Background: Previous studies revealed that maternal smoking exposure during pregnancy was an essential risk factor for offspring developing attention-deficit/hyperactivity disorder (ADHD). The impact of paternal smoking exposure 1 year before pregnancy on offspring ADHD risk is still unclear. Methods: The present study included 2,477 school-age children and their parents from the Shanghai Child and Adolescent Health Cohort who had complete data for offspring ADHD diagnosis and parents' smoking exposure before and during pregnancy information. A multivariate logistic regression model and Firth's logistic regression model were used to determine the associations of paternal smoking and parental smoke exposure patterns before and during pregnancy with offspring ADHD risk. Results: Children whose fathers smoked before pregnancy had a higher risk of developing ADHD [odds ratio (OR) = 2.59, 95% confidence interval (CI): 1.35-4.98] compared to those whose fathers had never been exposed to smoking. Similarly, parents who were exposed to smoking or second-hand smoke before pregnancy had 1.96 times (OR = 1.96, 95% CI: 1.19-3.22) more likely to have offspring with ADHD. Moreover, children whose parents were exposed to smoking both before and during pregnancy were 2.01 times (OR = 2.01, 95% CI: 1.29-3.12) more likely to develop ADHD. Conclusion: Paternal smoking before pregnancy and parental smoking exposure 1 year ahead of and throughout pregnancy were all risk factors for offspring developing ADHD.

Air pollution is a major public health challenge in the highly urbanized megacities of China. However, knowledge on exposure to ambient unregulated air pollutants such as black carbon (BC) and ultrafine particles (UFP) among the Chinese population, especially among urban high school students who may have highly variable time-activity patterns, is scarce. To address this, the personal exposures to BC and UFP of high school students (aged 17 to 18) in Chengdu, China were measured at 1-min intervals via portable samplers. Monitoring lasted for 2 consecutive 24-h periods with days classified as "school days" or "non-school days". Time-activity diaries and measurements were combined to explore spatial, temporal, and behavioral factors that contribute to different exposure profiles. The overall geometric means of BC and UFP were 3.60 µg/m3 and 1.83 × 104p/cm3, respectively with notable spatiotemporal variation in exposures observed. In general, the household and transport microenvironments were the predominant contributors to total BC (74.5%) and UFP (36.5%) exposure. However, the outdoor public microenvironment was found to have significantly higher overall average levels of BC than the household and transport microenvironments (p < 0.001) while also presenting the greatest exposure dose intensity (EDI - a measure of exposure in a microenvironment in proportion to time spent in that environment) of 4.79. The largest overall average level of UFP occurred in the indoor public microenvironment followed by transport. The outdoor public microenvironment also presented the greatest EDI of UFP (4.17). This study shows notable spatiotemporal variety in exposure patterns and will inform future exposure and population health studies. The high EDI outdoors may mean that health positive activities, such as exercise, may be being undermined by ambient pollution.

Objective@#To obtain the dietary exposure of lead in primary, middle and high school students, and to provide basic foundation for food safety risk assessment and management for children and adolecsents.@*Methods@#Weighing method, 3-days hour dietary survey, combining with the food frequency questionnaire, was applied to obtain basic dietary data in 616 primary, middle and high school students selected through multi-stage randdom sampling method from Pudong new area of shanghai. Data on lead concentrations were derived from the food safety risk monitoring system, which included 1 145 samples.@*Results@#Lead was detected in 568 of 1 145 samples (49.61%) from 10 categories of foods. The exceeding standard rate was 3.58% (41 samples exceeded). The average content of lead was (0.12±0.25) mg/kg, fungi and algae, beans and the aquatic products were found to be the primary food sources of lead exposure. The average dietary lead exposure in primary, middle and high school students was 9.94 μg/kg per week, accounted for 39.76% of PTWI. The lead exposure level at the 97.5th percentile which accounted for 118.24% of PTWI was 29.56 μg/kg. Vegetables, beans and cereal foods were the mainly source of dietary lead exposure, with average dietary lead exposure 2.57, 2.44 and 1.43 μg/kg peer week, accounted for 10.26%, 9.76% and 5.74% of PTWI, respectively.@*Conclusion@#Lead is present in some foods available in local markets in Pudong New Area, Shanghai. Low level of dietary exposure to lead is found in primary, middle and high school students, however, it depends on dietary pattern. Continued efforts are needed to reduce the dietary exposure of lead in school students.

BACKGROUND: Few studies have applied the Chinese Diet Balance Index (DBI) in evaluating dietary quality across seasons. METHOD: The Shanghai Diet and Health Survey (SDHS) included 1680 participants from all districts of Shanghai from 2012 to 2013. Dietary data were obtained using three-day 24-h recall in spring, summer, fall, and winter. Higher bound score (HBS), lower bound score (LBS) and diet quality distance (DQD) were calculated according to compliance with the dietary guidelines and based on the recommendations for consumption within the main food groups. HBS, LBS, and DQD represent over-intake, under-intake, and overall imbalance of the diet, respectively. RESULTS: 836 males and 844 females were included. The HBS indicated that 10.08%, 11.84%, 10.31%, and 12.73% people have moderate or high levels of over-intake of food in spring, summer, fall, and winter, respectively; and 74.04%, 37.61%, 53.09%, and 42.72% people have moderate or high levels of deficit food intake for each of the four seasons. The mean HBS and LBS among the four seasons were statistically significant difference (p < 0.001). The mean (SD) DQD was 43.27 (10.21), 35.67 (9.71), 39.19 (9.36), and 36.84 (9.45) in each season. A multivariable model showed statistically significant differences in DQD according to age, gender, occupational status, education, smoking, drinking status, season, and residency (p < 0.001). CONCLUSION: An unbalanced diet is common among people living in Shanghai. Seasonality and area of residence were found to be two significant predictors. Strengthening the accessibility and the supply of food across seasons and regions should be considered.
