RESUMO

Pregnancy-associated hypertension is a health problem in Mexico due to its high frequency of morbidity and mortality in mother and fetus as well. Research in this area has been restrained by limitations upon epidemiologic information, unknown etiology and the somewhat easy resolution provided when pregnancy is interrupted. We have reviewed contributions made in our country and up-to-date management concepts.

RESUMO

Pituitary adenoma, familiar type, with apparent affection over the involved somatomammotropin of secretion of growth hormone and prolactin, with dominant autosomic transmission, is described for the first time. It was seen that a couple of members without demonstrable tumour, showed clinical data (phenotype) of acromegaly. On investigation of histocompatibility antigens it was observed that the patients with tumour and other symptomatic ones, but without tumour, shared the same haplotypes, and so is very possible that investigation of HLA antigens in patients with pituitary tumour, contributes to better identify its nature and frequency.

RESUMO

The uterine leiomyomatosis represents the largest amount of tumors of the uterus, and the treatment in most of the cases is surgical. This tumor of smooth muscle has been associated with the stimulus given of sexual steroids, mainly estrogens. Several analogues by the hypothalamic hormone liberating gonadotropins (GnRH) show an stimulating-inhibiting action upon hypophyseal gonadotropins secretion (LH-FSH), variable effect on ovarian steroidogenesis, depending on the dose and frequency of use. When the decapeptide is used daily and during more than a month, it may block the production of LH, FSH and estrogens, and produces amenorrhea. As uterine myomatosis is a condition that may depend on estrogens, it was decided to treat with an analogue of GnRH (Intranasal Buserelin Acetate), one patient, 28 years old with clinical, histopathologic and ultrasonographic of uterine myomatosis, who presented manifestations of hypochromic, microcytic anemia by frequent intermenstrual bleedings. The application of GnRH analogue was done daily during eight months. The therapeutical result control of the tumor was done by an ultrasonographic study and with hysterosalpingography, before and after. The treatment was stopped because there was a significant diminution in the tumor size and bleeding disappearance. Two months of follow up have not shown changes.

RESUMO

Evidence obtained from the study of polycystic ovarian disease in the female is reviewed. The etiology of this problem remains unknown, in spite of advances achieved by means of biochemical and morphologic studies. No correlation has been observed between histological findings and the pituitary or gonadal hormone production. However, an abnormal hypothalamic-pituitary-gonadal interaction prevails, characterized by a increased LH-FSH ratio associated with high level of androgens, mainly androstenedione. The pituitary reserve test performed with potent GnRH agonists have confirmed the hyper-response of LH along with that of androstenedione, as well as augmentation of 17-hydroxyprogesterone. Since the pattern of gonadotropins and steroid hormone secretion in women with polycystic ovarian disease resembles that seen in normal men, the basic alteration may well consist in a functionally "masculinized" hypothalamus.

Collagen content, acid-soluble collagen, degradation activity, and collagen biosynthesis were measured in 22 normal and 20 prematurely ruptured membranes from pregnancies near term (37 weeks or more). Although no significant differences were found in the collagen content, a clear correlation was found between this value and the interval from premature rupture to delivery. Collagenolytic activity and collagen solubility were higher and collagen synthesis was lower in amniotic membranes that ruptured prematurely. These studies suggest that premature rupture of membranes is associated with extensive changes in collagen metabolism.

With the purpose of evaluating the impact of spermatic capacitation on different spermatozooa populations, 49 samples of semen, before and after in vitro spermatic capacitation with Ham F-10 medium, were studied; motility of cells was evaluated according to WHO criteria. There was diminution of percentage of immobile cells, 27.8 to 20.0, as well as increase in population of cells with more mobility, 28.6% to 39.1%. Both difference were statistically significant (p = less than 0.05 and p = less than 0.005, respectively). These data suggest that spermatic capacitacion activates "in cascade" all groups of gametes.
