Although an MRI scanner is a single stand-alone modality, different acquisition techniques may be applied to collect structural and functional information, including vascular or angiogenic properties measured by dynamic contrast enhanced MRI (DCE-MRI) and Choline (Cho) metabolism measured by proton MR spectroscopy imaging (MRSI). They may provide complementary information for a better characterization of neoplasm. In this study, we investigated the correlation between Choline measured by MRSI and vascular parameters measured by DCE-MRI in breast cancer. Fourteen patients with histologically proven invasive breast cancer were included. MRSI from a grid of 8 x 8 voxels within a selected slab from each lesion was performed. Each voxel was 1.0 x 1.0 x 1.2 cm3. Choline signal-to-noise ratio (SNR) was measured from each voxel showing an identifiable Choline peak. Corresponding DCE kinetics was measured from each voxel, and analyzed with a 2-compartmental model to obtain pharmacokinetic parameters Ktrans and k(ep). All parameters showed a wide variation within each lesion, and there were no consistent correlations between regional Cho and DCE parameters within the lesion of each individual patient. This finding might be attributed to the heterogeneous nature of breast cancer. The characteristic Cho and DCE-MRI parameters were obtained for each patient by averaging over all Cho-positive voxels. In these 14 patients there was a significant linear correlation between Cho with percent enhancement at 2 min after injection, SE%-2min (r = 0.75, p = 0.002), and pharmacokinetic parameters Ktrans (r = 0.74, p = 0.003), and k(ep) (r = 0.76, p = 0.002). The results suggested that overall there is a correlation between Choline metabolism and angiogenesis activity. Since Choline is associated with cell replication and angiogenesis is required to support tumor growth, this might explain the correlation between these two sets of measures among different lesions.

The quantification of choline-containing compounds (Cho) in breast tumors by proton MR spectroscopy (1H-MRS) has been of great interest because such compounds have been linked to malignancy. In this study, an internal reference method for the absolute quantification of Cho metabolite in malignant breast tumors was presented using a clinical 1.5 T scanner. We performed in vitro measurements to examine the accuracy of absolute quantification using four phantoms of known choline chloride concentrations. There was a high correlation between the calculated concentrations by MRS and the known concentrations (r2 > 0.98). We applied the technique to in vivo breast study conducted on 45 patients with biopsy-confirmed breast cancer. After T1 and T2 relaxation times were corrected, the Cho levels in this work had a range of 0.76-21.20 mmol/kg from 34 MR spectra of 32 patients with malignant breast lesions. This result was rather consistent with the previously published value (i.e., 1.38-10 mmol/kg, Bolan et al. in Magn Reson Med 50:1134-1143, 2003). Therefore, we conclude that the internal method using the fully relaxed water as a reference could be used for quantifying Cho metabolite accurately in breast cancer patients using a clinical 1.5 T scanner.

This study was conducted to investigate the metabolic changes in the motor and motor association cortices following axonal injury in the internal capsule that was caused by deep intracerebral hematoma. Using proton magnetic resonance spectroscopy (1H MRS), the authors studied the primary motor cortices (M-1) and supplementary motor areas (SMA) of 9 hemiparetic patients with documentable hemiparesis of varying severity, and we studied 10 normal volunteers as controls. To measure the M-1 and SMA biochemical changes, 4 separate single volumes of interest (VOIs) were located bilaterally in the affected and unaffected hemisphere (AH and UH). 1H MRS provided a neuronal and axonal viability index by measuring levels of N-acetylaspartate (NAA) and creatine/phosphocreatine (Cr). The M-1/SMA NAA/Cr ratios of the AH and UH in patients, and the AH and normal volunteers were compared. The NAA/Cr ratios of the M-1 and SMA in AH, and the SMA in UH were significantly lower than those of normal volunteers. These 1H MRS findings indicate that axonal injury in the descending motor pathway at the level of internal capsule could induce metabolic changes in the higher centers of the motor pathway.

To investigate whether there are significant changes in regional brain metabolism in patients with Parkinson's disease before and after thalamotomy using proton magnetic resonance spectroscopy (1H MRS). Fifteen patients underwent 15 stereotactic thalamotomies for control of medically refractory parkinsonian tremor. Single-voxel 1H MRS was carried out on a 1.5 T unit using stimulated-echo acquisition mode (STEAM) sequence (TR/TM/TE, 2000/14/20 ms). Spectra were obtained from substantia nigra, thalamus and putamen areas with volumes of interests (7-8 ml) in patients before and after the surgery. Metabolite ratios of NAA/Cho, NAA/Cr and Cho/Cr were calculated from relative peak area measurements. We evaluated alterations of metabolite ratios in brain metabolism in Parkinson's disease patients with clinical outcome following thalamotomy. NAA/Cho ratios showed generally low levels in substantia nigra and thalamus in Parkinson's disease patients with clinical improvement following thalamotomy. In 80% (12/15) patients, decreased NAA/Cho ratios were observed from the selected voxels in substantia nigra after thalamic surgery (P<0.05). The ratios were also significantly decreased in thalamus in 67% (10/15) patients with clinical improvement (P<0.05). Our results suggest that NAA/Cho ratio may be a valuable criterion for evaluation of Parkinson's disease patients with the clinical improvement following surgery. 1H MRS may be a useful utility for the aid in better understanding the pathophysiologic process in Parkinson's disease patients on the basis of the variation of NAA/Cho ratio.

To prove feasibility of proton chemical shift imaging ((1)H CSI) during stereotactic procedure, authors performed (1)H CSI in combination with a stereotactic headframe and selected targets according to local metabolic information, evaluated the pathologic results. The (1)H CSI directed stereotactic biopsy was performed in four patients. (1)H CSI and conventional Gd-enhancement stereotactic MRI were performed simultaneously after the fitting of a stereotactic headframe. Focal areas of increased phosphocholine(Cho)/phosphocreatine(Cr) and Lactate/Cr ratios were selected as target sites in the stereotactic MR images. (1)H CSI is possible with the stereotactic headframe in place. Pathologic samples taken from areas of increased Cho/Cr ratios and decreased NAA/Cr ratios provided information upon increased cellularity, mitoses and cellular atypism, and facilitated diagnosis. Pathologic samples taken from areas of increased Lac/Cr ratio showed predominant feature of necrosis. (1)H CSI was feasible with the stereotactic headframe in place. The final pathologic results obtained were concordant with the local metabolic information from (1)H CSI. We believe that (1)H CSI-directed stereotactic biopsy has the potential to significantly improve the accuracy of stereotactic biopsy targeting.

OBJECTIVE: To determine, using proton magnetic resonance spectroscopy (1H MRS) whether thalamotomy in patients with Parkinson's disease gives rise to significant changes in regional brain metabolism. MATERIALS AND METHODS: Fifteen patients each underwent stereotactic thalamotomy for the control of medically refractory parkinsonian tremor. Single-voxel 1H MRS was performed on a 1.5T unit using a STEAM sequence (TR/TM/TE, 2000/14/20 msec), and spectra were obtained from substantia nigra, thalamus and putamen areas, with volumes of interest of 7-8 ml, before and after thalamotomy. NAA/Cho, NAA/Cr and Cho/Cr metabolite ratios were calculated from relative peak area measurements, and any changes were recorded and assessed. RESULTS: In the substantia nigra and thalamus, NAA/Cho ratios were generally low. In the substantia nigra of 80% of patients (12/15) who showed clinical improvement, decreased NAA/Cho ratios were observed in selected voxels after thalamic surgery (p < 0.05). In the thalamus of 67% of such patients (10/15), significant decreases were also noted (p < 0.05). CONCLUSION: Our results suggest that the NAA/Cho ratio may be a valuable criterion for the evaluation of Parkinson's disease patients who show clinical improvement following surgery. By highlighting variations in this ratio, 1H MRS may help lead to a better understanding of the pathophysiologic processes occurring in those with Parkinson's disease.
