated with log-rank and log-rank for trend tests. Children's survival was age-standardised using: (1) the age distribution of cases and the corresponding trends assessed with Joinpoint; and (2) European weights for comparison with Europe. RESULTS: ASRw 1983-2007: children: 32.7 cases/106; adolescents: 23.5 cases/106. The overall incidence of all tumours increased across 1983-2007 in children and adolescents. Considering change points, the APCs were: (1) children: 1983-1993, 4.3%^ (1.1; 7.7); 1993-2007, -0.2% (-1.9; 1.6); (2) adolescents: 1983-2004: 2.9%^ (0.9; 4.9); 2004-2007: -7.7% (-40; 41.9). For malignant tumours, the trends were not significant. 5-year survival was 65% (1991-2005), with no significant trends (except for non-malignant tumours). CONCLUSIONS: CNS tumour incidence in Spain was found to be similar to that in Europe. Rises in incidence may be mostly attributable to changes in the registration of non-malignant tumours. The overall malignant CNS tumour trend was compatible with reports for Southern Europe. Survival was lower than in Europe, without improvement over time. We provide a baseline for assessing current paediatric oncology achievements and incidence in respect of childhood and adolescent CNS tumours.

Global trade facilitates the introduction of invasive species that can cause irreversible damage to agriculture and the environment, as well as stored food products. The raisin moth (Cadra figulilella) is an invasive pest that poses a significant threat to fruits and dried foods. Climate change may exacerbate this threat by expanding moth's distribution to new areas. In this study, we used CLIMEX and MaxEnt niche modeling tools to assess the potential global distribution of the raisin moth under current and future climate change scenarios. Our models projected that the area of suitable distribution for the raisin moth could increase by up to 36.37% by the end of this century under high emission scenario. We also found that excessive precipitation decreased the probability of raisin moth establishment and that the optimum temperature range for the species during the wettest quarter of the year was 0-18 °C. These findings highlight the need for future research to utilize a combined modeling approach to predict the distribution of the raisin moth under current and future climate conditions more accurately. Our results could be used for environmental risk assessments, as well as to inform international trade decisions and negotiations on phytosanitary measures with regards to this invasive species.

Introduction: The genus Trissolcus includes a number of egg parasitoids that are known to contribute to the control of Halyomorpha halys. The number of progenies, particularly females, is important for the efficient mass rearing of species used in augmentative biological control programs. Cold storage is an important technique for extending the shelf life of natural enemies used in such programs. Methods: We assessed how fecundity, sex ratio, lifespan, and the number of hosts parasitized within 24 h were affected by host density for T. japonicus and T. cultratus when offered fresh H. halys eggs and how these parameters were affected if adult parasitoids were first placed in cold storage (11°C in the dark) for 19 weeks before being used for propagation. Results: The fecundity were 110.2 and 84.2 offspring emerged at 25°C, for parasitoids not placed in cold storage; among the offspring that emerged, 82.6% and 85.6% were female for T. japonicus and T. cultratus, respectively. If first placed in cold storage, T. japonicus and T. cultratus produced 35.1 and 24.6 offspring per female, respectively, although cold storage significantly extended the shelf life. The survival rates of parasitoids that were placed in cold storage were 90.3% and 81.3% for females, and 3.2% and 0.9% for males of T. japonicus and T. cultratus, respectively. The number of hosts parasitized within 24 h was not shown to be density dependent, but it was significantly lower after cold storage. Discussion: This information can be used to estimate the likely production for augmented rearing colonies for use in biological control programs.

The generalist predatory mite Amblyseius swirskii is a widely used natural enemy of phytophagous pests. Due to the negative effects of conventional pesticides on non-target organisms, the development of selective natural and eco-friendly pesticides, such as essential plant oils, are useful pest control tools to use in synergy with biological control agents. Essential oils of Nepeta crispa, Satureja hortensis, and Anethum graveolens showed promising results to control Tetranychus urticae. Hence an experiment was carried out to evaluate the effects of these essential oils on the biochemical and demographic parameters of A. swirskii. A significant reduction of carbohydrate, lipid, and protein contents of oil-treated predatory mites was observed. However, essential oils of S. hortensis and A. graveolens had no effect on lipid reserves. The glutathione S-transferase activity of A. swirskii was influenced by A. graveolens oil treatment. In addition, the enzyme activity of the α-esterases was elevated by all treatments. The essential oils showed no effect on ß-esterases activity compared to the control treatment. None of the concentrations of the different tested oils affected the population growth parameters of A. swirskii. However, a significant reduction was observed in oviposition time and total fecundity of predatory mites. A population projection predicted the efficacy of predatory mites will likely be decreased when expose to the essential oils; however, population growth in the S. hortensis treatment was faster than in the other two treatments not including the control. The results presented in this study may have critical implications for integrated pest management (IPM) programs. However, our observations show that using the tested essential plant oils requires some caution when considered as alternatives to synthetic pesticides, and in combination with A. swirskii. Semi-field and field studies are still required to evaluate the effects on T. urticae and A. swirskii of the essential oils tested in this study, before incorporating them into IPM strategies.

El estudio de caso que se presenta a continuación busca resolver el desafío de establecer una relación terapéutica mediante la escucha activa en el contexto remoto de teleenfermería. Para ello se trabajó en un plan de cuidados de enfermería, en base a encuentros remotos con una usuaria de un condominio de viviendas tuteladas por medio de videollamadas, poniendo a prueba la forma en que se realizaba la escucha activa al encontrarse en espacios físicos diferentes. Se realizó una valoración a través de los 11 patrones funcionales de Gordon, priorizando las necesidades detectadas en la usuaria y estableciendo objetivos e intervenciones. Dentro de los resultados obtenidos se destaca que, al existir una óptima metodología de trabajo y retroalimentaciones constantes junto con un buen uso del tiempo, se puede lograr a cabalidad la escucha activa por medio de la teleenfermería a pesar de estar en espacios físicos diferentes. Esto último permite el establecimiento de una relación terapéutica entre el profesional de enfermería y la persona. Se concluye que la teleenfermería es un recurso valioso que es necesario continuar trabajando para que las diferencias entre una atención presencial y una atención remota sean cada vez mínimas[AU]


The case study presented below seeks to resolve the challenge of establishing a therapeutic relationship through active listening in the remote context of tele-nursing. To this end, we worked on a nursing care plan, based on remote meetings with a user of a supervised housing condominium by means of video calls, testing the way in which active listening was carried out when they were in different physical spaces. An assessment was carried out through Gordon's 11 functional patterns, prioritizing the needs detected in the user and establishing objectives and interventions. Among the results obtained, it stands out that, with an optimal work methodology and constant feedback together with a good use of time, active listening can be fully achieved by means of teleconferencing despite the fact that they are in different physical spaces. The latter allows the establishment of a therapeutic relationship between the nurse and the person. It is concluded that tele-nursing is a valuable resource that needs to be further developed so that the differences between face-to-face and remote care become increasingly minimal[AU]


O estudo de caso apresentado abaixo procura resolver o desafio de estabelecer uma relação terapêutica através da escuta activa no contexto remoto da tele-nursão. Para o efeito, trabalhámos num plano de cuidados de enfermagem, baseado em reuniões remotas com um utilizador de um condomínio habitacional supervisionado através de videochamadas, testando a forma como a escuta activa era realizada quando se encontravam em diferentes espaços físicos. Foi feita uma avaliação através dos 11 padrões funcionais de Gordon, dando prioridade às necessidades detectadas no utilizador e estabelecendo objectivos e intervenções. Entre os resultados obtidos, destaca-se que, com uma metodologia de trabalho óptima e um feedback constante juntamente com uma boa utilização do tempo, a escuta activa pode ser plenamente alcançada por meio de teleconferência, apesar de se encontrarem em espaços físicos diferentes. Este último permite o estabelecimento de uma relação terapêutica entre a enfermeira e a pessoa. Conclui-se que a tele-nurmagem é um recurso valioso que precisa de ser mais desenvolvido para que as diferenças entre cuidados presenciais e remotos se tornem cada vez mais mínimas[AU]

We report the discovery of two types of stability rings in the control parameter space of a vertical-cavity surface-emitting semiconductor laser. Stability rings are closed parameter paths in the laser control space. Inside such rings, laser stability thrives even in the presence of small parameter fluctuations. Stability rings were also found in rather distinct contexts, namely, in the way that cancerous, normal, and effector cells interact under ionizing radiation and in oscillations of an electronic circuit with a junction-gate field-effect transistor (JFET) diode. We argue that stability-enhancing rings are generic structures present in the control parameter space of many complex systems.

Introducción: Estudios previos han demostrado una elevada mortalidad de los pacientes en tratamiento con hemodiálisis, aunque en pocos de ellos se analiza la supervivencia de los que reciben exclusivamente este tratamiento. Nuestro objetivo fue analizar la mortalidad de los pacientes que recibieron tratamiento con hemodiálisis. Métodos: Se analizó la cohorte de pacientes que iniciaron tratamiento sustitutivo entre los años 2010 y 2012 en la comunidad de Castilla-La Mancha y permanecieron en tratamiento con hemodiálisis. Se estudiaron las variables edad, sexo, enfermedad renal primaria, acceso vascular, hemoglobina, índice de Charlson y albúmina sérica al comienzo del tratamiento y se realizó un seguimiento hasta final de 2017. Resultados: La mortalidad fue del 63,4% a los 5 años y del 76% al final del periodo de seguimiento, sin diferencias entre varones y mujeres, y se relacionó con una mayor edad, el comienzo urgente o en aquellos con enfermedad renal reagudizada, la utilización de catéteres o una albúmina inferior a 3,5g/dl. Conclusiones: La mortalidad en los pacientes que permanecen en diálisis es muy elevada y se asocia a factores no modificables como la edad pero también a otros que podemos prevenir o tratar, como el tipo de acceso vascular o el estado de nutrición al comienzo del tratamiento. Introduction: Previous reports have shown very high mortality among hemodialyisis patients. Our goal was to analyze the mortality of patients in the Renal Registry of Patients who remained exclusively on hemodialysis treatment. Methods: The cohort of patients who started treatment in the community of Castilla-La Mancha between 2010 and 2012 and remained on hemodialysis treatment was analysed until the end of 2017. Age, sex, primary kidney disease, vascular access, hemoglobin, Charlson index and serum albumin were included. Results: Mortality rate was 63.4% after 5 years and 76% at the end of the study, with no difference between males and females, and was linked to an older age, urgent onset or in those with acute deterioration of chronic kidney disease, the use of catheters or albumin less than 3.5g/dl. Conclusions: Mortality in patients who remain on hemodialysis is very high and is associated with non-modifiable factors such as age but also others that we can prevent or treat such as type of vascular access or nutrition status at the beginning of treatment. We performed a scientometric analysis of Chaos papers from 1991 to 2019, applying a careful disambiguation process for identifying the authors correctly. First, we used standard scientometric tools based on descriptive statistics. This analysis enabled us to compute productivity and the degree of collaboration. The evolution in the number of authors, countries, and topics per article has an increasing trend. An analysis of the citations considering their temporal mean number exhibits a growing tendency in time. Second, we dealt with Lotka-Zipf's law considering the rank distributions of 15 datasets. We found that the sum of Crossref citations by country was the only dataset for which the power-law was the only plausible distribution. Next, we examined the networks of authors, countries, and topics, going from the simplest case of undirected and unweighted networks to the general case of weighted and directed networks and assigning a weight to the individual nodes. Based on the networks' topology and features, we introduced diversity, collaboration, influence, and productivity measures and found a significant increase in the diversity of all the considered networks (authors, countries, and topics) but manifesting a very different network structure. The computation of typical network quantities combined with the communities' identification reveals the presence of several hubs and the existence of various communities that encompass nodes of all the continents in the case of countries. Finally, using the most general networks, it was possible to compute influence and productivity indexes to find the USA, China, and Germany's leadership inside the network.

The brown marmorated stink bug Halyomorphahalys (Stål) (Hemiptera: Pentatomidae) is native to Northeast Asia, but has become a serious invasive species in North America and Europe, causing major damage to crops. While it has not established itself in Australia, it has been intercepted at the border several times, indicating that future incursions and establishment are a case of when, not if. Biological control is one of the few control options for this species and will be important for managing H.halys should it become established in Australia. Prioritizing species that could be used as biological control agents would ensure Australia is prepared. This study summarizes the literature on natural enemies of H. halys in its native and invaded ranges and prioritizes potential biological control agents of H.halys that could be used in Australia. Two egg parasitoid species were identified: Trissolcusjaponicus (Ashmead) and Trissolcusmitsukurii (Ashmead) (Hymenoptera: Scelionidae). Future efforts to develop biological control should focus on T. mitsukurii, as it is already present in Australia. However, little is known about this species and further work is required to: (1) assess its potential effectiveness in parasitizing H. halys, (2) determine its current distribution and (3) host range in Australia.

Classical biological control is a pest control tool involving the release of imported natural enemies. The Sterile Insect Technique (SIT) comprises releasing sexually sterile insects of a pest into the wild population for suppression or eradication. Both these approaches are environmentally friendly and their combination can result in a synergistic impact on pest populations and improve eradication. However, stringent regulation surrounding the introduction of biological control agents limits their use in eradication owing to the perceived risk of effects on non-target organisms. We investigated the irradiation biology of the egg parasitoid Trissolcus basalis to ascertain whether sterile parasitoids could mitigate the risk of potential sustained non-target impacts. Mated female T. basalis were gamma-irradiated at doses between 120 and 150 Gy and exposed to egg masses of their host Nezara viridula throughout their lifespans. This resulted in host mortality, despite a substantial reduction in developing parasitoid offspring, which followed a negative dose-response. There was no emergence of parasitoid offspring at 140 Gy and above. Irradiation did not affect oviposition behaviour but caused an increase in longevity. Consequently, sterile parasitoids could possibly alleviate concerns regarding the irreversibility of biological control release, which promotes further investigation of their potential role in eradication.

The brown marmorated stink bug, Halyomorpha halys (Stål) (Hemiptera: Pentatomidae), is native to northeast Asia. It was accidentally introduced to Europe and North America, where it has become a key pest, feeding on many important crops. Previous eco-climatic niche modelling indicates that H. halys could expand its distribution vastly, and numerous border interceptions of this pest in many countries, including Australia and New Zealand, indicate that it would be prudent to prepare for its eventual arrival. Similar niche modelling was used to assess the potential distribution of Trissolcus japonicus (Ashmead) (Hymenoptera: Scelionidae), the key parasitoid of H. halys in China. Trissolcus mitsukurii (Ashmead) is one of the main parasitoids of H. halys in Japan. It is known to have existed in Australia since the early 20th century and was also specifically introduced to Australia in the 1960s, and it has now also invaded Italy. We used CLIMEX to model the climatic niche of T. mitsukurii to estimate its global potential distribution. We found that T. mitsukurii should be able to significantly expand its range globally, and that there is a significant degree of overlap in the projected ranges of T. mitsukurii, T. japonicus and H. halys. From a biological control perspective, this implies that the two Trissolcus species may be able to help mitigate the potential impacts of H. halys.

To develop effective and targeted biocontrol tactics for the brown marmorated stink bug, Halyomorpha halys, in crop habitats, a good understanding is essential of the abundance and diversity of its parasitoids in different crop habitats in its native range. To obtain information on the egg parasitoid communities of H. halys in kiwifruit, surveys using sentinel egg masses were conducted in 2018 and 2019. These assessed the species composition of egg parasitoids of H. halys in green-fleshed 'Hayward' kiwifruit orchards, and quantified their season-long abundances in orchards under two different management systems. Parasitism was observed from June to August 2018 (mean parasitism: 48%) and from May to August 2019 (mean parasitism: 29%) across the experimental orchards. In total, five different parasitoid species were found across the two surveys seasons in the kiwifruit orchards, Trissolcus japonicus, T. cultratus, T. plautiae, Anastatus japonicus, and Acroclisoides sp., where T. japonicus and T. cultratus were the predominant species. Monthly T. japonicus abundance data had a unimodal distribution in 2018, peaking in July. There were two peaks (May-June and August) in the 2019 season. Overall, T. japonicus was significantly more abundant in the organic orchard than the conventionally managed orchard only in 2018, and its monthly abundance differed significantly in the two orchards in the two survey seasons. Results and their implications for future classical biological control for H. halys in kiwifruit are discussed.

OBJECTIVES: To assess screening quality metrics and to describe mortality rates eight years after redesign of breast cancer screening and diagnosis pathways, and the introduction of digital breast tomosynthesis. SETTING: Breast Unit of the Toledo Health Area in the region of Castilla-La Mancha (Spain). METHODS: We recorded screening metrics and mortality data following the introduction of digital breast tomosynthesis in 2011 for screening and diagnosis pathways. We then compared the mortality between Toledo Health Area and the rest of Castilla-La Mancha, where digital breast tomosynthesis is not available. RESULTS: All screening quality metrics improved following the introduction of digital breast tomosynthesis. The cancer detection rate significantly increased from 2.3 (95% confidence interval (CI): 1.9-3.6) to 4.5 per 1000 women (95% CI: 3.2-5.2) on average between the periods 2005-2009 and 2015-2018, while the recall rate significantly decreased from 7.0% (95% CI: 6.8%-8.2%) to 2.6% (95% CI: 2.0%-3.6%). Comparing breast cancer mortality rates for 2014-2018 in the Toledo Health Area with the rest of Castilla-La Mancha, which had similar cancer treatment access and management protocols but without digital breast tomosynthesis, the crude mortality rate was 17.79 (95% CI: 15.38 -20.19) vs. 24.76 per 100,000 (95% CI: 26.12-23.39), respectively. The cumulative risk of death was also significantly lower for the Toledo Health Area than for Castilla-La Mancha. CONCLUSION: The introduction of digital breast tomosynthesis improved screening quality indicators. Breast cancer mortality simultaneously decreased with respect to the rest of Castilla-La Mancha. Further research is needed to assess the long-term results, and the role that the redesign may have played in reducing mortality.

OBJECTIVES: To develop short versions of the Frailty Trait Scale (FTS) for use in clinical settings. DESIGN: Prospective population-based cohort study. SETTING AND PARTICIPANTS: Data from 1634 participants from the Toledo Study for Healthy Aging. METHODS: The 12-item Frailty Trait Scale (FTS) reduction was performed based on an area under the curve (AUC) analysis adjusted by age, sex, and comorbidity. Items that maximized prognostic information for adverse events were selected. Each item score was done at the same time as the reduction, identifying the score that maximized the predictive ability for adverse events. For each short version of the FTS, cutoffs that optimized the prognostic information (sensitivity and specificity) were chosen, and their predictive value was later compared with a surrogate gold standard for frailty (the Fried Phenotype). RESULTS: Two short forms, the 5-item (FTS5) (range 0-50) and 3-item (FTS3) (range 0-30), were identified, both with AUCs for health adverse events similar to the 12-item FTS. The identified cutoffs were >25 for the FTS5 scale and >15 for the FTS3. The frailty prevalence with these cutoffs was 24% and 20% for the FTS5 and FTS3, respectively, whereas frailty according to Fried Phenotype (FP) reached 8% and prefrailty reached 41%. In general, the FTS5 showed better prognostic performance than the FP, especially with prefrail individuals, in whom the FTS5 form identified 65% of participants with an almost basal risk and 35% with a very high risk for mortality (OR: 4) and frailty (OR: 6.6-8.7), a high risk for hospitalization (OR: 1.9-2.1), and a moderate risk for disability (OR: 1.7) who could be considered frail. The FTS3 form had worse performance than the FTS5, showing 31% of false negatives between frail participants identified by FP with a high risk of adverse events. CONCLUSIONS AND IMPLICATIONS: The FTS5 is a short scale that is easy to administer and has a similar performance to the FTS, and it can be used in clinical settings for frailty diagnosis and evolution.

Triatominae insects are vectors of the parasite Trypanosoma cruzi, the etiological agent of Chagas disease affecting millions of people in Latin America. Some species, such as Triatoma infestans, live in the human neighborhood, aggregating in walls or roof cracks during the day and going out to feed blood at night. The comprehension of how sex and T. cruzi infection affect their aggregation and geotaxis is essential for understanding their spatial organization and the parasite dispersion. Experiments in laboratory-controlled conditions were carried out with groups of ten adults of T. infestans able to explore and aggregate on a vertical surface. The influence of the sex (male vs. female) and the proportion of infected insects in the group were tested (100% of infected insects vs. a small proportion of infected insects, named infected and potentially weakly infected groups, respectively). Therefore, four distinct groups of insects were tested: infected males, infected females, potentially weakly infected males, and potentially weakly infected females, with 12, 9, 15, and 16 replicates, respectively. The insects presented a high negative geotaxis and a strong aggregation behavior whatever the sex or their infection. After an exploration phase, these behaviors were stable in time. The insects exhibited a preferential vertical position, head toward the top of the setup. Males had a higher negative geotaxis and a higher aggregation level than females. Both behaviors were enhanced in groups of 100% infected insects, the difference between sexes being maintained. According to a comparison between experimental and theoretical results, geotaxis favors the aggregation that mainly results from the inter-attraction between individuals.

RESUMO

A través de un recorrido histórico por la relación del Sistema Nacional de Salud español y la Salud Comunitaria, se describen las diferentes etapas de auge o declive de la Atención Primaria -y en concreto de su orientación comunitaria-, los acercamientos y distanciamientos en su relación con la Salud Pública y la influencia de las ideologías políticas dominantes. Considerando la salud comunitaria como parte esencial de la Atención Primaria, y de acuerdo con la estrategia de Alma Ata y los principios de la Carta de Ottawa, se defiende una reorientación comunitaria de los servicios de salud y se propone retomar la Atención Primaria como eje estratégico del Sistema Nacional de Salud, dada su capacidad para un abordaje integral de los procesos salud-enfermedad, sus posibilidades de reducir desigualdades y enfrentar determinantes sociales, así como de superar inequidades en salud, con la participación de la población y en coordinación con otros sectores. Los desafíos del futuro inmediato, así como las consecuencias de la crisis económica y de los recortes y debilitamiento del Estado de Bienestar, hacen evidente la necesidad de promover procesos participativos que impliquen al conjunto de actores sociales y, sobre todo, a la ciudadanía, considerada no como destinataria y 'usuaria' o consumidora, sino como sujeto activo y colaborativo. Se señala la imperiosa necesidad de incorporar procesos participativos dirigidos a crear una nueva cultura colectiva respecto a la sostenibilidad y universalidad de los recursos públicos de salud existentes

We defend a community reorientation of health services, considering community health as an essential part of Primary Health Care, and in accordance with the Alma Ata strategy and the principles of the Ottawa Charter. The Primary Health System is taken up as the strategic axis of the National Health System, given its capacity for an integral approach to health-disease processes, and the possibility it poses for reducing inequalities and confronting social determinants as well as overcoming inequities in health, with the participation of the population and in coordination with other sectors. The challenges of the immediate future, as well as the consequences of the economic crisis, the cuts, and the weakening of the Welfare State, make evident the need to promote participatory processes that involve all the social actors and, above all, the citizenship -considered not as a recipient and 'user' or consumer, but as an active and collaborative subject. These participatory processes aim at creating a new collective culture regarding the sustainability and universality of existing public health resources
