RESUMO

Systemic lupus erythematosus (SLE) is the prototype of autoimmune diseases with multiple autoantigens as targets,resulting in damage to many organs of the body. The disease is more common in females (> 90%) and around 15% of the cases present during childhood.Systemic lupus erythematosus is a complex disease in which both genetic (susceptibility/protection alleles) and environmental factors (infections, drugs, stress, etc.) contribute to its development.The current knowledge on genetic factors involved in SLE is based on the results of linkage analyses in multi-case families as well as from case-control or family-based genetic association studies. These types of genetic analyses have contributed to identifying susceptibility genes and constitute the first step towards understanding the molecular mechanisms underlying SLE.The aim of this review is to provide a current picture of the genes identified as susceptibility factors for SLE, and to highlight the ones described in the Mexican population.

Introducción. Hasta 43% de pacientes pediátricos con enfermedad reumática que reciben esteroides llegan a presentar obesidad. El objetivo de este estudio fue determinar la evolución del estado de nutrición en niños con enfermedad reumática que reciben esteroides como parte de su tratamiento. Métodos. Se realizó un estudio prospectivo de dos cohortes, vigiladas durante 24 semanas. Se incluyeron pacientes con enfermedad reumática de diagnóstico reciente. Se formaron dos grupos, sin tratamiento (grupo A) y con tratamiento con esteroides (grupo B). Se realizaron determinaciones de peso, talla, circunferencia de cintura, perímetro braquial y porcentaje de grasa corporal cada 6 semanas, previa estandarización. Resultados. Se analizaron 20 pacientes del grupo A y 32 del grupo B. En el grupo A se presentó con mayor frecuencia la artritis reumatoide juvenil (40%) y en el grupo B, el lupus eritematoso sistémico (56%). Al inicio y durante el periodo de estudio no hubo diferencias en los promedios de cada una variables antropométricas estudiadas entre los dos grupos; sin embargo, hubo 4 casos nuevos de obesidad en el grupo B (12.5%). En 11 pacientes del grupo A (55%) y 18 del B (56%) se incrementó el porcentaje de grasa; en estos pacientes, solo los del grupo B tuvieron ganancia significativa en las variables antropométricas (p < 0.01). Conclusiones. Entre los pacientes pediátricos con enfermedad reumática que reciben prednisona a dosis bajas, parece que existe mayor probabilidad de incrementar significativamente de peso.


Background. Prevalence of obesity in pediatric patients with rheumatic disease (RD) receiving steroids can be as high as 43%. The aim of this study was to determine the association between low-dose of prednisone and weight gain among children with RD. Methods. We carried out a prospective cohort study. One cohort was comprised of patients exposed to steroids (group B) and the other group was comprised of patients not exposed to steroids (group A). After standardization, all patients were followed for 24 weeks and weight, height, waist circumference, arm circumference and body fat percentage were assessed. Results. There were 20 patients in group A and 32 in group B. In the first group, juvenile rheumatoid arthritis was the main diagnosis (40%) and systemic lupus erythematosus (56%) in the second one. Between groups, from the beginning and during the entire study period there was no difference in the averages of each anthropometric variable, but there were four (12.5%) new cases of obesity in group B and none in group A. Eleven (55%) patients in group A and 18 (56%) of group B showed an increase in the percentage of fat; of these, only in group B patients was there was a statistically significant gain (p <0.01) in all the anthropometric variables studied. Conclusions: Among pediatric patients with RD, receiving low-doses of prednisone seems to be a factor in weight increase.

RESUMO

Introducción. El presente trabajo es un reporte de cuatro pacientes con enfermedad reumática autoinmune que presentaron retinopatía secundaria al uso de cloroquina. Casos clínicos. Dos pacientes con artritis reumatoide juvenil desarrollaron esta compliación tardíamente, es decir después de 14 y 20 meses de tratamiento. En constraste, dos pacientes con lupus eritomatoso sistémico la desarrollaron a los tres y seis meses. Conclusiones. Ambos pacientes con lupus inciaron el cuadro clínico con manifestaciones a nivel de sistema nervioso central lo que pudiera ser un factor predisponente para el desarrollo de retinopatía secundaria al uso de cloroquina
