RESUMO

The goal of this report is to introduce a technique of transcorneal activation of the site of previous hypotensive surgery, its complications and long-term results. The study enrolled 100 patients aged from 39 to 84 who underwent a transcorneal activation procedure due to decompensation of intraocular pressure 2 weeks - 20 years after the previous hypotensive surgery. Group I included 40 patients after a nonpenetrating deep sclerectomy; group II consisted of 60 patients after a penetrating hypotensive surgery. There were no intraoperative complications during the transcorneal activation procedure in both groups. Postoperative complications occurred in 5% of patients in group I (two ciliochoroidal detachments) and in 8.3% of patients in group II (three hyphemas and two ciliochoroidal detachments). In both groups a statistically reliable decrease of intraocular pressure comparing to preoperative values was present during the whole period of follow-up (up to 3 years) and averaged 34.6%.

RESUMO

Tracing Schlemm's canal in the course of sinusotomy, added to multiple-modality treatment of open-angle glaucoma, helps improve the localization of the site of initially traced external wall of Schlemm's canal in gonioscopy and carry out laser trabeculopuncture at the site of the intervention with a lower level of laser energy, as well as reduce the possibility of complications and cicatrix of the trabecula and the adjacent tissues. A follow-up of 23 patients over 1.5 years has proved that the described intervention provides a good hypotensive effect and stabilization of the glaucomatous process in the overwhelming majority of patients.

RESUMO

Subscleral removal of the external wall of Schlemm's canal followed by laser trabeculo-puncture provides retention exposure of the intrascleral and trabecular levels without resorting to eyeball opening; intraocular reduction after such operation is gradual. A follow-up of up to 5 years of 134 patients have demonstrated that such combined intervention is low traumatic and ensures a stable hypotensive effect and stabilization of the glaucomatous process in the overwhelming majority of patients.

RESUMO

The authors have proved the possibility to prevent cicatrization between scleral laminae after antiglaucoma surgery in 16 rat experiments. They analyze late and remote results of 341 surgeries for open-angle glaucoma, 52 of these traditional trabeculectomies and 289 trabeculectomies with placing hemostatic viscose under the scleral flap. The results evidence the efficacy of viscose application: the incidence of complications early after surgery and in the late periods was essentially reduced and a more stable hypotensive effect achieved. The method is simple and available.

By means of an ophthalmological device "Iatagan", laser iridectomy, in personal modification, was made in 54 patients with chronic (in 40 cases--tuberculosis) uveitis in 67 eyes with predominantly organic changes in the anterior chamber angle. Before operation, ophthalmic hypertension was recorded in 55 eyes, of them glaucoma of different stages--in 43 (in 12 eyes intraocular pressure was normal). In immediate and remote terms, normalization of intraocular pressure was achieved in more than 60% of cases. Complications were rare and, as a rule, easily removed. There were no essential differences in effectiveness of laser iridectomy used in tuberculous and nontuberculous uveitis. The results obtained speak about the expediency of wide usage of laser iridectomy in chronic uveitis for both therapeutic and prophylactic purposes.
