Purpose: There is little information about the association between zinc sulfate (ZnSO4) supplementation and metabolic profiles in zinc-deficient diabetic patients on hemodialysis (DHPs). Therefore, we aimed to investigate the association between ZnSO4 supplementation and serum levels of galectin-3 (Gal-3) and cardiometabolic parameters in zinc-deficient DHPs. Methods: In the present randomized double-blind placebo-controlled clinical trial, 46 zinc-deficient DHPs (35-62 years) were included and assigned to receive either 220 mg/d ZnSO4 or placebo for 8 weeks. Serum levels of Gal-3, lipid profile, and blood pressure (BP) were assessed at baseline and the end of trial. Results: We found a significant effect of ZnSO4 intake on the reduction of serum Gal-3 (P = < 0.001), triglycerides (P = < 0.001), total cholesterol (P = < 0.001), low-density lipoprotein cholesterol (P = < 0.001) and increased high-density lipoprotein cholesterol (P = < 0.001) as compared to the control group. Additionally, systolic blood pressure (SBP) (P = 0.006) and diastolic blood pressure (DBP) (P = 0.01) were significantly reduced following 8 weeks of ZnSO4 supplementation. Conclusion: Taken together, 220 mg ZnSO4 supplementation per day for 8 weeks among zinc-deficient DHPs had beneficial effects on Gal-3 and metabolic profiles. BACKGROUND & AIMS: Critically ill patients are provided with the intensive care medicine to prevent further complications, including malnutrition, disease progression, and even death. This study was intended to assess nutritional support and its' efficacy in the Intensive Care Units (ICUs) of Iran. METHODS: This cross-sectional study assessed 50 ICU's patients out of 25 hospitals in the 10 major regions of Iran's health system and was performed using the multistage cluster sampling design. The data were collected from patient's medical records, ICU nursing sheets, patients or their relatives from 2017 to 2018. Nutritional status was investigated by modified NUTRIC score and food frequency checklist. RESULTS: This study included 1321 ICU patients with the mean age of 54.8 ± 19.97 years, mean mNUTRIC score of 3.4 ± 2.14, and malnutrition rate of 32.6%. The mean time of first feeding was the second day and most of patients (66%) received nutrition support, mainly through enteral (57.2%) or oral (37%) route during ICU stay. The patients received 59.2 ± 37.78 percent of required calorie and 55.5 ± 30.04 percent of required protein. Adequate intake of energy and protein was provided for 16.2% and 10.7% of the patients, respectively. The result of regression analysis showed that the odds ratio of mNUTRIC score was 0.85 (95% confidence interval [CI] = 0.74-0.98) and APACHE II was 0.92 (95%CI = 0.89-0.95) for the prediction of energy deficiency. Nutrition intake was significantly different from patient's nutritional requirements both in terms of energy (p < 0.001) and protein (p < 0.001). Also, mean mNUTRIC score varied notably (p = 0.011) with changing in energy intake, defined as underfeeding, adequate feeding, and overfeeding. CONCLUSION: The present findings shown that, provided nutritional care for ICU patients is not adequate for their requirements and nutritional status.

BACKGROUND: Obesity and Overweight at an early age can contribute with many chronic diseases such as cancers, diabetes and cardiovascular diseases. Diet-related inflammation is one of the most important underlying mechanisms that may has a key role in obesity and overweight. This paper aimed to compare the dietary inflammatory index (DII®) in normal weight and overweight adolescent boys. METHODS: A total of 535 adolescent boys (214 cases were overweight and obese and 321 controls with normal weight) participated in this study from two schools in Tehran, Iran. The student's weight and body composition were measured using a Bio-Impedance Analyzer (BIA) scale. A validated semi-quantitative Food Frequency Questionnaire (FFQ) was used to assess dietary inflammatory index. RESULTS: Results obtained from modeling DII® as a continuous variable identified a positive association between DII® and obesity (OR = 1.08, CI 1.01-1.16). After multivariable adjustment, subjects with DII® > 0.02 had at 1.5 times higher odds of obesity and overweight compared to subjects with DII® ≤ 0.02 (OR = 1.52; CI 1.04-2.22). CONCLUSION: Our study indicated the importance of dietary-induced inflammation in the obesity and overweight during adolescence. Therefore, advising adolescent to consume diet with lower DII® with more fruits and vegetables, rich sources of fiber, flavonoids, zinc, magnesium and selenium and avoiding the consumption of saturated fatty acids (SFA), trans-fatty acids, and cholesterol may support a healthy weight.
