RESUMO

Beam spray measurements suggest thresholds that are a factor of ≈2 to 15× less than expected based on the filamentation figure of merit often quoted in the literature. In this moderate-intensity regime, the relevant mechanism is forward stimulated Brillouin scattering. Both weak ion acoustic wave damping and thermal enhancement of ion acoustic waves contribute to the low thresholds. Forward stimulated Brillouin scattering imparts a redshift to the transmitted beam. Regarding the specific possibility of beam spray occurring outside the laser entrance holes of an indirectly driven hohlraum, this shift may be the most concerning feature owing to the high sensitivity of crossed-beam energy transfer to the interacting beam wavelengths in the subsequent overlap region.

There is an established correlation between the PNPLA3 rs738409 C > G single nucleotide polymorphism (SNP) and hepatic steatosis and fibrosis in hepatitis C virus (HCV) infected patients. However not all data is convergent regarding the exact impact of this SNP on the pattern of disease progression in different clinical settings. In this study, we aimed to further bridge the knowledge gap on this topic by investigating the role of the G allele in promoting steatosis, fibrosis and disease progression in relation to other metabolic and anthropometric host factors. Two hundred and fifty consecutive patients, previously diagnosed with chronic hepatitis C (CHC) underwent liver biopsy. Histology was assessed using the Metavir scoring system. Transient elastography was used for follow-up. Ninety-eight patients were genotyped for PNPLA3 rs738409 and followed up for fibrosis progression. PNPLA3 rs738409[G] allele was significantly correlated with severe steatosis (P = 0.04), severe fibrosis at the time of enrollment (P = 0.0005) and fibrosis progression with an OR of 10.31 (95% CI 1.06 - 99.59, P = 0.04), after a mean follow-up time of 62.85 (95%CI: 52.21 - 76.15) months. Severe steatosis at the time of enrollment had an OR of 11.02 (95% CI 1.48 - 82.09, P = 0.01) for the association with fibrosis progression. The HOMA-IR index was also positively correlated with severe fibrosis (P = 0.03) and fibrosis progression on univariate analysis (P = 0.02). PNPLA3 rs738409[G] allele is a reliable predictor for steatosis and fibrosis in CHC. The presence of G allele, along with severe steatosis and insulin resistance are significant predictors for fibrosis progression.

The structure of microemulsions prepared by the anionic gemini surfactant didodecyl diphenyl ether disulfonate (C12-DADS) was investigated by a solvatochromic probe and nuclear magnetic resonance (NMR) diffusion measurements. The NMR measurements indicate the presence of bicontinuous and oil-in-water microemulsions depending on microemulsion composition. The absorbance spectra of the solvatochromic probe, Nile red, indicate the solubilization of the probe in different sites, in agreement with the NMR findings. It was also found that the microemulsions were capable of dissolving the hydrophobic probe, Nile red, up to four times better than expected if it were simply dissolved in the toluene phase.

Recent studies suggested an improving of left ventricular (LV) systolic function during exercise training in patients with old myocardial infarction. Twenty patients with old myocardial infarction (3-6 weeks) were included in an exercise training programme (mainly cycloergometer) for 2 to 8 weeks. Before and after the exercise training programme, the effort capacity and LV systolic function were determined through exercise testing on cycloergometer and echocardiography. The peak effort raised from 4.39 to 5.27 METs (p < 0.05) and the difference DAF-DAM was reduced from 25% to 12% (p < 0.05). The double product (DP) at peak effort raised nonsignificantly (4.91%, p > > 0.05), but at submaximal effort levels DP decreased significantly on each effort step (12.16%, p < < 0.05). In turn, LV systolic function calculated parameters were practically the same before and after the exercise training programme. (EDD 54.3 vs 5.3 mm, ESD 39.3 vs 38 mm, EDV 160 vs 169 ml, ESV 60.6 vs 54.8 ml, EF 54.1% vs 57.8%, SF 27.6% vs 31.2%). It is concluded that exercise training raised the effort capacity in patients with old myocardial infarction, mainly through peripheral mechanisms, LV systolic function being unchanged. It is also important that exercise training have not any detrimental effect upon LV systolic function and consequently exercise training programmes can be applied and can be useful also in patients with old myocardial infarction and impaired LV systolic function.

Changes in conceptions and attitudes towards mental illness and the mentally ill were investigated in Israel. Questionnaires were presented to 69 student nurses, assessing stereotyped image, authoritarianism, social restrictiveness, benevolence, mental hygiene ideology and interpersonal aetiology before and after psychiatric affiliation in two kinds of mental hospitals, or before and after classroom instruction only. The findings lend cultural generality to previous observations and demonstrate that: (a) classroom instruction is ineffective as an agent of change; (b) practical experience involving personal confrontation with mental patients in a progressive psychiatric hospital, which contradicts the stereotyped image of mental patients and inculcates positive attitudes towards them, results in a change to a more professional scientific orientation and a more humane, accepting and liberal attitude; (c) under hospital conditions which support the stereotyped conception and authoritarian restrictive attitudes, the confrontation will result in preservation, even consolidation and strengthening, of the stereotyped image and undesirable attitudes towards mental patients. The findings were discussed and the implications for training programmes were drawn.
