RESUMO

The authors report a case of Creutzfeldt-Jakob disease in a man aged 60 years from a family without a history of similar disease. The disease extended over 11 months. In the clinical picture initially equilibrium disturbances and dementia with psychotic symptoms predominated, EEG pattern was not typical of CJD. Neuropathological examination revealed extensive spongiform lesions in the cortex of all cerebral lobes, in striatum and substantia nigra, moreover a considerable number of kuru plaques was found in cerebellar cortex. The authors consider that the case meets the criteria accepted for the sporadic form of CJD but believe that the final differentiation from the Gerstmann-Streussler-Scheinker syndrome should be based on genetic studies.

RESUMO

We have analysed the correlation between the direction of needle bevel insertion and the occurrence of post-puncture syndrome appearing after diagnostic lumbar puncture. Post-puncture headache was observed in 38 of 380 patients (13.6%). The syndrome occurred in 11 patients (7.9%) in whom the needle bevel had been inserted parallel to the patient's backbone and in 27 patients (19.3%) in whom the needle bevel had been inserted perpendicularly to the backbone. The complications after lumbar puncture could be reduced by inserting the needle bevel parallel to the patient's backbone.

RESUMO

We present three patients with unilateral upper limb weakness (with muscular atrophy)-two of them with distal and one with proximal localization. The disease onset was between 18th end 35-th year of life; the disease course was biphasic (i.e. progressive within first 1 to 3 years, and stabilized during following 4-24 years). The laboratory investigations permitted to diagnose juvenile monomelic amyotrophy, an entity that is very rare outside Japan. Electromyography revealed neurogenic involvement with spinal features also in clinically unaffected muscles. We suggest that these results may support the hypothesis of this disease being a benign variant of spinal muscular atrophy.

RESUMO

F wave parameters are connected with spinal motoneuron excitability. In view of this, the authors performed the study of the evolution of that response in 30 patients after cerebral stroke in three subsequent periods: I--1st or 2nd day, II--14th or 15th day, III--5 to 6 weeks after stroke. We found F wave occurrence to be reduced in the first period after stroke; subsequently its parameters increased greatly. Changes of F wave parameters could be observed earlier than changes of clinical status, especially of muscle tonus.

RESUMO

Two cases of antidiabetic drug poisoning are presented. They resulted from a suicidal attempt. Transient hypoglycaemic coma, abnormalities of the cognition functions, pyramidal and extrapyramidal symptoms were due to a prolonged hypoglycaemia.

RESUMO

Retrospective analysis of patients with ischaemic cerebral stroke with particular relation to an effect of alcohol has been made. A group of 277 male patients aged upto 60 years was distinguished. Out of these patients alcohol abuse in anamnesis concerned 128 patients, and ischaemic cerebral stroke occurred in 16 of them during or immediately after drinking. An analysis included neurological status of the patients and the status of vascular system in correlation with changes in cerebral vessels angiography.
