RESUMO

Familial Mediterranean fever (FMF) is an autosomal recessive disease distributed among populations of Mediterranean origin - Armenians, Sephardi Jews, Arabs, Turks. There are numerous clinical observations regarding combination of FMF, as a classical representative of autoinflammatory diseases, with systemic diseases of connective tissue. Seronegative spondyloarthritis (SpA) are the most interesting disorders from this point of view, as far as sacroiliitis - an essential feature of SpA, may also present as a part of joint syndrome in FMF. The main objective of this clinical study was the investigation of the peculiarities of courses of FMF and SpA in case of their coexistence. We studied 126 patients with FMF, SpA and coexistence of both. According to results, patients with the overlap of FMF with SpA had relatively milder course of disease in comparison with each disease separately. Comparative clinical and instrumental characteristics of FMF-associated disorders had shown that in FMF-SpA overlap the symptoms of both diseases are less severe.

AIM: Determination of the concentration of interleukin-10 (IL-10) and interleukin-6 (IL-6) in serum of patients with periodic disease (PD) before and after treatment with colchicin, as well as the identification of correlation between the indicators of these parameters. MATERIALS AND METHODS: We examined 188 patients with PD (89 men, 99 women) aged from 12 to 69 years, as well as 44 patients with rheumatoid arthritis (RA) as a comparison group and 41 healthy people of the control group. Patients were divided into groups: 1 - PD colchicinotherapy patients with seizures without amyloidosis that do not respond to treatment maximum dose of colchicine 2.0 mg/day; 2nd-PD patients without amyloidosis, not responding to treatment 1.5 mg/day colchicine; 3rd - PD patients that responds to certain doses of colchicine (0.5 to 2.0 mg/day); 4th - PD patients without amyloidosis who did not receive treatment; 5-I of the healthy persons of the control group; 6-I - RA patients. The concentration of IL-10 in blood serum was determined by enzyme immunoassay ELISA, and IL-6 - immunochemiluminescent method. For statistical processing the computer program SPSS is used. The results were considered statistically significant at the level of reliability p<0.05. RESULTS: The results of the studies showed the same nature of changes in IL-10 production in different groups of patients. Statistically significant positive correlation of elevated serum concentrations of IL-6 and IL-10 (p<0.05) was found in patients with PD of all groups, as well as in patients with RA. CONCLUSION: In patients with PD (both colchicin-resistant and colchicin-sensitive) increased serum concentration of IL-10 was accompanied by an increased level of IL-6 in serum. Changes in the level of IL-10 in PD have a certain prognostic and pathogenetic significance and lead to the development of "persistent, sluggish" inflammatory process in the extracurricular period of PD in both colchicin-resistant and other groups of patients with PD.

The therapeutic effect of doxazosin (40 µg/kg/day over one month) on urinary bladder was examined in female rats with modeled chronic infravesical obstruction (IVO) produced by graduated mechanical constriction of the proximal urethral segment. In one month, IVO induced a pronounced vesical hypertrophy both in treated and untreated rats that manifested in increased bladder weight and capacity, the latter increment being pronouncedly greater in treated rats. In untreated IVO rats, infusion cystometry revealed elevated basal intravesical pressure of void bladder P0, markedly increased maximal (premicturitional) pressure Pmax, and increased amplitude of spontaneous oscillations of intravesical pressure ΔPdet in filled bladder. Doxazosin produced no significant effect on Pmax rise during IVO, but prevented elevation of P0 and increment of ΔPdet in filled bladder. During gradual filling of urinary bladder in control (intact) rats, the parasympathetic vesical influences increased progressively, while in untreated IVO rats, the adrenergic influences prevailed even at maximal filling of the bladder. In IVO rats, doxazosin prevented the bias of the sympathetic-parasympathetic balance in the filled bladder in favor of sympathetic influences, but did not prevent this bias in a void bladder. It is hypothesized that α-adrenoblockers improve micturition during IVO caused by benign prostatic hyperplasia not only by decreasing the urethral resistance to urine flow due to down-regulation of prostate smooth muscle tone, but also by a direct action of these blockers on detrusor adrenergic receptors and central structures involved in urinary bladder control.

High-resolution impedancometry and harmonic (Fourier) analysis of variable component of bioimpedance revealed rhythmic oscillations of urinary bladder bioimpedance at the Mayer wave, respiration, and heartbeat frequencies. The power values of the corresponding Mayer, respiratory, and cardiac peaks were calculated to assess circulation in the urinary bladder wall and its autonomic nervous control at various stages of infusion cystometry in intact rats and in the rats with preliminary formed infravesical obstruction (IVO). In intact rats, filling of the bladder with physiological saline diminished the power of the first (fundamental) cardiac peak attesting to a decrease of the blood flow in the bladder wall. Simultaneously, the power of low-frequency Mayer peak reflecting sympathetic activity increased, while the power of respiratory peak decreased supposedly reflecting abatement of the parasympathetic influences. Bladder voiding was accompanied by a decrease of Mayer peak and increase of the respiratory one. Prior to infusion cystometry, the intravesical pressure in IVO rats was elevated while the power of fundamental cardiac peak was below the control value. Filling the bladder in these rats was accompanied by further decrease of the cardiac peak reflecting still greater drop in blood supply. In control rats, voiding the bladder normalized the vesical circulation assessed by the cardiac peak, while in IVO rats this peak remained decreased. The reciprocal changes of Mayer and respiratory peaks observed during infusion cystometry in the norm were replaced by unidirectional decrease in the power of both peaks in IVO rats, which probably attest to disturbance of autonomic nervous control in the hypertrophic urinary bladder in these rats.

Decrease of enzyme activity in the bladder wall in combination with increase of enzyme activity in the urine, affirm that exactly the bladder is the source of enzymuria increase and it is related with marked damage. Taking into consideration, that from the bladder wall not only cytoplazmatic enzymes come out but also enzymes connected with cell membranes, it is expected that high enzymuria is not only because of increase permeability of cytoplazmatic membrane for large molecules but also is connected with cell parts distruction. Similarity of changes during acute urinary retention and bladder wall ischemia let us assume vital value of the factor of changes of bladder wall blood supply during acute urinary retention and its role in the development of metabolic and functional changes.

RESUMO

During high pressure in the bladder, particularly during delay of urination because of infravesical obstruction, raisings of blood pressure were marked with simultaneous increase of renal and spleen sympathetic nerves activity. During urination the activity of sympathetic nervous system decreases while the activity of parasympathetic system which is responsible for detrusor contraction increase. All these data testify to changes of vegetative regulation of hypertrophic bladder during inftavesical obstruction, but on the other hand mechanism of those changes and their functional consequences require further studies.

RESUMO

The analysis of the data presented in the review convincingly shows the correlation between infravesical obstruction (IVO), caused by pressing of urethra by enlarged prostate, and the development of detrusor hyperactivity. The clinical importance of correlation between IVO and hyperactivity of the detrusor is convincingly illustrated by different results of effectiveness of surgical treatment of patients with benigh prostate hyperplasia (BPH) with and without hyperactivity of bladder. In patients with BPH and hyperactivity of bladder the probability of irritative symptoms is high. This data proves the necessity of the further study of the pathogenesis of development of bladder hyperactivity in patients with BPH, and the importance of a preoperative estimation of functional frustration of the bladder.
