Objective: This study aimed to assess and compare the effects of intranasal administration of lidocaine and remifentanil on the condition of LMA insertion and cardiovascular response. Methods: From March 2019 to March 2020, this double-blind randomized clinical trial study was conducted on 60 patients, who underwent general anesthesia with LMA insertion at Faiz Hospital, Isfahan, Iran. After induction of anesthesia and before placing the laryngeal mask, the first group received remifentanil 1 µg/Kg, the second group received lidocaine 2% 1 mg/Kg, and the third group received normal saline with the same volume intranasally. The conditions of LMA insertion and hemodynamic changes that occurred during its insertion were investigated. Results: In terms of demographics characteristics (p>0.05), success in placing the LMA on the first try (p=0.73), number of attempts to insert LMA (p=0.61), performance of LMA (p=0.73), need for additional propofol (p=0.53), frequency of gagging (p=0.53), cough (p=0.15) p), and laryngospasm (p=0.99) did not differ significantly. In the remifentanil group, the cardiovascular response to LMA injection was less than that of the lidocaine group. Moreover, both groups were lower than the saline group, but no significant difference was observed. Conclusion: In facilitating LMA insertion, the effect of intranasal remifentanil was comparable to intranasal lidocaine. Intranasal remifentanil was somewhat more effective than intranasal lidocaine in weakening the cardiovascular response to LMA insertion, but it did not outperform lidocaine.

BACKGROUND: Creating a stable and long-term relationship called 'longitudinality' between the population and general practitioners is crucial for the family physician program. The constant change of family physicians (FPs) can deteriorate longitudinality. This study aims to reveal what factors people usually consider when choosing a new FP or changing their current FP. METHOD: A qualitative study with a thematic analysis approach was carried out in Ilam province, Iran, in 2019. Purposeful sampling with a maximum variation strategy was followed to select the key informants. We did 34 interviews with following groups: patients (rural residents); FPs; and experts from Iran Health Insurance Organization, Ilam University of Medical Sciences, and Health Network Development Center. Data were analyzed using a thematic analysis to identify and contextualize the preferences of people in choosing a FP in rural areas. All the processes related to data coding and emerging themes were carried out using MAXQDA 2012 software. RESULTS: The content of the interviews was categorized into 2 main themes, 6 sub-themes, and 39 codes. The first theme was 'family physician characteristics' including four sub-themes: general behaviors, social and physical characteristics, professional expertise, and pharmaceutical prescriptions. The second theme was 'health center' consisting of two sub-themes including location and physical features and properties of the health center. CONCLUSION: Some of the factors extracted from the interviews may have a different effect on the choice of people with different demographics. For instance, patients may have different ideas about the age, gender, years of medical practice, and finally, language and origin of the birthplace of FPs. Quantitative studies are needed to rank the factors identified in this study according to their significance for choosing FP and reveal patients' preferences for each factor.

BACKGROUND: Job satisfaction refers to a person's attitude toward his/her job and its various aspects. Job satisfaction improves the quality of service and employees' physical and mental health. The present study aimed to design a valid and reliable instrument to assess Iranian midwives job satisfaction instrument (MJSI). METHODS: This is a sequential exploratory study for tool design. This study in two phases; (qualitative and tool's psychometric evaluation) was conducted in Ilam, Iran, 2019 years. In the first phase, a qualitative content analysis was carried out by in-depth and semi-structured individual interviews with 10 experts. Then, the pool of items extracted from the qualitative phase was completed by reviewing the existing texts and tools. The second phase of the study involved reducing the overlapping items and validating the tool. In order to investigate the construct validity, a cross sectional study was conducted with the participation of 121 midwives with census sampling. Data analysis was performed by SPSS-19 software using exploratory factor analysis and reliability tests (Cronbach's alpha). RESULTS: In the qualitative phase and after reviewing the existing texts and tools by the research team, a 58-item questionnaire was developed and then entered into the psychometric phase. Then, the tool was finalized with five factors, including: 1) communication features, 2) professional features, 3) responsibility aspects, 4) physical-mental aspects and 5) social aspects, respectively. After the psychometric process, by removing the items in different stages, a specific questionnaire was developed to measure the midwives' job satisfaction with 25 items which explained a total of 49.95% of the total variance. Reliability of the tool was approved by Cronbach's alpha = 0.71 and test-retest with 2-weeks intervals, indicating an appropriate stability for the scale (ICC = 0.898). CONCLUSION: The 25-item self-reporting midwives job satisfaction tool had acceptable validity and reliability. We recommend the use of this tool for evaluating the job satisfaction of midwives, as well as management and research purposes.
