RESUMO

Recently, an increased number of reports have described pathogens of animal origin that cause a variety of infections and a rise in their transmission to humans. Streptococcus gallolyticus, a member of the Streptococcus bovis/Streptococcus equinus complex (SBSEC), is one of these pathogens and infects a wide range of hosts from mammals to poultry and has a broad functionality ranging from pathogenicity to food fermentation. As S. gallolyticus causes complications including bacteremia, infective endocarditis, and colorectal malignancy in humans, it is important to investigate its occurrence in various hosts, including geese, to prevent potential zoonotic transmissions. This study aimed to investigate the presence of S. gallolyticus in the droppings of clinically healthy and diarrheic geese, which were raised intensively and semi-intensively, by the in vitro culture method, characterize the isolates recovered by PCR and sequence-based molecular methods and determine their antibiotic susceptibility by the disk diffusion and gradient test methods. For this purpose, 150 samples of fresh goose droppings were used. Culture positivity for S. gallolyticus was determined as 8% (12/150). PCR analysis identified 54.55% (n = 6) of the isolates as S. gallolyticus subsp. gallolyticus and 45.45% (n = 5) as S. gallolyticus subsp. pasteurianus. Following the 16S rRNA sequence and ERIC-PCR analyses, S. gallolyticus subspecies exhibited identical cluster and band profiles that could be easily distinguished from each other and were clonally identified. High rates of susceptibility to florfenicol, penicillin, rifampicin, and vancomycin were detected among the isolates, regardless of the subspecies diversity. Both subspecies showed high levels of resistance to bacitracin, clindamycin, doxycycline, tetracycline, trimethoprim-sulfamethoxazole, and erythromycin and multiple MDR profiles, indicating their potential to become superbugs. This first report from Türkiye demonstrates the occurrence of the S. gallolyticus subspecies in geese. In view of the recent increase of geese production and the consumption of goose meat in Türkiye, the occurrence of S. gallolyticus in geese should not be ignored to prevent zoonotic transmission.

RESUMO

The aim of this study is to determine the relationship between internet addiction and fear of childbirth in pregnant women. The study was conducted with 200 pregnant women who applied to two hospitals in province located in eastern Türkiye between April and August 2023 and were voluntary to participate in the study. In this descriptive, cross-sectional and correlational study,a"Personal Information Form", the "Internet Addiction Scale", and the "Wijma Delivery Expectancy/Experience Questionnaire" were used. The data were analyzed using independent samples t test, analysis of variance, and regression analysis. The pregnant women 40.5% expressed that they always accessed the information they sought. Pregnant women had high levels of internet addiction and fear of childbirth. In pregnant women, internet addiction was a significant predictor of fear of childbirth. With a multidisciplinary approach a preventive and supportive environment should be established in health services for the internet used during pregnancy.

Hydatid Cyst is a public health problem in Turkey. Although it can lead to cyst formation in all organs and systems, the most frequently affected organs are the liver and lungs. Here, we reported a 14-year-old girl who presented with low back pain and hydatid vomita symptoms and signs. The abdominal and thoracic CT examinations of the patient revealed that hydatid cyst found in the liver was opened to the inferior vena cava and it was determined that it caused multiple emboli in the right atrium, pulmonary arteries and lungs. Albendazole treatment was commenced and the hydatid cyst in the liver was excised by surgery, and surgery was planned for the hydatid cyst lesion in the right atrium. However, in the control thorax BT before the surgery, it was found that the cysts in the cardiovascular structures disappeared, causing widespread pulmonary embolism and spread to the entire pulmonary field. The most important complication of intense hydatid pulmonary embolization is the development of pulmonary hypertension and right heart failure. In our patient, pulmonary artery pressures and right heart functions were normal in repeated echocardiography. With this case, we wanted to emphasize that the cardiovascular involvement of hydatid cyst may not always require surgery.

RESUMO

Cancer can be identified as an uncontrolled growth and reproduction of cell. Accumulation of genetic aberrations (mutations of oncogenes and tumor-suppressor genes and epigenetic modifications) is one of the characteristics of cancer cell. Increasing number of studies highlighted importance of the epigenetic alterations in cancer treatment and prognosis. Now, cancer epigenetics have a huge importance for developing novel biomarkers and therapeutic target for cancer. In this review, we will provide a summary of the major epigenetic changes involved in cancer and preclinical results of epigenetic therapeutics.

RESUMO

BACKGROUND: Irreparable rotator cuff tears lead to superior translation of the humeral head. Numerous surgical management options are available to treat the condition. PURPOSE: To compare superior capsule stability among different types of patch grafting in irreparable rotator cuff tears. METHODS: Six cadaveric shoulders were tested in a custom-designed shoulder testing system. Superior translation of the humerus and subacromial contact pressure were quantified in an intact condition (condition 1), after cutting the supraspinatus tendon (condition 2), and after additionally cutting the superior capsuloligamentous complex (condition 3). The results were compared among 3 types of patch grafting, in which capsule reconstruction was achieved by glenoidal 3-point (condition 4) or 2-point (condition 5) fixation or by affixing a graft below the acromion (condition 6). RESULTS: No significant difference in subacromial pressure was measured by reconstruction with 2 or 3 anchors compared with conditions 1 and 2 (P > .05). However, with 3-point fixation, lower levels of pressure were measured than with 2-point fixation. Moreover, superior translation values were lower with 3-point fixation; the same applied for values of the preserved capsule as compared with the torn capsule. In condition 6, a significant increase in pressure in the neutral position was documented (P < .05). CONCLUSION: The superior capsuloligamentous complex plays an important role in stabilizing the glenohumeral joint. The results suggest that with additional medial anchoring at the coracoid base, the depressing and centering effect of the superior complex can probably be regained in a more physiological way compared with a reconstructed capsule with 2 glenoid attachments or with an interpositional graft below the acromion.
