BACKGROUND: This study aims to assess the effect of primary tumour resection (PTR) on patients with metastatic colorectal cancer (mCRC) treated with cetuximab. METHODS: This retrospective cohort study was conducted in a tertiary cancer center in Turkey. Patients with mCRC between January 2009 and December 2020 were extracted from the electronic hospital management system. Patients with RAS wild-type synchronous metastatic left-sided colon or rectum cancer who had cetuximab-containing treatment protocol were included in the study. The primary outcomes were overall survival (OS) and progression-free survival (PFS). The secondary outcome was response rates. RESULTS: A total of 111 patients with mCRC were included in this study. PTR was performed in 57.7% of all patients. Fifty-nine (53.2%) and 52 (46.8%) patients had rectal and left colon tumours, respectively. The combination treatment with cetuximab was FOLFIRI in 62.2% and FOLFOX in 29.7% of all patients. In subgroup analysis, the median PFS was 7.9 and 9 months in PTR (+) and PTR (-) patients, respectively. The difference between the groups was not statistically significant (P = 0.3). The median OS was 33 months in all patients. In subgroup analysis, the median OS was 39 and 27.9 months in PTR (+) and PTR (-) patients, respectively. The difference between the groups was statistically significant (P = 0.002). After adjusting for confounding factors, PTR and ECOG performance score were the independent prognostic factors for OS. CONCLUSION: PTR improved the OS in patients with RAS wild-type synchronous left-sided colon or rectum cancer treated with cetuximab-containing chemotherapy regimens.

BACKGROUND: An intestinal stoma has severe psychosocial effects on patients. OBJECTIVE: This study aimed to measure the effects of prehabilitation on stoma self-care, quality of life, anxiety, and depression levels. DESIGN: This study was a a randomized controlled trial. SETTINGS: Patients were allocated into 3 groups according to enterostomal management: group A (stoma marking and postoperative education), group B (marking and pre- and postoperative education), and group C (prehabilitation group) (marking and preoperative education with introduction of the stoma appliance preoperatively and postoperative education). A stoma care follow-up form, the Hospital Anxiety and Depression Scale, and a stoma-specific quality-of-life questionnaire were used for evaluation. PATIENTS: Patients who underwent colorectal surgery and fecal diversion at the Ankara University, Ankara, Turkey, between 2011 and 2016 were included. MAIN OUTCOME MEASURES: The primary outcome was the effect of stoma prehabilitation on self-care ability. RESULTS: Two hundred forty patients were included in the study. The outcome of stoma self-care revealed that 24 patients (31.2%) in group A, 51 patients (78.5%) in group B, and 72 patients (94.7%) in group C could perform stoma care independently ( p < 0.001). Among patients with temporary stomas, group C had a better outcome than patients in groups A and B ( p < 0.001). There was no similar relationship among patients with permanent stomas. The prehabilitation group had fewer patients with anxiety and depression than the other groups. Among patients with temporary stomas, similar findings were found. No differences were found in either anxiety or depression among patients with permanent stomas. Stoma quality-of-life scores were significantly higher in the prehabilitation group ( p < 0.001). Among patients with temporary stomas, similar findings were found. There was no difference among those with permanent stoma. LIMITATIONS: This was a single-center study of heterogeneous groups. CONCLUSIONS: Prehabilitation facilitates stoma self-care, decreases predisposition to anxiety and depression and improves quality of life in patients with stomas. Para la evaluación se utilizaron el formulario de seguimiento del cuidado de la estoma, la Escala Hospitalaria de Ansiedad y Depresión y un cuestionario específico de la estoma sobre la calidad de vida.PACIENTES:Se incluyeron pacientes que fueron sometidos a cirugía colorrectal y derivación fecal en la Universidad de Ankara entre 2011 y 2016.PRINCIPALES MEDIDAS DE RESULTADO:El resultado principal fue el efecto de la prehabilitación de la estoma sobre la capacidad de autocuidado.RESULTADOS:Se incluyeron en este estudio 240 pacientes. Autocuidado de la estoma: Veinticuatro (31,2%), 51 (78,5%), y 72 (94,7%) pacientes de los grupos A, B, y C, respectivamente, pudieron realizar el cuidado de la estoma de forma independiente ( p < 0,001). En el análisis de subgrupos, entre los pacientes con ostomía temporal, los pacientes del grupo de prehabilitación también fueron mejores que los pacientes de los grupos B y C ( p < 0,001); sin embargo, no hubo una relación similar entre los pacientes con ostomía permanente. HADS: El grupo de prehabilitación tuvo menos pacientes con ansiedad y depresión con respecto a los demás grupos. Entre los pacientes con estomas temporales, el grupo de prehabilitación también tuvo menos pacientes con ansiedad y depresión. No hubo diferencias con la ansiedad o depresión entre los pacientes con estomas permanentes. Estoma-QoL (calidad de vida): Las puntuaciones de QoL (calidad de vida) fueron significativamente más altas en el grupo de prehabilitación ( p < 0,001). BACKGROUND: Although radical surgery for colorectal cancer improves the oncological outcomes, a significant portion of patients suffer from alterations in their quality of life (QoL). There are many studies investigating the QoL of patients who have colorectal cancer but none of these focus on the QoL of spouses. AIM: To compare the QoL of patients after colorectal surgery to the QoL of spouses. METHODS: This prospective study consisted of patients who were married and who underwent surgery at the University of Ankara, Department of Surgery between March 2006 and November 2010. Patients' spouses were also enrolled. The study was approved by the Ethics Committee of the Faculty of Medicine, Ankara University, and all patients provided written informed consent. The study included patients who underwent curative surgery for colorectal carcinoma [n = 100; abdominoperineal excision (n = 33), low anterior resection (n = 33), left hemicolectomy (n = 34)] and their spouses (n = 100). The patients and spouses completed the Medical Outcome Study 36-item Short Form Survey (SF-36) and the World Health Organization Disability Assessment Schedule II (WHODAS-II) preoperatively and at postoperative months 15 to 18. RESULTS: During this 4.5-year study period, 273 patients with sigmoid or rectal cancer were admitted to the hospital. Of these patients, 119 were eligible and willing to participate. Eleven patients had either systemic or locally inoperable disease, three patients had a severe surgical complication, and five patients were lost to follow-up. Therefore, a total of 100 patients completed the follow-up period. There was a statistically significant positive correlation between the disability scores of patients and the scores of their spouses for some of the WHODAS-II subscales, such as "self-care," "life activities," and "participation in society," as well as for the total WHODAS-II score. There was also a positive correlation between the QoL of patients and the QoL of their spouses in most of the SF-36 subscales. Statistically significant correlations were observed for the "bodily pain," "general health," "vitality," "social function," "emotion," "mental health," and mental component summary score subscales of the SF-36. When gender differences were evaluated, the QoL of male patients' spouses changed more when compared with female patients' spouses for all of the WHODAS-II subscales. Colorectal cancer surgery has a significant effect on the QoL of both patients and their spouses, these effects were more significant among male patients' spouses. CONCLUSION: Preoperative counseling regarding potential problems should therefore collectively address patient and their spouse as a couple rather than the patient alone, particularly for patients undergoing low anterior resection and abdominoperineal resection procedures.

RESUMO

Detecting nuclear materials in mixtures is challenging due to low concentration, environmental factors, sensor noise, source-detector distance variations, and others. This paper presents new results on nuclear material identification and relative count contribution (also known as mixing ratio) estimation for mixtures of materials in which there are multiple isotopes present. Conventional and deep-learning-based machine learning algorithms were compared. Realistic simulated data using Gamma Detector Response and Analysis Software (GADRAS) were used in our comparative studies. It was observed that a deep learning approach is highly promising.

Low lighting images usually contain Poisson noise, which is pixel amplitude-dependent. More panchromatic or white pixels in a color filter array (CFA) are believed to help the demosaicing performance in dark environments. In this paper, we first introduce a CFA pattern known as CFA 3.0 that has 75% white pixels, 12.5% green pixels, and 6.25% of red and blue pixels. We then present algorithms to demosaic this CFA, and demonstrate its performance for normal and low lighting images. In addition, a comparative study was performed to evaluate the demosaicing performance of three CFAs, namely the Bayer pattern (CFA 1.0), the Kodak CFA 2.0, and the proposed CFA 3.0. Using a clean Kodak dataset with 12 images, we emulated low lighting conditions by introducing Poisson noise into the clean images. In our experiments, normal and low lighting images were used. For the low lighting conditions, images with signal-to-noise (SNR) of 10 dBs and 20 dBs were studied. We observed that the demosaicing performance in low lighting conditions was improved when there are more white pixels. Moreover, denoising can further enhance the demosaicing performance for all CFAs. The most important finding is that CFA 3.0 performs better than CFA 1.0, but is slightly inferior to CFA 2.0, in low lighting images.

In this paper, we introduce an in-depth application of high-resolution disparity map estimation using stereo images from Mars Curiosity rover's Mastcams, which have two imagers with different resolutions. The left Mastcam has three times lower resolution as that of the right. The left Mastcam image's resolution is first enhanced with three methods: Bicubic interpolation, pansharpening-based method, and a deep learning super resolution method. The enhanced left camera image and the right camera image are then used to estimate the disparity map. The impact of the left camera image enhancement is examined. The comparative performance analyses showed that the left camera enhancement results in getting more accurate disparity maps in comparison to using the original left Mastcam images for disparity map estimation. The deep learning-based method provided the best performance among the three for both image enhancement and disparity map estimation accuracy. A high-resolution disparity map, which is the result of the left camera image enhancement, is anticipated to improve the conducted science products in the Mastcam imagery such as 3D scene reconstructions, depth maps, and anaglyph images.

Preflight contingency planning that utilizes wind forecast in path planning can be highly beneficial to unmanned aerial vehicles (UAV) operators in preventing a possible mishap of the UAV. This especially becomes more important if the UAV has an engine failure resulting in the loss of all its thrust. Wind becomes a significant factor in determining reachability of the emergency landing site in a contingency like this. The preflight contingency plans can guide the UAV operators about how to glide the aircraft to the designated emergency landing site to make a safe landing. The need for a preflight or in-flight contingency plan is even more obvious in the case of a communication loss between the UAV operator and UAV since the UAV will then need to make the forced landing autonomously without the operator. In this paper, we introduce a preflight contingency planning approach that automates the forced landing path generation process for UAVs with engine failure. The contingency path generation aims true reachability to the emergency landing site by including the final approach part of the path in forecast wind conditions. In the contingency path generation, no-fly zones that could be in the area are accounted for and the contingency flight paths do not pass through them. If no plans can be found that fulfill reachability in the presence of no-fly zones, only then, as a last resort, the no-fly zone avoidance rule is relaxed. The contingency path generation utilizes hourly forecast wind data from National Oceanic and Atmospheric Administration for the geographical area of interest and time of the flight. Different from past works, we use trochoidal paths instead of Dubins curves and incorporate wind as a parameter in the contingency path design.

Aggressive angiomyxoma is an uncommon, benign, slow-growing, and locally infiltrative soft tissue neoplasm which is located primarily in the genital region and pelviperineal interstitial tissue of female patient in the fourth decade of life. Its occurrence in male patients is even more unusual and commonly appears at a later age. The mainstay of treatment typically involves surgical excision with tumor-free margins, and despite complete resection, local recurrences are common. Here, an unusual case of aggressive angiomyxoma occurring in the pelvic region of a 55-year-old man and its treatment is discussed due to its rarity.


O angiomixoma agressivo é uma neoplasia benigna rara, de crescimento lento, localmente infiltrativa que se localiza primariamente no tecido intersticial da região genital, pélvica e perineal de doentes do sexo feminino na quarta década de vida. O aparecimento em doentes do sexo masculino é ainda mais incomum e habitualmente aparece em idade mais avançada. O tratamento tipicamente envolve a excisão cirúrgica com margens livres e, apesar da ressecção completa, a recidiva local é comum. Apresentamos o caso clínico raro de angiomixoma agressivo na região pélvica de um homem de 55 anos de idade bem como o seu tratamento.

BACKGROUND: In this study we aimed to determine the need for 18F-flourodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) in the preoperative staging of rectal carcinoma in our large patient group according to level and location of tumor. METHOD: Totally, 97 patients diagnosed with primary rectal adenocarcinoma between May 2009 and July 2011 were included in the study. Preoperative staging was performed by evaluating contrast-enhanced thoracic, abdominal, and pelvic computed tomographies. After staging by conventional methods, all patients underwent an 18F-FDG PET/CT. In all cases, the relationship between 18F-FDG uptake and gender, tumor height at the anal canal, localization in the rectal wall, plasma carcinoembryonic antigen levels, histopathological tumor type, and tumor stage were examined. RESULTS: While the ceCT was normal in 4 (4%) patients, it was positive for the rectum in 93 (95%), pelvic lymph nodes in 22 (22%), and distant metastases in 14 (14%) (liver (8), lung (8), bone (2), distant lymph nodes (6), and uterus (1)). Using computed tomography, disease stages were determined as stage 0, 1, 2, 3, and 4 in 4, 8, 48, 23, and 14 patients, respectively; 18F-FDG PET/CT was normal in two (2%) patients. The mean SUVmax of FDG-positive rectal tumors was calculated as 17.31 ± 9.37. Additionally, 18F-FDG uptake was seen in pelvic lymph nodes in 15 (15%) patients and in distant organs in 24 (24%) patients (liver (9), lung (12), bone (5), distant lymph nodes (11), uterus (1), and sigmoid colon (1)). According to an 18F-FDG PET/CT, 2, 7, 47, 20, and 21 patients were staged as stage 0, 1, 2, 3, and 4, respectively. In 14 patients (14.4%), the stage of the disease was either changed, and there was a need to make adjustments to the patient's treatment strategy (n = 10), or the type of operation was changed (n = 4). In seven patients (0.7%), findings from 18F-FDG PET/CT images did not require any changes of the treatment plan. CONCLUSION: F-FDG PET/CT provides new findings in addition to conventional techniques in the staging of primary rectal cancer. These findings could change the patients' treatment strategies.

Sideritis libanotica ssp. linearis was screened for the isolation of new natural antioxidant compounds. The antioxidant activity of flavones obtained from the methanol extract of the plant was evaluated in vitro using total antioxidant, reduction power and free radical scavenging activity. Results were compared with the positive controls of antioxidant standards (α-tocopherol and butylated hydroxytoluene). The results indicate that the flavones possess a higher antioxidant activity when compared to the other components in the plant. The lowest antioxidant activity was observed in fatty acids (FAs) and hydrocarbons. The FAs were methylated with MeOH and KOH and analysed by GC-MS. The structures of the isolated compounds were established based on spectroscopic evidence (NMR, GC-MS, HPLC, IR and UV). In this work, the isolated pure flavone (3'-O-methylhypolaetin 7-O-[6″'-O-acetyl-ß-D-allopyranosyl-(1 → 2)]-6″-O-acetyl-ß-D-glucopyranoside) was found to possess the highest antioxidant activity.

PURPOSE: Dysfunction of pelvic floor may cause many different symptoms, such as urinary and anal incontinence, obstructed defecation and constipation. No previous studies have examined all of these symptoms together. The purposes of the present study were to determine prevalence of pelvic floor disorders among the female population and to evaluate the impact of age, parity, and mode of delivery on these disorders. METHODS: The study was performed on a general population of Turkish women. Women were excluded who were pregnant, who were within 6 months postpartum, who had cognitive disorders or neurological diseases, and who had a history of previous gastrointestinal, anorectal, or gynecological surgery. A questionnaire about urinary incontinence, anal incontinence, constipation, and obstructed defecation along with an extensive obstetric history was administered to 4002 women in face-to-face interviews. All symptoms were defined according to the standard terminology. RESULTS: The median age of the participants was 41 years (range, 15-86). Of the women interviewed, 1067 had no deliveries, 434 women delivered by cesarean only, and 2501 women had one or more vaginal deliveries. Five hundred thirty women had delivered one child, 1880 women had delivered 2 to 3 children, and 582 women had delivered ≥4 children. Overall, 67.5% of women experienced pelvic floor dysfunction of at least one major type. The prevalence of each pelvic floor disorder evaluated in this study was as follows: anal incontinence, 19.8%; urinary incontinence, 50.7%; constipation, 33.2%; and obstructed defecation, 26.8%. Analysis of risk factors demonstrated that age was the major factor associated with the development of pelvic floor dysfunction. Vaginal delivery and higher parity increased the risk of both urinary and defecatory symptoms of pelvic floor dysfunction. CONCLUSION: The study data demonstrate that pelvic floor dysfunction is a common problem among women and it is strongly linked to childbirth and aging.

BACKGROUND AND AIM: Hepatitis B virus infection is an important public health problem in Turkey. Although hepatitis B vaccination is regarded as safe and effective for the general population, recommendations for hepatitis B immunization in patients with Behcet's disease are not clear. The aim of the present study was to elucidate the response of patients with Behcet's disease to hepatitis B vaccination and to determine whether hepatitis B vaccination has any adverse effects on the course of the disease. METHODS: Thirteen patients with Behcet's disease and 15 healthy individuals were enrolled into a prospective study. All subjects received the 3-dose series of routine hepatitis B vaccine. Anti-hepatitis B surface response was evaluated 1-3 months after the third dose of vaccine. RESULTS: The responder rates for patient and control groups were 12/13 (92.8%) and 14/15 (93.8%), respectively. Statistical analysis showed no significant difference between the two groups in terms of both the responder rates and mean antibody titers. CONCLUSIONS: These preliminary findings might suggest that the majority of patients with Behcet's disease develop protective antibody response after hepatitis B vaccination and that the immune response against hepatitis B surface antigen is adequate, efficient and intact.

This experimental study was designed to assess and to compare intra-abdominal adhesions following the use of five commercially available prosthetic mesh grafts in the repair if abdominal wall defects. Sixty Wistar albino rats were randomly divided into six groups (n = 10). A 2 x 1 cm defect at abdominal wall was created and defects were closed either primarily or with one of the following prosthetic mesh grafts: monofilament polypropylene, polytetrafluoroethylene, sodium hyaluronate/carboxymethylcellulose-coated polypropylene, polypropylene/polyglactin 910 composite, or resorbable hydrophilic collagen-coated multifiber polyester. The severity of adhesions was graded, tensile strengths of adhesions were measured, and histopathological grades of inflammation and fibrosis were evaluated. Polypropylene mesh resulted in more adhesion formation in comparison to primary repair and other grafts used in this study, except polypropylene/polyglactin 910 composite mesh. In addition, the highest tensile strength of omental adhesions was detected in the polypropylene group (chi2 = 26.249; p = .0001). Polyester composite mesh caused the least adhesion formation among the groups. Sodium hyaluronate/carboxymethylcellulose-coated polypropylene and polyester composite meshes revealed the highest fibrosis scores (chi2 = 50.776; p = .0001). The highest inflammatory activity was detected in the polytetrafluoroethylene mesh group (chi2 = 16.564; p = .005). Thus, sodium hyaluronate/carboxymethylcellulose-coated polypropylene and polytetrafluoroethylene meshes following polyester composite mesh were the minimal adhesion-forming grafts in this study. Disadvantages of the polytetrafluoroethylene mesh were lower fibrotic activity and higher inflammatory reaction to the graft.
