RESUMO

It is a case report of young female that had diagnostic criteria of Systemic Lupus Erithematosus in activity, according American Rheumatology Association. The patient had fever, anemia, arthritis, cellular casts, positive LE cells, positive antinuclear antibody. She has evolved to bilateral pleural effusion and pericardic effusion that both have been initially attributed to lupus. Due to she has also maintained low fever, sudoresis, loss of weight and a persistent serositis, a thoracocenthesis with pleural biopsy has been done and the result of it has revealed granulomatous chronic pleuritis, diagnosticing pleural tuberculosis. A pericardiocenthesis has also been done. After six months of anti -tuberculosis therapy, there was a regression of radiologic imaging. Some concepts referring to tuberculosis and systemic lupus erythematosus are discussed, including symptoms, diagnosis and specific situations. It is emphasized the necessity of early diagnosis and appropriate management of tuberculosis disease in lupic patients, in areas where tuberculosis is endemic.

RESUMO

The author provides a critical analysis of systemic lupus erythematosus associated with tuberculosis. A brief review of the lupus-tuberculosis association is also given, and stresses that extra-pulmonary TB is the most usual form of TB in these cases. Other issues considered are the heat shock proteins of Mycobacterium tuberculosis HSP70KDa and HSP65KDa families and TLR2, TLR4, TLR9 that can be involved in interaction between bacilli antigen and host tissue causing autoimmune induction by lupus. The author concludes that early diagnosis and appropriate management are mandatory in SLE associated with TB, in areas where TB is endemic.

RESUMO

The authors report the case of a 13 year-old boy with disseminated tuberculosis evidenced by bilateral otomastoiditis associated to abdominal pain, ascites and apical alveolar pneumonia, prior history of contact with tuberculosis (TB) patients and radiology findings at mastoiditis, abdominal tract and thorax. Diagnosis was confirmed through positive culture for Mycobacterium tuberculosis from mastoiditis surgical drainage. After 6 months of anti-tuberculosis treatment there was remission of symptoms and regression of radiology findings.

RESUMO

The author proposes a comparison of tuberculids and extrapulmonary tuberculosis, highlighting the differences in definition, pathogenesis, clinical manifestations, diagnosis and treatment, proving that they are different expressions of Mycobacterium tuberculosis. The article further discusses updated information on tuberculids pathogenesis and diagnosis. A review of different presentations of extrapulmonary tuberculosis is also presented.

RESUMO

The authors report a case of a patient with a prior history of alcohol abuse who developed nodules and ulcerated skin lesions on his trunk and lower extremities along the line of the lymphatic draining area. The patient's X-ray showed reticular nodular intersticial infiltrates at the lung bases. There was a positive culture of supraclavicular lymph node for Sporothrix schenckii. After specific treatment using mainly potassium iodide, there was regression of cutaneous lesions and lung infiltrates. The authors present a discussion on the deseases' forms of presentation, highlighting the lung involvement ans further discuss the diagnosis and treatment of sporotrichosis.

RESUMO

In this article, the authors discuss briefly the leptospirosis, emphasizing mainly the pulmonary form of disease. The authors review pathology, clinical findings, imaging and broncoscopy diagnosis, treatment of pulmonary leptospirosis. It is also remembered about early clinics and radiology diagnosis to start therapeutics. The authors conclude that pulmonary form of disease must always be remembered and considered as cause and differential diagnosis of Diffuse Alveolar Hemorrhage and Adult Respiratory Distress Syndrome.
