RESUMO

To explore the possibility to use the lens extract as an in vitro stimulator to conduct a test of stimulated leukergy in liquidators of the accident consequences (LAC) on Chernobyl Nuclear Power Plant (CNPP) with a cataract in the long-term period. The study sample included 72 men-LAC on CNPP, at the age from 42 to 65 y, who have a cataract. The comparison group consisted of 60 men, with a cataract, of the same age, and who were not exposed to radiation. The control group was composed of 60 men, at the age of 42-58 y without lens pathology. Phenomenon of the stimulated leukergy was revealed in persons who had been exposed to radiation in the dose of 18.2 ± 0.58 cGy and was observed in 5.7-8.05 % (P < 0.001), suggesting a continued high auto-aggression to the lens antigens, and the strength of cell-mediated immunity.

RESUMO

The retrospective research analyzed the data of 893 patients undergoing total hip arthroplasty. There were used the data about patients undergoing hip arthroplasty without complications (the first group--808 cases) and patients with developed deep surgical site infection following total joint arthroplasty after 12 months (the second group--85 patients). It was applied as the training matrix in creation of mathematical prognosis and algorithm of prophylaxis of deep infection in patients undergoing the primary total hip arthroplasty. There were revealed 21 prognostic significant criteria of deep infection development in surgical site. The program was tested in prospective investigation (293 clinical cases) with follow-up term of 12 months after operation. The rate of development of postoperative deep infection in surgical wound reduced as compared with the rate in group of retrospective research from 1.7% to 0.7%. The efficacy of proposed program was 80%.

On the basis of the data of a retrospective investigation of 541 patients with nonrecurrent noncomplicated postoperative ventral hernia the criteria of prognosis of pyo-inflammatory complications in the operative wound after liquidation of the abdominal wall defect became more exact and quantitated. It was established that 18 criteria can be used for prognosis, 14 of them--before operation. During the operative procedure it can be possible to make the prognosis more exact with the intraoperative findings taken into account. Among them of special significance are 4 prognostic criteria: multichamber hernial sac, commissural process in the abdominal cavity, the fact of separation of the commissures and possible perforation of the hollow organ. The efficiency of the program was 80%. The approbation of the program has shown that prognosis of the development of pyo-inflammatory complications must be made on the basis of both the preoperative and intraoperative criteria. It allows to reliably reduce the amount of postoperative pyo-inflammatory complications from 8.6% (retrospective data) to 5.2% (prospective data).
