Resumen Introducción: Ser cuidador de personas con dependencia funcional conlleva el riesgo de presentar deterioro de la calidad de vida, esto puede deberse a que la prolongación del cuidado altera la dinámica familiar y genera sobrecarga en el cuidador. Objetivo: 1) Identificar la relación entre funcionamiento familiar y calidad de vida del cuidador del adulto mayor y 2) Determinar la sobrecarga y calidad de vida del cuidador del adulto mayor. Metodología: Estudio descriptivo y correlacional, muestreo por bola de nieve, la muestra conformada por 86 cuidadores de un centro comunitario. Para las mediciones se utilizó el índice de Barthel, el Test de APGAR Familiar, la prueba de Zarit y la Escala de Calidad de Vida WHOQOL-BREF. Se aplicó la Prueba de Kolmogorov Smirnov y Prueba de Correlación de Spearman. Resultados: El 11.6% de los adultos mayores presentó dependencia total, la percepción de disfuncionalidad familiar grave representó el 3.5%. Los cuidadores con sobrecarga intensa fueron el 32.6%, el 8.1% percibió la calidad de vida deficiente. A mayor funcionamiento familiar mayor calidad de vida del cuidador (p<.05) y a mayor sobrecarga, menor calidad de vida del cuidador (p<.05) en todas las dimensiones. Conclusiones: Se observó que la sobrecarga y funcionalidad familiar están relacionadas con la calidad de vida del cuidador, por lo que se convierten en un escenario disciplinar que plantea la posibilidad de diseñar estrategias de intervención, centradas en la sobrecarga y el funcionamiento de la familia para el mejoramiento de la calidad de vida del cuidador.


Abstract Introduction: The prolonged provision of healthcare to functionally dependent persons can result in the deterioration on the caregiver's quality in terms of family dynamics and overload. Objectives: 1) To identify the relationship between the family dynamics and the quality of life of the caregiver of older adults; and 2) to estimate the quality of life and the overload of the caregiver of older adults. Methodology: This is a descriptive and correlational study with a snowball-derived sample of 86 caregivers in a communitarian center. The corresponding measures were estimated using the Barthel Index, the Family APGAR Test, the Zarit Test, and the WHOQOL-BREF Quality of Life Scale. The Kolmogorov-Smirnov and Spearman Correlation tests were calculated. Results: 11.6% of the older adults being cared showed total dependency. 3.5% of the caregiver's sample perceived having a severe family dysfunction. 32.6% of the sample were found having heavy overloads. 8.1% of the sample perceived having poor quality of life. In all dimensions, the better the family dynamics were, the better the quality of life of the caregivers was (p<.05), and the heavier the overload was, the worse the quality of life of the provider was (p<.05). Conclusions: It was noted that both, overload and family dynamics, are related to the quality of life of the caregivers of older adults; therefore, the design of intervention strategies should be focused on the overload and family dynamics of these professionals in order to improve their quality of life.


Resumo Introdução: Ser cuidador de pessoas com dependência funcional implica o risco de apresentar deterioro da qualidade de vida, isto pode dever-se a que o prolongamento do cuidado altera a dinâmica familiar e gera sobrecarga no cuidador. Objetivo: 1) Identificar a relação entre funcionamento familiar e qualidade de vida do cuidador do idoso e 2) Determinar a sobrecarga e qualidade de vida no cuidador do idoso. Metodologia: Estudo descritivo e correlacional, amostragem por bola de neve, a amostragem conformada por 86 cuidadores de um centro comunitário. Para as medições utilizou-se o índice de Barthel, o Teste de APGAR Familiar, a prova de Zarit e a Escala de Qualidade de Vida WHOQOL-BREF. Aplicou-se a prova de Kolmogorov Smirnov e Prova de Correlação de Spearman. Resultados: O 11.6% dos idosos apresentou dependência total, a percepção de disfuncionalidade familiar grave representou o 3.5%. Os cuidadores com sobrecarga intensa foram o 32.6%, o 8.1% percebeu a qualidade de vida deficiente. Quanto maior funcionamento familiar, maior qualidade de vida do cuidador (p<.05) e quanto maior sobrecarga, menor qualidade de vida do cuidador (p<.05) em todas as dimensões. Conclusões: Observou-se que a sobrecarga e funcionalidade familiar estão relacionadas com a qualidade de vida do cuidador, pelo que se tornam em um cenário disciplinar que envolve a possibilidade de desenhar estratégias de intervenção, centradas na sobrecarga e o funcionamento da família para o melhoramento da qualidade de vida do cuidador.

Resumen Introducción: En México, cuando se inició la aplicación de la vacuna PCV13 (neumocócica conjugada), se cubría el 70.6% de los serotipos causantes de enfermedad invasiva por neumococo en menores de 5 años. Después de varios años, los casos de enfermedad causada por los serotipos incluidos en la vacuna han disminuido; sin embargo, se ha producido un reemplazo por los serotipos no incluidos en la vacuna. Caso clínico: Se presentan tres casos de pacientes pediátricos que desarrollaron enfermedad invasiva por serotipos no incluidos en la PCV13: uno con meningitis y bacteriemia (serotipo 15C) y dos con neumonía, uno de ellos complicado con derrame (serotipo 35B). Los pacientes fueron atendidos en un hospital pediátrico en Saltillo, Coahuila, durante el periodo de 2015 a 2018. Conclusiones: Resulta alarmante que se presenten tres casos graves por serotipos de Streptococcus pneumoniae no incluidos en la PCV13 en un solo hospital pediátrico en el norte del país. Este es un fenómeno que esta sucediendo a escala nacional e internacional: un incremento de casos de enfermedad invasiva por serotipos de neumococo no incluidos en la vacuna utilizada actualmente.

Resumen Introducción La alteración visual puede producir cambios en la marcha y afectar la autonomía del adulto mayor, entendida como la necesidad de ayuda de otras personas o aditamentos para realizar, adecuadamente, sus actividades de la vida diaria. Objetivo Conocer la relación entre la capacidad de marcha y dependencia funcional en adultos mayores con alteración de la agudeza visual. Material y métodos Estudio descriptivo y correlacional, en una población de adultos mayores con alteración de la agudeza visual, muestra conformada por 94 adultos mayores. La marcha se midió con el sistema GAITRite, para la dependencia en Actividades Básicas de la Vida Diaria, se utilizó el Índice de Barthel, para las Actividades Instrumentales de la Vida Diaria el índice de Lawton y Brody. Resultados La capacidad de marcha y dependencia funcional, presentó relación positiva entre la velocidad de marcha (p = 0.000) y longitud del paso (p = 0.000) con las ABVD; la velocidad de marcha (p = 0.000), cadencia (p = 0.023) y longitud del paso (p = 0.000) con las AIVD y relación negativa entre la amplitud del paso (p = 0.012) y AIVD. Conclusión La valoración de la marcha en los AM con alteración en la agudeza visual, desde el primer nivel de atención, permitirá desarrollar intervenciones y programas encaminados a preservar la independencia del adulto mayor, reducir las tasas de morbilidad, disminuir costos en salud y contribuir en una mejor calidad de vida.


Abstract Introduction Disturbances in the visual function of the elderly can lead to gait changes which can have an impact on their autonomy, leading to the need of appliances and help of other persons in order for them to adequately perform their daily activities. Objective To gain understanding on the relationship between gait capacity and functional dependence among older adults with vision disturbances. Materials and Methods This is a descriptive and correlational study on a sample of 94 older adults with vision disturbances. The gait was measured using the GAITRite System. The functional status in terms of Activities of Daily Living (ADL), and Instrumental Activities of Daily Living (IADL) were estimated using the Barthel, and the Lawton & Brody scales respectively. Results The gait capacity and functional dependence showed a positive relationship in terms of velocity (p = 0.000) and steps (p = 0.000) regarding the ADL; and in terms of velocity (p = 0.000), cadence (p = 0.023), and steps (p = 0.000) regarding the IADL. A negative relationship between step amplitude (p = 0.012) and IADL was also found. Conclusion From the primary healthcare point of view, accurate gait assessments on the older adults with vision disturbances will allow the development of programs and interventions aimed at preserving the independence, reducing the morbidity rates and costs, and contributing to maintain a better quality of life among older adults.


Resumo Introdução A alteração visual pode produzir câmbios na marcha e afetar a autonomia do idoso, entendida como a necessidade de ajuda de outras pessoas ou aditamentos para realizar, adequadamente, suas atividades da vida diária. Objetivo Conhecer a relação entre a capacidade de marcha e dependência funcional em idosos com alteração da agudeza visual. Material e métodos Estudo descritivo e correlacional, em uma população de idosos com alteração da agudeza visual, amostra conformada por 94 idosos. A marcha mediu-se com o sistema GAITRite, para dependência em Atividades Básicas da Vida Diária, utilizou-se o Índice de Barthel, para as Atividades Instrumentais da Vida Diária o índice de Lawton e Brody. Resultados A capacidade de marcha e dependência funcional, apresentou relação positiva entre a velocidade de marcha (p = 0.000) e longitude de passo (p = 0.000) com as ABVD; a velocidade de marcha (p = 0.000), cadência (p = 0.023) e longitude do passo (p = 0.000) com as AIVD e relação negativa entre a amplitude do passo (p = 0.012) e AIVD. Background: The prevalence of food allergy is on the rise on a global scale. Objective: To determine the prevalence of food hypersensitivity (FHS) and probable food allergy (PFA), as well as the foods and factors associated with these occurrences. Methods: A cross-sectional study was carried out among 1992 adolescents (aged 15-18 years). Each adolescent answered a structured questionnaire. A multivariate analysis was used to identify the association between the variables. Results: The prevalence of FHS was 10.6% (the most commonly associated foods were shrimp, cow's milk and avocado) and the PFA was 7.8% (shrimp, cow's milk and pecan). The prevalences of oral allergy syndrome, food-associated urticaria and systemic reaction were 4.9%, 3.6% and 1.5%, respectively. The following factors were associated with FHS: personal history of asthma (OR 1.63; 95% CI: 1.11-2.41), allergic rhinitis (OR 2.60; 95% CI: 1.75-3.87), atopic dermatitis (OR 2.07; 95% CI: 1.25-3.43), maternal history of asthma (OR 1.80; 95% CI: 1.02-3.16), atopic dermatitis (OR 6.11; 95% CI: 2.45-15.29), and female sex (OR 1.89; 95% CI: 1.38-2.59). PFA was associated with a personal history of asthma (OR 1.65; 95% CI: 1.06-2.56), allergic rhinitis (OR 2.46; 95% CI: 1.56-3.88), atopic dermatitis (OR 2.02; 95% CI: 1.15-3.54), paternal allergic rhinitis (OR 2.52; 95% CI: 1.15-5.51), maternal atopic dermatitis (OR 7.46; 95% CI: 2.93-19.00), and female sex (OR 1.89; 95% CI: 1.31-2.72). Conclusion: The adverse reactions associated with foods among late adolescents are a frequent occurrence, and the most commonly associated factor is atopy

Summary: Background. Oral allergy syndrome (OAS) is the most common food allergy manifestation amongst adults. However, population studies aimed at estimating its prevalence and associated factors are scarce in Mexi-co. Objectives. To establish the prevalence of OAS in a sample of university students and to describe their clinical characteristics and its associated factors. Methods. From a sample group made up of 25,269 university students, the data corresponding to 1,200 students aged 18 to 25 was analyzed with a cross-sectional approach. A structured questionnaire was used to identify OAS, its symptoms and related foods, and the personal history of atopic diseases. The associations between variables were calculated through logistical regression analysis. Results. The prevalence of OAS was 3.4%, with a 95% confidence interval (95% CI) of 2.5 to 4.6. The main oral symptoms reported were lip pruritus, edema and the sensation of pharyngeal oppression. Among the extra-oral complaints were: reddish coloration of the skin, body pruritus, abdominal pain, and abdominal bloating. The foods that were most frequently associated with OAS were fruits (68.5%), vegetables (22.0%) and seafood (19.3%). Through multivariate analyses, allergy to pollen and latex were found to be associated with OAS, OR 3.29; 95%: CI 1.53 to 7.10 and OR 5.53; 95% CI: 1.08 to 28.2, respectively. Conclusions. Notably, the prevalence of OAS varies according to the geographic area. Personal histories of allergy to pollen or latex were the main factors linked to OAS.

El neumomediastino espontáneo se define como la presencia de aire dentro del mediastino. Se origina generalmente por una fuga de aire por aumento de presión en el alveolo. La incidencia en la edad pediátrica se encuentra entre 1/8000 y 1/15 000, con dos picos de edad: menores de cuatro años y de entre 13 a 17 años. En el primer grupo suele asociarse a una infección del tracto respiratorio, una crisis asmática o por aspiración de cuerpo extraño, mientras que en el segundo suele originarse tras actividad física intensa. Se ha descrito la implicación de virus como influenza o bocavirus en la fisiopatología de esta entidad, pero hasta el momento muy pocos casos se han descrito en relación con el virus respiratorio sincitial. La clínica más frecuente es dolor torácico junto con disnea y enfisema subcutáneo como signo característico. El diagnóstico en casi todos los casos lo dará la radiografía de tórax. El manejo dependerá del grado de afectación y su repercusión, por lo que variará desde observación hasta ingreso en una Unidad de Cuidados Intensivos. El tratamiento será de soporte y el de las complicaciones asociadas, no se suelen dar recurrencias y el pronóstico suele ser bueno en la mayor parte de los casos


Spontaneous pneumomediastinum is defined as free air or gas contained within the mediastinum. It originates from the rupture of alveolar space because of high pressures. The incidence of spontaneous pneumomediastinum in pediatrics is between 1/8000 to 1/15 000 with two age peaks; under 4 years old and between 13 to 17 years old. First group is characterized by being secondarily accompanied by an infection of the respiratory tract, asthmatic exacerbation or foreign body aspiration while the second one usually originates after intense physical activity. Influenza or bocavirus have been related with the pathology of this entity, but few cases have been written in relation to respiratory syncytial virus. The most common symptoms are chest pain and dyspnea with subcutaneous emphysema as a characteristic sign. Diagnosis in almost all cases will be given by a chest X-ray. Management will depend on the degree of involvement and its repercussion, which will spread from observation to admission to the ICU. Conservative therapy and treatment of associated complications are usually sufficient, there are few recurrences and the prognosis is good in most of cases

BACKGROUND: Both breastfeeding and the moment at which introduction to solid food occurs have been associated with food allergy. OBJECTIVE: To evaluate whether prolonged breastfeeding and the delayed introduction of whole cow's milk into an infant's diet are factors that can be associated with egg sensitization. METHODS: This was a hospital-based case-control study, matched by age and sex: each study group comprised 97 atopic children. Additionally, logistic regression was used to identify the factors associated with egg protein sensitization. RESULTS: The most common type of allergic disease among both groups was allergic rhinitis. After adjusting for possible confounding variables, a delayed introduction to whole cow's milk decreased the odds of egg protein sensitization; OR = 0.16 (95% CI: 0.07-0.36, p < 0.0001). Notably, breastfeeding during the first six months of life, regardless of whether it was the only milk an infant drank, increased the risk for sensitization to chicken eggs; OR = 5.54 (95% CI: 2.41-12.7, p < 0.0001). CONCLUSION: Prolonged breastfeeding, regardless of whether it was the only milk an infant drank, greatly increased the risk of egg sensitization. Interestingly, a delayed introduction to whole cow's milk was associated with a reduced possibility of becoming sensitized to eggs. Further studies are required to elucidate these findings

INTRODUCTION: The association regarding the exposure to pets, especially cats and dogs, and the prevalence of allergic diseases is inconsistent. OBJECTIVE: We analyzed the role played by early exposure to dogs or cats in the prevalence of allergic diseases amongst school-aged children. METHOD: Through a cross-sectional study, we examined 756 children, aged 6-7; these candidates were selected through cluster sampling. We inquired about the exposure that these children had had to dogs and cats, and whether these pets spent most of their time indoors or outdoors during the first year of the child's life. In order to identify the prevalence of allergic diseases and their symptoms, each child's parent completed the International Study of Asthma and Allergies in Childhood questionnaire. RESULTS: Exposure to outdoor dogs was associated to nocturnal coughing, odds ratio (OR) 0.64, with a confidence interval of 95% (95% CI) 0.43-0.95 and with atopic dermatitis (OR: 0.39; 95% CI: 0.20-0.76). Interestingly, exposure to outdoor cats was associated to nocturnal coughing (OR: 0.51; 95% CI: 0.32-0.83) and current rhinitis symptoms (OR: 0.59; 95% CI 0.36-0.97). After carrying out the multivariate analyses, only exposure to dogs, both indoor and outdoor, was significantly associated to a decrease in the prevalence of atopic dermatitis OR 0.40 (95% CI: 0.20-0.79) and OR 0.38 (95% CI: 0.18-0.83), respectively. CONCLUSION: Our findings suggest that exposure to dogs, whether they be indoor or outdoor pets, is associated to a decreased prevalence in atopic dermatitis

BACKGROUND: With the availability of high-quality asthma guidelines worldwide, one possible approach of developing a valid guideline, without re-working the evidence, already analysed by major guidelines, is the ADAPTE approach, as was used for the development of National Guidelines on asthma. METHODS: The guidelines development group (GDG) covered a broad range of experts from medical specialities, primary care physicians and methodologists. The core group of the GDG searched the literature for asthma guidelines 2005 onward, and analysed the 11 best guidelines with AGREE-II to select three mother guidelines. Key clinical questions were formulated covering each step of the asthma management. RESULTS: The selected mother guidelines are British Thoracic Society (BTS), GINA and GEMA 2015. Responses to the questions were formulated according to the evidence in the mother guidelines. Recommendations or suggestions were made for asthma treatment in Mexico by the core group, and adjusted during several rounds of a Delphi process, taking into account: 1. Evidence; 2. Safety; 3. Cost; 4. Patient preference - all these set against the background of the local reality. Here the detailed analysis of the evidence present in BTS/GINA/GEMA sections on prevention and diagnosis in paediatric asthma are presented for three age-groups: children with asthma ≤5 years, 6-11 years and ≥12 years. CONCLUSIONS: For the prevention and diagnosis sections, applying the AGREE-II method is useful to develop a scientifically-sustained document, adjusted to the local reality per country, as is the Mexican Guideline on Asthma

BACKGROUND: Both breastfeeding and the moment at which introduction to solid food occurs have been associated with food allergy. OBJECTIVE: To evaluate whether prolonged breastfeeding and the delayed introduction of whole cow's milk into an infant's diet are factors that can be associated with egg sensitization. METHODS: This was a hospital-based case-control study, matched by age and sex: each study group comprised 97 atopic children. Additionally, logistic regression was used to identify the factors associated with egg protein sensitization. RESULTS: The most common type of allergic disease among both groups was allergic rhinitis. After adjusting for possible confounding variables, a delayed introduction to whole cow's milk decreased the odds of egg protein sensitization; OR=0.16 (95% CI: 0.07-0.36, p<0.0001). Notably, breastfeeding during the first six months of life, regardless of whether it was the only milk an infant drank, increased the risk for sensitization to chicken eggs; OR=5.54 (95% CI: 2.41-12.7, p<0.0001). CONCLUSION: Prolonged breastfeeding, regardless of whether it was the only milk an infant drank, greatly increased the risk of egg sensitization. Interestingly, a delayed introduction to whole cow's milk was associated with a reduced possibility of becoming sensitized to eggs. Further studies are required to elucidate these findings.

We used logistic regression and multivariate analysis to determine the correlation between asthma, allergic diseases, and rural environment. Results: We included 189/1003 (18.8%) children from a rural environment, and 814/1003 (81.2%) from an urban area. BA and AR were associated to a family history of atopy (OR = 2.15, p = 0.001; OR = 2.58, p = 0.002, respectively). BA was more prevalent in males (OR = 1.92, p = 0.007). Notably, a higher number of siblings seems to protect against AR (OR = 0.45, p = 0.008). A paternal history of allergies was associated to AD. Conclusions: In our study, we were unable to find protective factors in a rural environment that might decrease the prevalence of asthma or allergic diseases (AU)

RESUMO

The overarching goals of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) are to enable European citizens to lead healthy, active and independent lives whilst ageing. The EIP on AHA includes 74 Reference Sites. The aim of this study was to transfer innovation from an app developed by the MACVIA-France EIP on AHA reference site (Allergy Diary) to other reference sites. The phenotypic characteristics of rhinitis and asthma multimorbidity in adults and the elderly will be compared using validated information and communication technology (ICT) tools (i.e. the Allergy Diary and CARAT: Control of Allergic Rhinitis and Asthma Test) in 22 Reference Sites or regions across Europe. This will improve the understanding, assessment of burden, diagnosis and management of rhinitis in the elderly by comparison with an adult population. Specific objectives will be: (i) to assess the percentage of adults and elderly who are able to use the Allergy Diary, (ii) to study the phenotypic characteristics and treatment over a 1-year period of rhinitis and asthma multimorbidity at baseline (cross-sectional study) and (iii) to follow-up using visual analogue scale (VAS). This part of the study may provide some insight into the differences between the elderly and adults in terms of response to treatment and practice. Finally (iv) work productivity will be examined in adults.

INTRODUCTION: The oral allergy syndrome (OAS) is a particular type of food allergy rarely explored in the paediatric population that is already considered an adult problem. OBJECTIVE: Identify the prevalence of OAS, symptoms and pollen species associated with its presence in children affected by allergic diseases. METHODS: A cross-sectional study was conducted. Consecutive sampling included children from 6 to 14 years who needed allergy treatment for the first time. A structured questionnaire was carried out to collect demographic and clinical data and history of OAS. Besides sensitisation to various allergens, the skin prick-by-prick test was performed to corroborate sensitisation to food related to OAS. Prevalence of OAS and its association with pollens was established following the covariate adjusted logistic regression. RESULTS: 267 subjects were included. Overall prevalence of OAS was 8.9% (95%CI 6.1-13.1%). Prevalence of OAS for allergic rhinitis and asthma were 8.8% and 9.1%, respectively. In patients sensitised to pollen, the prevalence ranged from 9.6% to 12.2% depending on the type of pollen. 62.5% of children with OAS were sensitive to pineapple. After adjusting for gender and family history of atopic disease, trees from the Quercus species showed an association with OAS (OR=2.7, 95%CI 1.2-6.2). CONCLUSIONS: OAS is not uncommon in our environment. Pineapple, a typical fruit from the region, was the main food related. Quercus sp., but not birch nor olive, was the pollen associated with this syndrome
