CONTEXT: Positive airway pressure (PAP) is the first-line therapy for obstructive sleep apnea (OSA). Overnight PAP titration for determining optimal PAP requirements is expensive and often inconvenient. Prediction of optimal PAP requirements from diagnostic polysomnography via mathematical equations is possible but variable across populations. AIMS: We aimed to (1) determine the optimal PAP requirement, (2) determine differences in optimal PAP requirements across OSA severity groups, (3) determine the relationship between optimal PAP requirement and diagnostic polysomnography measurements of OSA severity, and (4) develop a pilot equation to predict the optimal PAP requirement from diagnostic polysomnography in a sample from the Saudi population. METHODS: We analyzed records pertaining to adult OSA patients (n = 215; 63% of males) who underwent standardized diagnostic and titration polysomnography in our sleep laboratory between 2015 and 2019. Demographic, anthropometric, and clinical information were also collected for the analysis. Inferential statistics were performed for comparisons between diagnostic and titration studies and between OSA severity groups. Regression analyses were also performed to determine the potential predictors of optimal PAP requirements. Data were presented as the mean (± standard deviation) or median (25th-75th quartiles) according to normality. RESULTS: The median optimal PAP requirement was 13 (9-17) cmH2O. The optimal PAP requirement was significantly greater for male versus female participants (14 [10-17] vs. 12 [8-16] cmH2O) and for participants with severe OSA (16 [12-20] cmH2O, n = 119) versus those with moderate (11 [8-14] cmH2O, n = 63) or mild (9 [7-12] cmH2O, n = 33) OSA. When combined, nadir oxygen saturation, oxygen desaturation index, and arousal index could be used to predict the optimal PAP requirement (R 2= 0.39, F = 34.0, P < 0.001). CONCLUSIONS: The optimal PAP requirement in the Saudi population is relatively high and directly correlated with OSA severity. Diagnostic polysomnography measurements of OSA severity predicted the optimal PAP requirement in this sample. Prospective validation is warranted.

Introduction Isotretinoin is one of the most commonly prescribed drugs among dermatologists because it's used in the treatment of Acne vulgaris. Despite having an adequate safety profile, isotretinoin-related adverse events are common, with ocular manifestations being one of them. Although being generally mild, ophthalmologic manifestations associated with isotretinoin may cause significant ocular morbidity. Objectives The objective is to evaluate the awareness of the ocular side effects of isotretinoin treatment in patients with acne and to understand the treating physicians' practice of prescribing isotretinoin from the patient's point of view. Method A descriptive cross-sectional study was conducted on a Saudi Arabian population from June to September 2021. We used a self-administered questionnaire to collect data on drug dose, treatment duration, ocular adverse effects, patients' awareness, and patients' perception of treating physicians' practice. Results Of 1,157 responders, 452 (39.1%) consisted of participants who met the inclusion criteria, therefore they were enrolled in the study. Out of our responders, 308 (68.1%) were women. Approximately, 91.6% of participants had a dermatologist's prescription for isotretinoin. Eye dryness was the most commonly reported ocular adverse effect (83.4%). Blepharitis and conjunctivitis were the most commonly diagnosed complications after starting isotretinoin (5.4% for each). Approximately, 71.2% participants were not advised to see an ophthalmologist for ophthalmological screening. Physicians asked only 78 (17.3%) respondents about a recent refractive surgery before starting isotretinoin or if they intended to undergo a refractive surgery in the next six months. Approximately, 57.5% participants were unaware of possible contact lens intolerance. Lubricant eye drops were prescribed to 305 (67.5%) respondents during isotretinoin treatment. Approximately, 36.7% participants were advised to consult their treating physicians if they had any serious ocular signs or symptoms. Approximately, 67.9% participants reported their awareness of the ocular side effects of isotretinoin therapy. However, 236 (52.2%) participants responded that they were not provided with sufficient information on the ocular side effects of isotretinoin therapy.

Objectives The purpose of this study was to examine the effects of social media on acne treatment among the Saudi Arabian population. Methods This was a cross-sectional survey-based study conducted from January 2021 to August 2021. A self-administered survey was distributed through social media to different regions of Saudi Arabia. The survey obtained participants' sociodemographic information and details on whether people used social media for advice on acne treatment. It also asked whether they noticed any change in their acne based on social media recommendations. Results Of the 5,539 respondents, 4,227 experienced acne, of which 1,793 were influenced by social media. Most respondents were women. The majority of social media users were between 18 and 25 years old and chose social media as their first approach for acne advice. The most commonly used platform was Instagram (34%). The most frequent social media recommendation chosen was to increase water intake. Many noticed a slight change in their acne (64%), and 14.9% had side effects. There was a significant association between the use of social media for advice and gender (p-value < 0.001), education level (p-value = 0.002), and severity of acne (p-value < 0.001). Conclusion Social media has an influence on acne treatment, with many advice not recommended by established guidelines. These findings imply that dermatologists should pinpoint inaccuracies resulting from advice found on social media.

Background Breast cancer accounts for 11.6% of all neoplasms worldwide and is the commonest cancer among Saudi females (29.7%). Chemotherapy-induced nausea and vomiting (CINV) is a very common side effect of chemotherapy that has a great impact on the quality of life (QoL) of patients. Literature is still scarce about this effect on the Saudi population. This study aims to explore breast cancer patients' perception of their experience with CINV and its impact on QoL. Methods This is a cross-sectional retrospective study conducted on Saudi adult female breast cancer patients who underwent chemotherapy at King Abdulaziz Medical City, Jeddah, Saudi Arabia. Data collected through patients' records review, face-to-face and telephone structured interviews using a questionnaire composed of four parts: sociodemographic characteristics, nature of acute CINV (within 24 hours) and delayed CINV (after 24 hours), impact on QoL, and general information on their experience. Results Out of a total population of 173, 98 (56.65%) patients participated in the study. The main findings show that 78.6% experienced nausea, whereas 35.7% experienced vomiting. Most participants had a moderate-to-extreme impact on their QoL due to nausea (74.0%) and vomiting (62.9%). Overall, 57.5% rated anti-emetics as excellent for controlling CINV, whereas 22.9% rated them as moderate to good; 83.5% were completely compliant on anti-emetics and 71.1% reported that they received completely comprehensive education about CINV. Religious practices (74.4%), diet (57.7%), and relaxation techniques (44.9%) were found to be the most common non-pharmacological methods used to control CINV. No significant correlation was found between the effect of CINV on QoL and sociodemographic characteristics (p > 0.05). Conclusions CINV is very common among Saudi adult female breast cancer patients; despite being completely compliant and receiving comprehensive education and effective anti-emetics; CINV still had a high impact on different aspects of QoL. Health care professionals should consider CINV as an issue and should find effective strategies for alleviating patients' suffering.
