Actinic granuloma of O'Brien and giant molluscum contagiosum in immunocompetent adults are rare skin disorders. There have been no previous reports of these two conditions occurring together. We report this occurrence in a 57-year-old Caucasian woman.

A 60-year-old man presented with hypertension, proteinuria and renal failure. He had an IgM lambda plasma and urinary paraprotein, but bone-marrow examination and skeletal survey were normal, and renal biopsy was felt to be consistent with chronic pyelonephritis. Renal failure progressed, and haemodialysis and later cadaveric renal transplantation were performed. He died 7 months after transplantation from pulmonary thromboembolism. Postmortem examination and review with immunohistochemical techniques of his original renal biopsy revealed his renal disease to have been macroglobulinaemic nephropathy which had recurred in his renal allograft.

Oncocytomas of the kidney are infrequently reported tumours. They have distinct clinical and pathological features, but most cases have been discovered retrospectively. Nine series have been analysed (including four cases in Norwich) to define the features of these tumours. One of our cases is noteworthy because it is the largest individually reported tumour of this type and it contained a large cyst. Oncocytomas are benign and have an excellent prognosis. Failure to identify this subgroup may bias the perceived effectiveness of treatment for renal carcinoma.
