Marjolin's ulcer is a well-recognized, rare, long-term complication of post-burn scar and other chronically irritating local skin conditions. Its infiltration into the skull bones and the underlying dura causing complete destruction is very rare and quiet exceptional. We present an unusual case of spontaneous brain fungation through the scar tissue present over the scalp as a result of flame burn 18 years back. Patient was treated surgically, but expired after 10 days. Though we were not able to save the patient, its presentation was so unique that we want to report this case.

Ophthalmoplegia due to 3(rd) nerve palsy is a common occurrence, and is usually a sign of diabetes mellitus or a serious intracranial disease. We report a rare case of pupil sparing 3(rd) nerve palsy caused by mucocele of the sphenoid sinus. The patient regained 3(rd) nerve functions after successful decompression of the mucocele. Early and correct diagnosis of this relatively benign condition is important to prevent permanent neurological deficits, including visual loss by optic nerve atrophy. Etiology, clinical manifestations and treatment of the sphenoid mucoceles is discussed and available literature is reviewed.

Penetrating injuries other than gunshot wounds or low-velocity wounds to the head are extremely rare. We report the case of a 19 year old male who sustained a penetrating craniocerebral trauma following a road traffic accident. Noncontrast CT scan revealed three foreign bodies embedded in left frontal lobe, which on surgery turned out to be human teeth. Mechanism of injury seems to share characteristics of low velocity projectiles. The survey of management pattern of these injuries showed a general agreement about CT scan, antibiotics and anticonvulsants. The prompt management resulted in an excellent outcome. We discuss the management of this unusual case reviewing the current literature on craniocerebral injuries caused by similar objects.

Spinal hematoma is an infrequently described complication of spinal anaesthesia. It has been described in the literature in patients with a deranged coagulation profile in the form of systemic diseases (e.g. chronic renal failure, liver failure) or anticoagulant therapy. Whatever the reason, it will have disastrous consequences in the form of permanent paraplegia or persistent neurological deficit if not treated promptly. One therefore needs to maintain a high index of suspicion at all times in any patient who has undergone spinal anesthesia and who exhibits any sign or symptom of a neuraxial hematoma We report a case of post-spinal puncture subdural hematoma in an otherwise healthy male without any predisposing factor.

Human tail or tail-like caudal cutaneous appendage is a rare fingerlike, midline protrusion at the lumbosacrococcygeal region, often associated with occult spinal dysraphism. A 2-month-old male child presented here had a lumbosacral tail-like appendage with underlying spinal dysraphism without any appreciable neurological deficit. In contradiction to a previous report, true vestigial tails are not benign because they may be associated with underlying dysraphic state. About 50% of the cases were associated with either meningocele or spina bifida occulta. Management of such lesions must include complete neurological history and examination as well as magnetic resonance or computed tomographic imaging. After diagnosis, microsurgery should be performed if there is any intraspinal component to avoid any damage and neurological deficit.

Two finite concentric spherical regions were considered as the tissue model for magnetic fluid hyperthermia treatment. The inner sphere represents the diseased tissue containing magnetic particles that generate heat when an alternating magnetic field is applied. The outer sphere represents the healthy tissue. Blood perfusion effects are included in both the regions. Analytical and numerical solutions of the one-dimensional bioheat transfer equation were obtained with constant and spatially varying heat generation in the inner sphere. The numerical solution was found to be in good agreement with the analytical solution. In an ideal hyperthermia treatment, all the diseased tissues should be selectively heated without affecting any healthy tissue. The present work optimized the magnetic particle concentration in an attempt to achieve the ideal hyperthermia conditions. It was found that, for a fixed amount of magnetic particles, optimizing the magnetic particle distribution in the diseased tissue can significantly enhance the therapeutic temperature levels in the diseased tissue while maintaining the same level of heating in the healthy tissue.

We present an interesting case of sacral perineural cyst which caused chronic perineal pain. Perineural cyst is relatively rare, especially the sacral region. Chronic perineural pain is an often encountered problem that is difficult to evaluate and sacral perineural cyst may be the etiology of chronic perineal pain in many instances.
