BACKGROUND: The low sensitivity and specificity of diagnostic aids and the low isolation in cultures make it difficult to recognize early-onset bacterial sepsis in neonates. AIM: Determine the diagnostic validity of C reactive protein (CRP) in early neonatal sepsis. METHOD: The role of CRP in the diagnosis of early-onset neonatal sepsis was evaluated. Levels were measured at 12 and 48 hours of life in patients with suspected sepsis. When evaluating the sensitivity and specificity of the CRP, the result was used quantitatively, using a non-parametric ROC curve to estimate sensitivity and specificity, likelihood ratios and percentage of correct classification for each possible cut-off point. RESULTS: The study included 198 patients. The sensitivity, specificity, positive predictive value, negative predictive value, positive probability index and negative probability index of CRP, were 72.2 - 82.4 - 45.2 - 93.7 - 4.1, and 0.3, respectively with area under the curve of 0.78. CONCLUSIONS: CRP is particularly useful to rule out infection. Two negative serial CRP in the absence of clinical symptoms and positive blood cultures have a high negative predictive value and a negative probability index in favor of excluding infection with high certainty and/or discontinuing antibiotic therapy.

INTRODUCCIÓN: La baja sensibilidad y especificidad de las ayudas diagnósticas y el bajo aislamiento en los cultivos dificulta el reconocimiento de la sepsis bacteriana de inicio temprano en neonatos. OBJETIVO: Determinar la validez diagnóstica de la de la proteína C reactiva (PCR) en la sepsis en la sepsis neonatal temprana. MÉTODO: Se evaluó el papel de la PCR en el diagnóstico de sepsis neonatal temprana. Las concentraciones se midieron a las 12 y 48 h de vida en pacientes con sospecha de sepsis. Al evaluar la sensibilidad y especificidad de la PCR, se utilizó el resultado de manera cuantitativa y mediante una curva ROC no paramétrica se estimó sensibilidad y especificidad, razones de verosimilitud y porcentaje de clasificación correcta para cada punto de corte posible. RESULTADOS: El estudio incluyó 198 pacientes. La sensibilidad, especificidad, valor predictor positivo (VPP), valor predictor negativo (VPN), índice de probabilidad positiva e índice de probabilidad negativa de la PCR fue de 72,2 - 82,4 - 45,2 - 93,7 - 4,1 y 0,3, respectivamente con área bajo la curva de 0,78. CONCLUSIONES: La PCR es particularmente útil para descartar una infección. Dos PCR seriadas negativas en ausencia de expresión clínica y de hemocultivos positivos tienen un alto VPN y un índice de probabilidad negativa a favor de excluir la infección con una alta certeza y/o de descontinuar la terapia antibiótica.

BACKGROUND: The aim was to analyze the fracture resistance of human teeth treated endodontically and restored with posts made of bovine dentin, human dentin, or glass fiber, and to evaluate the fracture pattern. METHODS: Cylindrical posts of 1.5 mm in diameter cemented to the roots of human maxillary canines presented a length of 15 mm, cervical diameter of 5-5.5 mm in the mesiodistal direction, and 7-7.5 mm in the vestibule-palatal direction. The groups studied were: Group I-10 glass fiber posts; Group II-10 human dentin posts; Group III-10 bovine dentin posts (self-adhesive resin cement); and Group IV-10 bovine dentin posts (resin-modified glass-ionomer cements). The coronal part of tooth was restored with a standardized core build-up using composite. All of the groups were submitted to a compression force test and the resistance to fracture was verified using a universal testing machine. The fracture pattern was likewise evaluated. RESULTS: The values of resistance to fracture were: 723.3N in group I, 561.5N in group II, 556.6N in group III, and, 613.27N in group IV. However, no statistically significant difference was observed among the groups. The fractures in groups I and II were most commonly found in the middle/apical third and were considered irreparable. For restored teeth in group III, half of the fractures appeared in the cervical third and were reparable. In group IV, all of the fractures were reparable, with the majority in the cervical thirds. CONCLUSION: Bovine dentin can be used as intraradicular post to substitute human dentin and glass fiber posts. The greater the malleability of the post, the greater the chances of survival of the teeth when subjected to fracture testing.

Spent coffee grounds are wastes generated annually worldwide in significantly large amounts in the soluble coffee industry and in household and commercial beverage preparation. Although spent coffee grounds are rich in several classes of compounds, predominantly polysaccharides, profitable applications have not yet been effectively implemented for such wastes. Thus, it was the aim of this study to verify the feasibility of producing biopolymeric films from the polysaccharide-rich fraction of spent coffee grounds, obtained by alkaline hydrogen peroxide treatment of the coffee waste. Produced films were characterized for their physicochemical, barrier and mechanical properties and these properties were comparable to those of similar polysaccharides films from the literature.

PURPOSE: Severe physical facial deformities due to surgical interventions can have significant psychosocial consequences to patient's relationships with friends and family and thus, has a considerable impact on their quality of life. We have developed a 3D prosthesis for a 56-year-old woman diagnosed with epidermoid carcinoma at the right hemiface, to improve her quality of life. METHODS: The patient started radiotherapy with modulated intensity. To deal with the advance of the process, a maxilectomy of supra structure with modified radical cervical emptying on the right hemiface was performed. Reconstruction of areas surgically affected by the displacement of islands of skin and muscle (flaps) from healthy regions was initiated. Although the procedure occurred without intercurrences, the patient developed necrosis and loss of the myocutaneous flap. After the removal of the flap, the esthetic result of the treatment was evident causing exposure of subcutaneous and granulation tissues. RESULTS: A computational model was used to develop a 3D structure of the affected area and then used to construct the prosthesis. The prosthesis was applied over the affected area, and the patient was able see her face on the mirror for the first time in years. The patient was grateful and hopeful. CONCLUSION: We have found that the application of this new technology greatly improves the social interaction of patients with deformities due to surgical interventions.

Differences and similarities between intrusive thoughts typical of obsessive-compulsive disorder, generalized anxiety disorder, and hypochondriasis are relevant for their differential diagnosis, formulation, and psychological treatment. Previous research in non-clinical samples pointed out the relevance of some process variables, such as responsibility, guilt, or neutralization strategies. This research is aimed to investigate the differences and similarities between clinical obsessions, worries, and illness intrusions in some of these process variables. A second aim is to identify models based on these variables that could reliably differentiate between them. Three groups of patients with obsessive-compulsive disorder (n = 35; 60% women, mean age 38.57), generalized anxiety disorder (n = 36; 61.1% women, mean age 41.50), and hypochondriasis (n = 34; 70.6% women, mean age 31.59) were evaluated using the Cognitive Intrusions Questionnaire-Transdiagnostic Version (Romero-Sanchiz, Nogueira-Arjona, Godoy-Ávila, Gavino-Lázaro, & Freeston, ). The results showed that some appraisals (e.g., responsibility or egodystonicity), emotions (e.g., guilt or insecurity), neutralization strategies, and other variables (e.g., verbal content or trigger from body sensation) are relevant for the discrimination between obsessions, worries, and illness intrusions. The results also showed 3 stable models based on these variables for the discrimination between these thoughts. The implication of these results in the diagnosis, formulation, and psychological treatment of obsessive-compulsive disorder, generalized anxiety disorder, and hypochondriasis is discussed.

Alteration of multidrug resistance-associated protein-1 (MRP1) expression has been associated with certain lung diseases, and this protein may be pivotal in protecting the lungs against endogenous or exogenous toxic compounds. The aim of this study was to evaluate and compare the expression of MRP1 in bronchoalveolar cells from subjects with and without lung cancer who had been chronically exposed to arsenic through drinking water. MRP1 expression was assessed in bronchoalveolar cells in a total of 102 participants. MRP1 expression was significantly decreased in those with arsenic urinary levels >50 µg/L when compared with the controls. In conclusion, chronic arsenic exposure negatively correlates with the expression of MRP1 in BAL cells in patients with lung cancer.

The judiciary needs forensic medicine to determine the difference between an entry hole and an exit hole in human skin caused by firearms for civilian use. This important information would be most useful if a practical and accurate method could be done with low-cost and minimal technological resources. Both macroscopic and microscopic analyses were performed on skin lesions caused by firearm projectiles, to establish histological features of 14 entry holes and 14 exit holes. Microscopically, in the abrasion area macroscopically observed, there were signs of burns (sub-epidermal cracks and keratinocyte necrosis) in the entrance holes in all cases. These signs were not found in three exit holes which showed an abrasion collar, nor in other exit holes. Some other microscopic features not found in every case were limited either to entry holes, such as cotton fibres, grease deposits, or tattooing in the dermis, or to exit holes, such as adipose tissue, bone or muscle tissue in the dermis. Coagulative necrosis of keratinocytes and sub-epidermal cracks are characteristic of entry holes. Despite the small sample size, it can be safely inferred that this is an important microscopic finding, among others less consistently found, to define an entry hole in questionable cases.

The aim of this paper is to investigate the effect of orthosis on the forearm muscle activation. Twenty-six adults, with average age of 26, performed the Jebsen Taylor Test. The volunteers also performed the grip strength test under three conditions: free hand (without orthosis), wearing a composite orthosis, and wearing a thermoplastic orthosis. Only the dominant hand was studied. The mean of three measures of grip strength and the time spent to perform the tests were used to compare the results. Statistical analysis was carried out using the Wilcoxon Test. There was a decrease in grip strength with the use of orthosis. Significant differences were noticed when the volunteers were tested with and without orthosis. No significant differences were observed when data from the two orthosis were compared. A decrease in time to perform the tasks during the Jebsen Taylor Test was noticed with the use of orthosis. Wrist extension orthosis interferes on grip strength and manual function, but it does not restrain its execution.(AU)


O objetivo deste estudo foi analisar o efeito do uso de órtese de imobilização do punho, confeccionada com diferentes materiais, na função manual e na força de preensão. Métodos: Vinte e seis adultos, com média de idade de 26 anos foram submetidos ao Teste Funcional da Mão de Jebsen Taylor e ao Teste de Força de Preensão (Dinamômetro Jamar®), em três condições: com a mão livre, com o uso de órtese de compósito e com a órtese de termomoldável. Os testes foram realizados apenas com a mão dominante. Utilizou-se, para comparação, a média de três medidas da força de preensão e do tempo gasto para a realização dos subtestes. Os resultados obtidos nas três condições foram comparados e analisados utilizando-se o teste estatístico de Wilcoxon. Detectou-se redução da força de preensão com o uso de órtese. Foram encontradas diferenças significativas quando comparadas a força de preensão na condição livre e com o uso de quaisquer das órteses. Não foram encontradas diferenças significativas quando comparadas a força de preensão com o uso dos dois tipos de órtese. Observou-se uma redução do tempo gasto para a realização das tarefas com o uso das órteses. A órtese de imobilização do punho em extensão tem influência na força de preensão manual e na função manual, porém sem implicar na impossibilidade de sua execução.(AU)

This paper reports the development of superhydrophobic nanocomposite systems which are also oleophilic. As hydrophobicity is based on low energy surface and surface roughness, the electrospinning technique was selected as the manufacturing technique. N,N' dimethylformamide (DMF) was employed as the polystyrene (PS) solvent. The "Tea-bag" (T-B) nanocomposite system is based on exfoliated graphite surrounded by PS superhydrophobic membranes. The T-B systems were tested regarding its adsorption and absorption rates. To test these properties, it was employed three different water/oil emulsions, i.e., new and used motor oil, which have physical properties (viscosity and specific gravity) similar to heavy crude oil extracted in Brazil, and vacuum pump oil (which does not form oil/water emulsion). It was observed that oil adsorption rate is dependent on oil surface tension, while the absorption rate is mainly dependent on membrane/exfoliated graphite surface area. Experimental data show that oil absorption rates ranged between 2.5g/g and 40g/g, while the adsorption rate oscillated from 0.32g/g/min to 0.80g/g/min. Furthermore, T-B systems were tested as containment barriers and sorbent materials with good results including its recyclability.

La Reforma de Salud emprendida en el año 2005 tiene como su principal componente el Plan AUGE-GES. Pese a que su propósito fundamental ha sido mejorar la equidad en el acceso a la atención, esta política no ha sido evaluada de manera integral. El presente estudio forma parte de una investigación mayor de carácter nacional, con el objetivo de caracterizar la implementación de la política de salud desde la perspectiva regional, a nivel de servicios de salud, de hospitales y otros organismos en relación a la reorganización estructural. Para ello se utilizó metodología de investigación cualitativa, efectuándo-se 42 entrevistas semiestructuradas a informantes clave (regionales y locales)a las cuales se realizó análisis de contenido. Los resultados señalan que la implementación de la Reforma AUGE-GES en Valparaíso es valorada positivamente por todos los actores al compararla con la situación previa a la Reforma. Tanto las garantías no cumplidas como las listas de espera NO GES son resultados criticados. Se percibe una“augización” de la política. Los equipos de los tres niveles de atención han sido tensionados y enfocados a la atención curativa, postergando las actividades de prevención y promoción de la salud. El GES se visualiza como una estrategia de priorización que ha aumentado la exclusión de grupos pobla-cionales. Los derechos de las personas son restringidos a los derechos propios de consumidores. En base a estos resultados, es posible concluir que la Reforma AUGE-GES no ha cumplido cabalmente con la finalidad de promover el acceso equitativo a la atención en salud. (AU)

The present study is part of a larger national investigation with the objective to characterize the implementation of health policy from a regional perspective, at the level of health services, hospitals and other agencies, in relation to structural reorganization. Methodology: The present study employed qualitative research methodology where 42 semi-structured interviews to key informants (local and regional) were conducted, and content analysis was performed. Results: The results show that the implementation of the GES Reformin Valparaiso is highly valued by all stakeholders when compared to the previous situation. Unfulfilled guarantees and Non-GES waiting lists are both criticized results. GES is perceivedas extremely focused public policy. Health teams have been stressed and have focused primarily on curative care, postponing prevention and health promotion. GES is seen as a prioritization strategy that has increased the exclusion of certain groups of the population. The rights of personsare restricted to their rights as consumers. Conclusion:Based on the present analysis, one can conclude that the AUGE-GES Reformhas not fully complied with the objective to promote equitable access to health care. (AU)

La Reforma de Salud emprendida en el año 2005 tiene como su principal componente el Plan AUGE-GES. GES is seen as a prioritization strategy that has increased the exclusion of certain groups of the population. The rights of persons are restricted to their rights as consumers. Conclusion. Based on the present analysis, one can conclude that the AUGE-GES Reform has not fully complied with the objective to promote equitable access to health care.

Lung cancer is the most common cancer in the world. The main cause of lung cancer is cigarette smoke; however, other important genetic and environmental risk factors play a significant role in the development of lung cancer. Among these factors, occupational and accidental exposure to polychlorinated biphenyls (PCBs) has been associated with an increased risk in lung cancer, suggesting that PCBs could be potent carcinogens. The aim of the present study was to investigate the association between PCB exposure levels, CYP1A1 polymorphisms and the risk of lung cancer. This study enrolled newly diagnosed lung cancer patients. Environmental and occupational information related to the patients studied was collected. Blood samples were taken for the measurement of serum levels of 20 PCB congeners and for CYP1A1 polymorphism analysis. The serum levels of two PCB congeners with potential estrogenic activity were higher in lung cancer patients. The risk of lung cancer was found to correlate with age, gender, smoking history and with agricultural workers, as well as with congener 18. No differences were found in the frequency of CYP1A1 polymorphisms. Furthermore, we did not find a correlation between CYP1A1 polymorphisms and PCB serum levels. The high levels of PCB with estrogenic activity found in our cases, could promote lung cancer inducing cell proliferation in non-neoplastic and neoplastic lung cells via ERß; inducing the formation of DNA adducts, producing oxidative stress with the subsequent DNA damage and increasing the endogenous catechol levels by catechol-O-methyl transferase (COMT) inhibition.

En este estudio se realiza la adaptación y validación del Obsessive Belief Questionnaire-Children Version (OBQ-CV) en una muestra no clínica de niños y adolescentes españoles. Los análisis factoriales confirmatorios revelaron que el modelo que mejor se ajusta a los datos es el de tres factores de primer orden (Responsabilidad/Estimación de la amenaza, Perfeccionismo/Incertidumbre, Importancia/Control del pensamiento) que, a su vez, constituyen un factor de segundo orden. Los resultados mostraron que las puntuaciones del OBQ-CV presentan una alta fiabilidad, así como adecuada validez de criterio con medidas de síntomas obsesivo-compulsivos y de creencias obsesivas, y una buena validez discriminante con medidas de depresión y ansiedad (AU)
