RESUMO

The results of treatment of 691 patients (70.8% patients were older than 60 years) with pancreatoduodenal cancer are analyzed. The obstructive jaundice was diagnosed in 95.8% patients, 82% patients had cancer stage III, IV. Ultrasonic diagnosis is expedient as a screening method. Percutaneous transhepatic cholecystocholangiostomy or tumor stenting is expedient as a preoperative method of biliary tract decompression. Radical operation--pancreatoduodenal resection--was possible in 17.5% patients. Lethality in this operation reduced from 31.9% to 12.2%. Mean survival was 20.8 months--3 times longer than after creation of palliative biliodigestive anastomosis.

RESUMO

Under analysis were results of 22 pancreatoduodenal resections when the Wirsung duct was drained outside. Giving up the previously used pancreaticojejunal+ anastomosis improved the immediate outcomes of the operative treatment although 5 patients died during the nearest period (within 12 days): three of them died of pancreonecrosis. Twelve patients were followed up, 7 of them died, 3 of the rest 5 patients had satisfactory results, 2 patients had good results.

RESUMO

An analysis of 685 operations for complex inguinal hernias has been made. 486 patients were examined within the terms from 1 to 14 years, recurrences being noted in 48 patients (9,87%). The authors recommend a method of radical operation for strengthening the posterior wall of the inguinal canal by an inferior graft of aponeurosis. This method is followed by only 4,44% of recurrences.
