AIM: To study the impact of incorporating micro-nano-bubbles (MNBs) in commonly used food antimicrobials (AMs) against Escherichia coli O157:H7 (EC) and Listeria monocytogenes (LM). METHODS AND RESULTS: Air, carbon dioxide (CO2 ) and nitrogen (N2 ) were used to incorporate MNBs in city water. AM solution (with or without MNBs) of 9 ml was individually taken into sterile test tubes and mixed with 1 ml of inoculum grown in brain heart infusion (BHI) broth to get the net AM concentrations of 28·4 ppm peracetic acid (PAA), 200 ppm chlorine (Cl2 ), 5·4% citric acid (CA) and 4·5% lactic acid (LA). After treatment time of 1·5 and 3·0 min, 1 ml of sample was neutralized using Dey-Engley neutralizing broth and plated on BHI agar. For EC, Cl2 -CO2 solutions resulted in significantly greater log reductions (5·2 logs) compared to that of Cl2 solutions without MNBs (3·8 logs). For LM, PAA-CO2 solutions resulted in significantly greater log reductions (4·4 logs) compared to that of PAA solutions without MNBs (1·7 logs). CONCLUSIONS: This study demonstrated that the efficacy of Cl2 and PAA AM solutions could be increased by incorporating CO2 -MNBs against EC and LM in microbiological growth medium. SIGNIFICANCE AND IMPACT OF THE STUDY: Incorporation of CO2 -MNBs in AM solutions could increase the efficacy of AMs against pathogens on/in food matrices, which should be tested in future research.

OBJECTIVES: Examine the relationship between supply of care provided by dental therapists and emergency dental consultations in Alaska Native communities. METHODS: Explanatory sequential mixed-methods study using Alaska Medicaid and electronic health record (EHR) data from the Yukon-Kuskokwim Health Corporation (YKHC), and interview data from six Alaska Native communities. From the Medicaid data, we estimated community-level dental therapy treatment days and from the EHR data we identified emergency dental consultations. We calculated Spearman partial correlation coefficients and ran confounder-adjusted models for children and adults. Interview data collected from YKHC providers (N=16) and community members (N=125) were content analysed. The quantitative and qualitative data were integrated through connecting. Results were visualized with a joint display. RESULTS: There were significant negative correlations between dental therapy treatment days and emergency dental consultations for children (partial rank correlation = -0.48; p⟨0.001) and for adults (partial rank correlation = -0.18; p=0.03). Six pediatric themes emerged: child-focused health priorities; school-based dental programs; oral health education and preventive behaviors; dental care availability; healthier teeth; and satisfaction with care. There were four adult themes: satisfaction with care; adults as a lower priority; difficulties getting appointments; and limited scope of practice of dental therapy. CONCLUSIONS: Alaska Native children, and to a lesser extent adults, in communities served more intensively by dental therapists have benefitted. There are high levels of unmet dental need as evidenced by high emergency dental consultation rates. Future research should identify ways to address unmet dental needs, especially for adults.

It identifies the current evidence base and role of health-related quality of life assessment for this group of patients. Recommendations • Health-related quality of life is integral to treatment planning, refining treatment protocols, and more personalised follow-up support. (G) • Health-related quality of life and patient concerns should be regularly assessed during patient care. (G) • Health-related quality of life assessment and patient concerns on an individual patient basis can be helpful to trigger multi-professional support and interventions. (G).

OBJECT: This study was undertaken to analyze factors associated with the size of unilateral vestibular schwannomas (VSs). METHODS: A retrospective analysis of an unselected and sequential series of 433 patients with unilateral VSs was conducted. Tumor size was defined by the largest dimension of the tumor in the cerebellopontine angle, and the size was tested for a relationship with patient age and sex. In a subgroup of 231 patients in whom data were available, tumor size was also tested for a relationship with tumor cysts or the absence of an internal auditory canal (IAC) component. Some patients underwent a period of surveillance with serial imaging studies to monitor for tumor growth. Data from these patients, excluding those with cystic tumors, were analyzed to see if tumor growth was related to patient age, sex, or tumor size. CONCLUSIONS; Larger tumors were found in younger patients, in females, in the subgroup of cystic tumors, and in patients in whom there was no tumor component in the IAC. The probable explanations for these larger tumors are a faster growth rate and/or a delay in symptom onset. When untreated tumors are managed with observation, measurable growth is more often seen in larger tumors, although smaller tumors have a faster relative growth rate than larger ones.

Mucosa-associated lymphoid tissue (MALT) is a specialized form of lymphoid tissue that may be acquired at sites in response to chronic inflammation. Most low-grade, B-cell, non-Hodgkin lymphomas that occur at extranodal sites derive from acquired MALT. Confusing and overlapping terms have been used to describe these lymphomas, but immunohistochemical advances now allow more precise subtyping. Our review of the literature yielded only 2 previous reports of sinonasal MALT-derived lymphoma, and we report an additional case in a patient with a history of chronic sinusitis. Current developmental theories of MALT-derived lymphomas are discussed. In addition, we review the clinical, histologic, and immunophenotypic features of MALT-derived lymphomas.

Although the role of laparoscopic cholecystectomy (LC) as a safe and cost effective procedure has been ascertained, its role in the geriatric population, the majority of whom present with coexistent diseases, has yet to be defined. We retrospectively reviewed outcome parameters of 144 consecutive patients over age 65 undergoing LC, for both acute cholecystitis and symptomatic cholelithiasis. These results were compared with 72 patients having open cholecystectomy (OC) during the same time period and in the year preceding the introduction of LC. Groups were well matched with respect to age, age distribution indication for surgery, and underlying comorbid illnesses. Of those with symptomatic cholelithiasis, LC did not prolong operative time when compared with OC, but resulted in significantly earlier discharge (1.8 +/- 2.9 vs. 6.7 +/- 5.7 days (P < 0.0001)), with comparable hospital costs and with no increase in postoperative complications. With respect to acute cholecystitis, LC significantly prolonged operative time (105.8 +/- 40.8 vs. 78.1 +/- 28.5 minutes (P < 0.05)), but when successful, significantly reduced postoperative stay (4.2 +/- 3.8 vs. 7.5 +/- 2.3 days (P < 0.05)). There was no increase in postoperative complications in those having LC, and hospital costs were comparable with OC. Seven patients were converted from LC to OC; 4 of these (16%) were for acute cholecystitis versus a 2.5 per cent incidence of conversion for symptomatic cholelithiasis, and these resulted in prolonged hospital stays and costs. There was no incidence of hypotension/hypercarbia, despite a 64 per cent incidence of cardiopulmonary cardiopulmonary diseases in those having LC. There was a 14 per cent incidence of cardiopulmonary complications in those having LC in contrast to a 43 per cent incidence in OC. LC in the geriatric population is a safe procedure for symptomatic cholelithiasis. The procedure should be undertaken with caution in those with acute cholecystitis with a low threshold for either early conversion or primary OC. Finally, our results suggest that extensive hemodynamic monitoring is not indicated.

This is the era of polypharmacy. Treatment with multiple therapeutic agents for multiple underlying medical conditions leads to greater vulnerability to such metabolic imbalances as hyperkalemia. The physician must know which underlying medical conditions and drugs predispose to abnormal intracellular-extracellular potassium balance and abnormal regulation of potassium excretion.
