As patient longevity continues to improve, the rate of lower limb revision arthroplasties will continue to increase as patients outlive the expiration of their implants. With continued bone loss and reduced stability, there is a limit to the number of revision operations that can be performed. Total femoral arthroplasty (TFA) is an increasingly popular limb-salvaging alternative that can restore some degree of daily function to patients. This report presents a 73-year-old male with multiple right lower-limb operations following two extreme motorcycle accidents in the last 22 years. Due to continued pain and poor femoral bone stock following multiple total knee arthroplasty (TKA) revisions, a TFA was performed. The procedure was successful and post-operative expectations were met despite setbacks in immediate rehabilitation. Overall, TFA is an effective alternative to lower limb amputation in the setting of aseptic, non-oncologic bone loss following multiple knee revisions. However, careful management is necessary to reduce the risk of infection and other complications.

The body schema is a much discussed aspect of body awareness. Although there is still no single definition, there is widespread consensus that the body schema is responsible for movement and interaction with the environment. It usually remains outside of active consciousness. There are only few investigations on influences on the body schema and none of them investigated feeling of satiety or hunger. Thirty-two healthy women were investigated twice, one time sat and the other time hungry. To measure the body schema, we used a door-like-aperture and compared the critical aperture-to-shoulder-ratio (cA/S). A cover story was used to ensure that the unconscious body schema has been measured. We found a significantly higher cA/S for satiety compared to hungry, which indicates that during satiety participants rotate their shoulders for relatively larger door compared to hunger, unconsciously estimating their body size to be larger. We showed that even a moderate rated feeling of hunger or satiety leads to an adjustment in body-scaled action and consequently also an adaptation of body schema. It suggests that, in addition to the visual-spatial and the proprioceptive representation, somatic information can also be relevant for the body schema.

Despite the many benefits of total knee arthroplasty (TKA) in the treatment of degenerative arthritis, infection of the total knee prosthesis presents a dangerous post-operative complication affecting 0.5-1.9% of all cases. Infection after the first three post-operative weeks is treated with either one or two-stage revision operations involving the removal of all prosthetic components. Two-stage revision operations are more commonly used and involve the removal of prosthetic components followed by the implantation of a cement mold infused with antibiotics (antibiotic spacer) as well as systemic antibiotic treatment for four to six weeks before prosthetic reimplantation. This case report details a TKA revision in a patient with osteoarthritis of the knee. The patient presented with an elevated erythrocyte sedimentation rate, C-reactive protein, and white blood cell count nearly two years after the primary operation and was found to have an infected total knee prosthetic. A two-stage revision was planned but due to scheduling disruption by the coronavirus disease 2019 pandemic, the second stage of the operation was delayed until 12 months after the stage one operation. The patient ambulated without pain on an antibiotic spacer for 12 months, providing information about the long-term use of spacers. This case also offers a look at a potential benefit to one-stage operations, which have been shown in the literature to have similar outcomes as two-stage operations. The patient had a medical history of psoriasis and immunosuppressive treatment with methotrexate, two risk factors for prosthetic joint infection, and may have benefited from prophylactic antibiotic therapy extending beyond the perioperative period. The goal of this case report is to detail the prolonged use of an antibiotic spacer, examine the risks and benefits of one and two-stage total knee revisions, and discuss prophylactic antibiotic use in high-risk patients following TKA.

OBJECTIVE: Besides all visible somatic manifestations, distorted body representation is a key symptom for anorexia nervosa (AN). Body representation can be divided into a conscious component, body image, and an unconscious action-related one, body schema. As behavioral studies already explored the impact of a distorted body image in AN, we aimed to explore whether distortion also extends into unconscious body schema. This study is the first with an unbiased measurement of the body schema in a homogeneous sample of AN patients. METHOD: Twenty-three patients diagnosed with AN and 23 healthy controls (HC) walked through a door like aperture varying in width. Door width was based on participants shoulder width and ranged from an aperture-to-shoulder-ratio of (A/S) 0.9 to 1.45. Shoulder rotation was measured as indication of perceived body width. To measure the unconscious body schema, we used a cover story pretending to investigate the influence of change of position on retention memory. RESULTS: We found a significantly higher critical A/S for AN than HC, which indicates that AN patients rotate their shoulders for relatively larger door widths than HC, thus unconsciously estimating their body size to be larger than in reality. Additionally, we found a correlation between negative body attribution and overestimation of bodily dimensions. DISCUSSION: As stated by the "allocentric-lock"-hypothesis, AN patients might be locked to a stored representation of their body that cannot be updated and remains at pre-AN conditions. We suggest future AN-therapy to counter body schema alterations by combining cognitive behavioral therapy and virtual reality therapy.

Total knee arthroplasty (TKA) systems are designed to maximize the longevity of the implant. However, an unusually high rate of aseptic tibial tray loosening was observed in the DePuy ATTUNE® total knee arthroplasty system (DePuy Synthes, Warsaw, USA). Affected patients noted symptoms of instability and reproducible pain at the bone-implant interface. However, there was no radiographic evidence of lucency or implant failure. Intra-operatively, the tibial component was grossly loose and lacked adhered cement. We hypothesize that the loosening is due to the smooth architecture of the undersurface of the tibial component, lacking either grit blasting or porous coating.

A simple surgical procedure is not without the risk of complications and when removing hardware from the bone, such as a previous patella fracture hardware, the surgeon must be well aware of this potential. Here we present the case of a 71-year-old male who presented for removal of retained hardware of united right patella fracture with overlying skin complications. Surgical intervention was uneventful. On post-operative day 1, the patient suffered a fall while using the restroom unassisted with a subsequent large amount of bleeding from the incision site. X-rays demonstrated a new displaced inferior pole patellar fracture. The patient returned to the operating room for debridement and repair of this new patellar fracture with primary closure. Intraoperative cultures of initial operative site were positive for Corynebacterium and Staphylococcus epidermidis. The patient was then started on intravenous Vancomycin based on culture sensitivities. The patella fixation/repair was protected with a knee immobilizer as there was increased risk of falls due to his age/underlying medical condition. This case discusses recommendations and guidelines for preventing and managing these various postoperative complications.

Epidemiological data on dementia and cognitive impairment are scarce in South America. In Argentina, no dementia/cognitive impairment population-based epidemiological studies are available. The Ceibo Study is a population based epidemiological study of dementia and cognitive impairment in individuals over 60 to be conducted. The present paper reports the results of the pilot phase (survey of cognitive impairment) conducted in Cañuelas (province of Buenos Aires). Methods: In a door-to-door survey, trained high school students evaluated 1453 individuals aged 60 years and over in one day using a demographic data and risk factors questionnaire, the Mini-Mental State Examination (MMSE) and the 15-item Geriatric Depression Scale (GDS). Results: Mean age of the individuals was 70.9 (±7.5) years, 61.4% were women, mean schooling was 5.5 (±3.5) years. Mean MMSE score was 24.5 (±4.7) and mean GDS 3.1 (±2.7). Risk factors of higher prevalence in the population under study were: hypertension (40.6%), smoking (35.1%), alcohol consumption (32.8%), high cholesterol (16.1%), diabetes (12.5%), cranial trauma with loss of consciousness (12.5%), 7 points or moreon the GDS (11.7%). Prevalence of cognitive impairment for the whole sample was 23%, and 16.9% in subjects aged 60-69, 23.3% in 70-79 and 42.5% in subjects aged 80 or over . A significant correlation of cognitive impairment with age, functional illiteracy, cranial trauma, high blood pressure, inactivity and depression was found. Conclusion: In this pilot study, the prevalence of cognitive impairment was comparable with previous international studies.

O presente artigo reporta os resultados da fase piloto (pesquisa de comprometimento cognitivo) conduzido em Cañuelas (província de Buenos Aires). Angiotensin II receptor blockers (ARBs) are used for the treatment of patients with heart failure and hypertension. Yet their safety has been questioned by some who observed delayed cardiac healing and scar thinning after myocardial infarction (MI). To clarify potential efficacy and safety of ARBs, we administered Azilsartan medoxomil, a prodrug of an ARB (Takeda Pharmaceutical Company Limited), assessed cardiac fibrosis (hydroxyproline content), left ventricular (LV) wall thickness (premortem echocardiography and caliper measurement at necropsy), and LV mass and cardiac function with high-resolution ultrasound in mice with either surgically induced LV pressure overload (aortic banding) or acute MI. Drug-treated aortic-banded mice exhibited less LV wall thickness, hypertrophy, and dilation compared with that exhibited by controls. Survival in drug-treated MI mice was greater though not significantly. Drug-treated mice with acute MI exhibited less cardiomyocyte injury reflected by LV creatine kinase content and less LV hypertrophy and dilation. Thus, Azilsartan exerted favorable biological effects on the hearts of mice subjected to LV pressure overload or MI without compromising survival consistent with its potential utility and tolerability in patients with analogous conditions.

Gender-dependent differences in echocardiographic characteristics of the heart have been thought to exist in human subjects, but comparisons have been difficult because of potential confounders such as differential prevalence of occult disease. Little data are available characterizing echocardiographic descriptors in normal experimental animals despite wide use of this modality. Accordingly, we characterized systolic and diastolic criteria of cardiac function echocardiographically in C57BL6 and BalbC age-matched mice to determine whether results in males and females differed. Left ventricular dimensions in absolute terms were lower, transmitral valve A-waves smaller and isovolumic relaxation time greater in females. When dimensional data were normalized for body weight, differences persisted but were directionally opposite. Thus, detection of abnormalities in cardiac function echocardiographically in various strains of mice requires establishing reference values that are gender as well as strain specific. Furthermore, the results suggest that gender-dependent differences are likely to be present in other species, including humans, and accordingly, that normal values should be established separately for males and females.

This study analyzed 399 Balance femoral stems to determine whether immediate full weight bearing following total hip arthroplasty affected radiographic osseointegration. Postoperatively, all patients were permitted unlimited weight bearing. Findings demonstrated osseointegration was successful in 99.5% of femoral components. Of these, 0.5% subsided in the first 6 postoperative weeks and then osseointegrated; 0.5% failed to osseointegrate. Patients with Dorr C bone had a significantly increased rate of subsidence (P=.006). These findings indicate the Balance stem reliably osseointegrates without subsidence in patients with Dorr A or B bone, despite immediate full weight bearing.

OBJECTIVES: Both tirofiban and eptifibatide release rapidly from glycoprotein IIb-IIIa but have different dissociation constants (KD of tirofiban=15 nmol/l, of eptifibatide=120 nmol/l). Binding of fibrinogen to glycoprotein IIb-IIIa is biphasic, forming an initial reversible complex (KD=155-180 nmol/l) and a second more stable complex (KD=20-70 nmol/l). Diabetes is known to alter platelet function. To determine the influence of affinity on inhibitory effects in blood from patients with (n=20) and without (n=20) diabetes mellitus, we characterized the extent of inhibition as a function of time. METHODS: Blood was added to reaction tubes containing tirofiban 100 ng/ml or eptifibatide 1.7 microg/ml (concentrations previously defined to be optimal) plus a platelet agonist (1 micromol/l adenosine diphosphate or 25 micromol/l thrombin receptor agonist peptide), and fluorochrome-labeled fibrinogen before analysis by flow cytometry. RESULTS: The extent of inhibition early on (30 s to 3 min) was similar (>85%) with either agent in blood from those with and without diabetes mellitus, whereas the extent of inhibition 10-15 min later was maintained more effectively with tirofiban than with eptifibatide (difference in slope P<0.01). After 15 min, the extent of inhibition in response to adenosine diphosphate in those with diabetes mellitus was 95+/-6% for tirofiban and 70+/-15% for eptifibatide (P<0.001); in those without diabetes mellitus, it was 91+/-9% for tirofiban and 73+/-19% for eptifibatide (P<0.001). CONCLUSION: For glycoprotein IIb-IIIa antagonists with a rapid rate of release, the biphasic binding of fibrinogen influences to a similar extent their ability to maintain inhibitory effects in blood from patients with and without diabetes mellitus.

Tirofiban and eptifibatide dissociate rapidly from glycoprotein IIb-IIIa but have different dissociation constants (KD of tirofiban = 15 nmol/L, that of eptifibatide = 120 nmol/L). Binding of fibrinogen to glycoprotein IIb-IIIa is biphasic, forming an initial reversible complex (KD = 155-180 nmol/L) and a second more stable complex (KD = 20-70 nmol/L). To test whether a comparable extent of inhibition would be maintained by pharmacologic antagonists that exhibit a rapid rate of release, blood from 26 patients with symptomatic coronary artery disease was added to reaction tubes containing a concentration of either agent that had been shown to achieve optimal inhibition (for tirofiban 100 ng/mL, for eptifibatide 1.7 microg/mL) plus a platelet agonist (1 microM adenosine diphosphate [ADP] or 25 microM thrombin receptor agonist peptide [TRAP]), and fluorochrome labeled fibrinogen before analysis by flow cytometry. The extent of inhibition early on (30 seconds to 3 minutes) was similar. By contrast, the extent of inhibition 10 to 15 minutes later was maintained more effectively with tirofiban than eptifibatide (difference in slope P < 0.01). The differences are consistent with the biphasic binding of fibrinogen to GP IIb-IIIa. The clinical implications of this observation merit evaluation to potentially improve care of patients and to guide future drug development.
