RESUMO
Scientific literature about the surgical tactics in traumatic instability sternocostal frame was conduct. Studies have shown that this type of trauma expediently are useful minimal invasive osteosynthesis. The original method of osteosynthesis ribs and sternum external fixation apparatus were proposed, its application promoted to significantly improved the results of treatment.

Basing on experience of treatment of 97 injured persons there was analyzed the course of cardiac and pericardial wounding, the conventional and modern diagnostic possibilities were compared, the staged approach, directed to the surgical treatment results improvement, was systematized.

Problems of diagnosis of the closed pancreatic injury (CPI) in combined trauma are discussed. The CPI peculiarity in closed combined trauma is the absence of pathognomonic symptoms. Rational application of instrumental diagnostic methods permits to diagnose CPI in combined trauma.

Results of medical care, given to injured persons with closed thoracoabdominal trauma on the prehospital stage were analyzed. Lacks and complications in tactic of treatment and diagnosis were determined. Mistakes of the medical ambulance care physicians were noted in 51.3% of observations.

Experience of surgical treatment of 1147 injured persons with severe closed thoracoabdominal trauma was summarized. Coexistence with the bones trauma was noted in 631 (55%) injured persons. The structure, peculiarities of surgical tactics, complications, causes of death for the patients with the bones fracture were suggested. Mortality had constituted 44.7%.

There were examined 103 patients, including 62 men and 41 women. Endoscopic method of treatment with removing of 32 things was applied in gastroduodenal localization of foreign body. Per vias naturalis 64 things had evacuated in application of conservative therapy. Planned operation was done in 2 patients in unefficacy of conservative treatment. All the patients are alive.

The experience of application of the blood reinfusion in complex of reanimational measures in treatment of 280 injured persons with thoracoabdominal damage in an acute period of trauma was summarized. Reinfusion was done in 42.8% of operated injured persons, promoting to preserve 243.1 l of preserved blood and simplifying rendering the assistance in an acute period of trauma.

The experience of estimation of the state and the results of treatment of 300 patients with the combined craniocerebral trauma (CCT) is summarized. The injured persons with severe CCT and severe outcranial injuries were included in the 1st group, with severe CCT and nonsevere outcranial injuries--in the 2nd group, with nonsevere CCT and severe outcranial injuries--in the 3d group, with nonsevere CCT and nonsevere outcranial injuries--in the 4th group. The dominating, competing and accompanying injuries, determining the surgical treatment tactics, were marked out in every group. The differentiative therapy general principles were marked out.
