Patient safety and professional liability are major concerns worldwide. Despite the pervasive influence of catastrophic malpractice payouts, little is known about the specific characteristics and the overall relevance and characteristics of these payouts, especially outside U.S. borders. Five hundred fifty claims led to a payout among 2,236 claims from January 1, 2004 to December 31, 2010, in Catalonia (Spain). We analyzed data on patient, provider, and claim characteristics. Accordingly to our sample, Spanish catastrophic payouts (SCP) were defined as payouts over &OV0556;200,000, which was found in 32 cases (5.8%). Diagnostic errors and patient death were not as relevant as previously reported. However, it is remarkable that the literature emanating from different countries shows similar trends besides contextual differences: patients suffering minor injuries are not likely to receive a catastrophic payout, catastrophic payouts are associated with patient age less than one; SCP are most associated with anesthesiology and resuscitation, general surgery, and obstetrics and gynecology; and SCP were more likely to occur when a case went to trial compared to when a case was settled out of court. Studies, such as this, provide a wider picture of the medical liability worldwide reality and helps avoiding isolated discourses.

RESUMO

Los juegos asfícticos son un comportamiento conocido entre los adolescentes, pero no descrito hasta el momento en la literatura científica española. Se trata de comportamientos que buscan una breve euforia debida a la disminución del flujo de oxígeno en el cerebro. Presentamos el caso de un adolescente de 15 años que falleció en una habitación de su domicilio habitual debido a una ahorcadura accidental durante un juego asfíctico. Se analizan las características del caso y se procede a una revisión de la literatura publicada. El juego de la asfixia es una actividad peligrosa, potencialmente mortal. Casos como el descrito pueden ayudar a que los pediatras y médicos en general conozcan tanto el fenómeno como la peligrosidad del mismo. Su conocimiento es importante para la prevención y detección precoz de comportamientos de riesgo entre los adolescentes (AU)

They are games which seek a brief euphoria due to the restriction oxygen flow into the brain. The case of a 15 year-old adolescent boy who died in a room due to accidental hanging while playing the choking game is presented. Case characteristics are described and literature reviewed. The choking game is a dangerous and potentially fatal activity. Cases like the one described may be helpful for paediatricians and physicians in recognising this phenomenon and its risks. Knowledge of this activity is important for the prevention and early detection of adolescents risk behaviours.

RESUMO

La muerte debida a sofocación por bolsa de plástico (SBP), reconoce diferentes etiologías medico-legales (suicida, homicida, accidental). Aunque en la actualidad es poco frecuente en nuestro medio, es un método tendente a aumentar su frecuencia, especialmente el de etiología suicida, según la bibliografía revisada. En el presente trabajo se describen dos casos de SBP con la finalidad de determinar similitudes y diferencias entre ellos, así como el grado de correspondencia con el cuadro descrito en los trabajos revisados para este tipo de muerte, haciendo especial referencia a los hallazgos encontrados en la región cervical (AU)


Death due to plastic bag suffocation (PBS) is associated with different manners of diying (suicide, homicide, accident). Currently it is infrequent in our area, but it is on the increase, especially as a method of suicide according to the bibliography revised. In this paper we describe two cases of deaths due to PBS with the aim of determining coincidences and differences between both. We also intend to determine the degree of correspondence with the findings described in the bibliography revised with several reference to the autopsy findings in the cervical area (AU)

RESUMO

La ahorcadura es el mecanismo suicida empleado con mayor frecuencia. Dentro de este tipo de muertes es una rareza que se produzca una decapitación completa, existiendo pocos casos documentados en la bibliografía. En este trabajo hacemos una revisión de los mecanismos determinantes de la decapitación por ahorcadura así como de las peculiaridades específicas del cuadro con el fin de realizar un análisis comparativo con nuestros hallazgos, siendo factores coincidentes: la caída libre, la resistencia y delgadez de la cuerda, y el peso importante del sujeto. Como factor no coincidente con los artículos revisados encontramos el nivel de decapitación occipito-atloideo, que aunque descrito como posibilidad en algún tratado, no lo hemos encontrado en los casos revisados. Por último se apuntan algunas sugerencias que consideramos útiles a la hora de analizar y evaluar este tipo de muertes (AU)
